Exploring women’s sexuality during pregnancy : sociodemographic, culturel and relational characteristics

Introduction
Pregnancy is a period of important physical, psychological and hormonal changes. All these changes affect daily life, relationships with others, the relationship with the body and particularly the intimate and sexual aspect within the couple.
Objectivesto describe the sociodemographic and clinical profile of pregnant women, explore their perceptions and practices regarding sexuality during an uncomplicated pregnancy and to assess the impact of pregnancy on the couple’s sexuality.
MethodsThis is a cross-sectional, descriptive study conducted among pregnant women followed at the outpatient prenatal and women in immediate postpartum who gave birth in the obstetrics department of EPS Tahar Sfar Mahdia, during a period of 6 months from 01 September 2019 to 28 February 2020. The evaluation of women’s sex life during pregnancy was conducted using a predefined questionnaire and the validated scale translated into Arabic “Arabic Female Sexual Function Index (ArFSFI)”.
ResultsA total of 110 patients were included. The average age of the patients was 30.2 +/- 4.98 years. In 60 % of the cases, the patients were from rural areas. More than half (55%) of our patients had a primary education and were housewives (64%). They were married for love in 59% and the marriage was arranged for 41% of the women. More than 2/3 of our patients (71%) reported a good marital relationship. In relation to the current pregnancy: the majority of patients (70%) were in immediate post partum, the pregnancy was desired (98.1%), well experienced (62%).
For most of the patients (58,2%), sexual intercourse was possible and without risk during the whole pregnancy, but some women thought about the risk of abortion or premature delivery (37.3%), the risk of metrorrhagia (10%) and the risk of infection (6.4%). Most patients (87.2%) reported a decrease in the frequency of sexual intercourse during pregnancy. Most women reported a decrease in desire (51.8%), and sexual satisfaction (55.6%) during their pregnancies. For partner sexuality, desire was stationary for 89% and sexual satisfaction was decreased in 61.1% of cases. Almost half of our patients (42.7%) talked about sexuality : to the husband in 74% of cases, followed by social networks in 58% of cases. The motivation for talking about sexuality was in the majority of cases (61.7%) secondary to a complication (metrorrhagia/ Threatned premature labor..).The mean FSFI total score was 25.3 ± 2.8. More than 2/3 of the patients (70%) had a score <26.55 indicating female sexual dysfunction.
ConclusionsPerforming sexual acts during pregnancy has very low risk, contraindications are rare and specific. It is therefore important that patients be informed and reassured on this subject. Information on sexuality during pregnancy should be systematically provided to women during their pregnancy follow-up.

Depression in the Elderly with Chronic Medical Illness

Background
There is a bi-directional relationship between depression and chronic medical disorders.
AimsThe objectives of our study were to measure the prevalence of depression in the elderly with chronic medical illness in primary care and to determine the socio-demographic and clinical factors related to depression.
MethodsWe conducted a descriptive and analytical cross-sectional study of patients aged over 65, followed at the outpatient chronic diseases in Oudhref's district hospital (south of Tunisia) during the month of September 2014. We used two instruments: the activity of daily living (ADL) to determine the degree of autonomy and the geriatric depression scale (GDS) validated in Tunisia.
ResultsAt the end of our investigation, 100 chronic disease patients met inclusion criteria. The average age of our population was 75 years. Prevalence of depression was 48%. The most frequent chronic pathology was hypertension (79%), followed by diabetes (70%). In analytical study, we noted no correlation between depression and socio-demographic variables such as age, sex and marital status. Regarding clinical variables, depression was significantly more frequent in patients with sensory impairments (82% vs 18%, P = 0.017), dependent (80% vs 20%, P = 0.002). Regarding chronic disease, depression was significantly more frequent in patients with respiratory disease (80% vs 20%, P = 0.033), a higher number of comorbidities (P = 0.005), who were hospitalised at least once (P = 0.015).
ConclusionDepression is common in elderly with a chronic disease. Using screening instruments for major depression by primary care clinicians will help to detect depression.

On the influence of the exciton-blocking layer on the organic multilayer cells properties

The performances of organic photovoltaic cells based on the layer couple electron donor/electron acceptor (ED/EA) are significantly improved when an exciton blocking layer (EBL) is inserted between the organic acceptor and the cathode. A new material, the (Z)-5-(4-chlorobenzylidene)-3-(2-ethoxyphenyl)-2-thioxothiazolidin-4-one, that we called (CBBTZ), has been synthesized, characterized and probed as EBL. The energy levels corresponding to the highest occupied molecular orbital (HOMO) and the lowest unoccupied molecular orbital (LUMO) of the CBBTZ have been determined from the first oxidation and reduction potential respectively, using cyclic voltametric (CV) measurements. From CV curves, CBBTZ in dichloromethane showed a one electron reversible reduction and oxidation waves. The values of its HOMO and LUMO have been estimated to be 6.42 eV and 3.42 eV respectively. Such values show that CBBTZ could be probed as EBL in organic solar cells based on the ED/EA couple copper phthalocyanine(CuPc)/fullerene (C60). The photovoltaic solar cells have been obtained by sequential deposition under vacuum of the different films. The different thin film thicknesses were measured in situ by a quartz monitor. Multilayer solar cells ITO/Au/CuPc/C60/EBL/Al have been probed, where EBL is the aluminium tris(8-hydroxyquinoline) (Alq3), the CBBTZ, the 2-(4-byphenylyl)-5-(4-tert-buthylphenyl)-1,3,4-oxadiazole (butyl PBD) or the bathocuproine (BCP). The optimum film thickness is 8–9 nm whatever the EBL used. When obtained in the same run, the averaged efficiency of the cells using the CBBTZ is of the same order of magnitude than that obtained with BCP and higher than that achieved with Alq3 or butyl PBD. It is shown by XPS study that some aluminium of the cathode is present in the buffer layer. This aluminium could justify the ability of the electrons to cross the insulating exciton blocking layer thick of 9 nm.
