Evaluation of a complex intervention for prisoners with common mental health problems, near to and after release: the Engager randomised controlled trial

Background
Many male prisoners have significant mental health problems, including anxiety and depression. High proportions struggle with homelessness and substance misuse.
AimsThis study aims to evaluate whether the Engager intervention improves mental health outcomes following release.
MethodThe design is a parallel randomised superiority trial that was conducted in the North West and South West of England (ISRCTN11707331). Men serving a prison sentence of 2 years or less were individually allocated 1:1 to either the intervention (Engager plus usual care) or usual care alone. Engager included psychological and practical support in prison, on release and for 3–5 months in the community. The primary outcome was the Clinical Outcomes in Routine Evaluation Outcome Measure (CORE-OM), 6 months after release. Primary analysis compared groups based on intention-to-treat (ITT).
ResultsIn total, 280 men were randomised out of the 396 who were potentially eligible and agreed to participate; 105 did not meet the mental health inclusion criteria. There was no mean difference in the ITT complete case analysis between groups (92 in each arm) for change in the CORE-OM score (1.1, 95% CI –1.1 to 3.2, P = 0.325) or secondary analyses. There were no consistent clinically significant between-group differences for secondary outcomes. Full delivery was not achieved, with 77% (108/140) receiving community-based contact.
ConclusionsEngager is the first trial of a collaborative care intervention adapted for prison leavers. The intervention was not shown to be effective using standard outcome measures. Further testing of different support strategies for prison with mental health problems is needed.
Anti-tumour necrosis factor therapy is associated with certain subtypes of chronic rhinosinusitis

Background:
Chronic rhinosinusitis has many risk factors; however, the effect of anti-tumour necrosis factor therapy has not been investigated in depth. Our experience points to a detrimental clinical effect in overall prevalence of chronic rhinosinusitis, despite its benefit in certain subtypes.
Method:A telephone survey was performed to parallel the findings of the Global Allergy and Asthma European Network chronic rhinosinusitis screening survey. This was itself based on the widely recognised European Position Paper on Rhinosinusitis and Nasal Polyps criteria.
Results:A total of 120 patients responded to the survey. The prevalence of chronic rhinosinusitis in the anti-tumour necrosis factor therapy population was 20 per cent (95 per cent confidence interval = 12.84–27.16). When compared using a chi-square test, for a two-by-two contingency table, this finding was significant against the prevalence recorded in the normal population.
Conclusion:This is the first observational study indicating increased prevalence of chronic rhinosinusitis in patients treated with anti-tumour necrosis factor therapy. These clinical findings require investigation in greater depth to clarify the nature of pathologies currently diagnosed and treated as chronic rhinosinusitis.

That parasitic worms may induce bacterial invasion from the intestine has frequently been asserted in explanation of the sporadic occurrence of some bacterial and virus diseases of animals. Weinberg, in 1907, gave an extensive review of the literature dealing with inflammatory conditions of the intestine, due to bacterial invasion associated with parasitic worms. He also gave an account of some original clinical observations. He concluded that, by biting and boring into the intestinal wall, or by wounding it with hooks and suckers, the majority of helminths favour the penetration of microbes, these being carried on the exterior and in the intestine of the worms. In one case, where a fatal septicaemia was associated with the presence of a single individual Trichocephalus, the author's conclusion, that even isolated worms are very dangerous, appears to be rather strained. Records of clinical observations of this kind frequently appear in the literature, but, as a general rule, the evidence showing the agency of the worm is very slender and amounts to little more than the observation of the presence of worms in animals suffering from some bacterial disease.

The object of this project was the evaluation of microwaves as a non-invasive tool for determining the thicknesses of the fat layers directly beneath the hides of beef cattle. The motive for the project is the elimination of the cost of feeding cattle beyond the point at which the muscle is optimally marbled. The nature of the animals and the conditions for operation require a simple, rugged, non-invasive system. Modified open-ended microwave S-band coaxial cavities were applied as contact radiators to the surface of a three-layer sample composed of lean meat and fat tissue, which simulated the configuration of hide, fat and muscle on the outside of an animal. The lean and fat layers loaded the cavity, affecting the resonance frequency, bandwidth, and center-frequency reflection coefficient. Measurements were made with a network analyzer. An exact analysis of the microwave circuit has not been possible, but the in vitro laboratory tests show that a system based on this device can be used to measure subcutaneous fat layer thicknesses up to 16 mm beneath hides up to 16 mm thick.
