This document introduces and explains common implementation concepts and frameworks relevant to healthcare epidemiology and infection prevention and control and can serve as a stand-alone guide or be paired with the “SHEA/IDSA/APIC Compendium of Strategies to Prevent Healthcare-Associated Infections in Acute Care Hospitals: 2022 Updates,” which contain technical implementation guidance for specific healthcare-associated infections. This Compendium article focuses on broad behavioral and socio-adaptive concepts and suggests ways that infection prevention and control teams, healthcare epidemiologists, infection preventionists, and specialty groups may utilize them to deliver high-quality care. Implementation concepts, frameworks, and models can help bridge the “knowing-doing” gap, a term used to describe why practices in healthcare may diverge from those recommended according to evidence. It aims to guide the reader to think about implementation and to find resources suited for a specific setting and circumstances by describing strategies for implementation, including determinants and measurement, as well as the conceptual models and frameworks: 4Es, Behavior Change Wheel, CUSP, European and Mixed Methods, Getting to Outcomes, Model for Improvement, RE-AIM, REP, and Theoretical Domains.

Development of robust concrete mixes with a lower environmental impact is challenging due to natural variability in constituent materials and a multitude of possible combinations of mix proportions. Making reliable property predictions with machine learning can facilitate performance-based specification of concrete, reducing material inefficiencies and improving the sustainability of concrete construction. In this work, we develop a machine learning algorithm that can utilize intermediate target variables and their associated noise to predict the final target variable. We apply the methodology to specify a concrete mix that has high resistance to carbonation, and another concrete mix that has low environmental impact. Both mixes also fulfill targets on the strength, density, and cost. The specified mixes are experimentally validated against their predictions. Our generic methodology enables the exploitation of noise in machine learning, which has a broad range of applications in structural engineering and beyond.

Impact of a Mandatory Infection Control Education Program on Nosocomial Acquisition of Methicillin-Resistant Staphylococcus aureus

Objective.
To assess the impact of an institution-wide infection control education program on the rate of transmission of methicillin-resistant Staphylococcus aureus (MRSA).
Design.Before-and-after study.
Setting.A 472-bed, urban, university-affiliated hospital.
Intervention.During the period March-May 2004, all hospital staff completed a mandatory infection control education program, including the receipt of hospital-specific MRSA data and case-based practice with additional precautions.
Outcome Measure.The rate of nosocomial MRSA acquisition was calculated as the number of cases of nosocomial MRSA acquisition per 100 days that a person with MRSA colonization or infection detected at admission is present in the hospital (“admission MRSA” exposure-days) for 3 time periods: June 2002-February 2003 (before the Toronto outbreak of severe acute respiratory syndrome [SARS]), June 2003-February 2004 (after the outbreak of SARS), and June 2004-February 2005 (after education). A case of nosocomial acquisition of MRSA colonization or infection represented a patient first identified as colonized or infected more than 72 hours after admission or at admission after a previous hospitalization.
Results.The rate of nosocomial acquisition of MRSA colonization or infection was 8.8 cases per 100 admission MRSA exposure-days for the period before SARS, 3.8 cases per 100 admission MRSA exposure-days for the period after SARS (P < .001 for before SARS vs after SARS), and 1.9 cases per 100 admission MRSA exposure-days for the period after education (P = .02 for after education vs before education). The volume of alcohol-based handrub purchased was apparently stable, with 4,010 L during fiscal year 2003-2004 (April 2003-March 2004) compared with 3,780 L during fiscal year 2004—2005. The observed rate of compliance with hand washing did not change significantly (40.9% during education vs 44.2% after education; P = .23). The total number of patients screened for MRSA colonization was not different in the 3 periods.
Conclusions.The rate of nosocomial acquisition of MRSA colonization or infection decreased after SARS and was further reduced in association with a hospital-wide education program.

Levels of I were determined in selected foods and dietary supplements, and in samples of the British ‘Total Diet’. The average concentration of I in British milk collected in thirteen areas on four occasions during 1990 and 1991 was 150 μg/kg (range 40–310 μg/kg), compared with 230 μg/kg in 1977–79. No difference was found between skimmed and whole milk. Winter milk contained 210 μg/kg while summer milk contained 90 μg/kg. Regional differences were less pronounced than seasonal differences. Levels in fish and fish products were between 110 and 3280 μg/kg. Edible seaweed contained I levels of between 4300 and 2660000 μg/kg. Kelp-based dietary supplements contained I at levels that would result in a median intake of 1000 μg if the manufacturers' recommended maximum daily dose of the supplement was taken, while other I-containing supplements contained a median level of 104 μg in the manufacturers' maximum recommended daily dose. Intake of I, as estimated from the Total Diet Study, was 173 μg/d in 1985 (277 μg if samples with very high I contents were included) and 166 μg/d in 1991. These levels are above the UK reference nutrient intake of 140 μg/d for adults but well below the Joint Expert Committee on Food Additives provisional maximum tolerable intake of 1000 μg/d.

Reports have been accumulating in the literature attesting to the failure of patients, particularly out-patients, to take medication as prescribed (Stewart and Cluff, 1972; Moulding et al., 1970). The incidence of these failures, which may include anything from missing occasional doses to taking no medication at all, has been reported to range from 25–59 Per cent. In addition, it has not been possible to identify those patients who will have the highest failure rates (Blackwell, 1972). While most of the studies have involved medical patients, it is probably legitimate to assume that schizophrenic patients are at least as unreliable in taking medication as the medical patients. Thus, in one study of 125 psychiatric patients, Willcox et al. (1965) found that 48 per cent took no medication at all.
