V. bicolor, V. trifolia s. str. and V. rotundifolia are part of a species complex that has recorded medicinal use in the Philippines. We assembled the first chloroplast genome of V. bicolor through next-generation sequencing and compared this to earlier established chloroplast genomes of V. trifolia s. str. and V. rotundifolia to provide additional insights into their genotypic differences. To ensure the continued utility of the research outputs in case of future taxonomic revisions, we characterized the morphology of PBN 2018-674, the reference germplasm utilized to generate the plastome. The complete chloroplast genome sequence of V. bicolor was 154,460 bp long with 131 coding genes comprising 87 mRNA genes, 36 tRNA genes and 8 rRNA genes. Using a separate accession from a different type locality, an identical chloroplast genome was equally established, indicating its conserved nature. When compared to V. trifolia s. str. and V. rotundifolia, slight variations were observed in genome features between these species; however, single nucleotide polymorphisms were exhibited in 13 protein-coding genes that often have a conserved nature. A phylogenetic analysis of the assembled genome, together with 12 other Lamiaceae species, exhibited high bootstrap support (>88%) within the species complex, and associated V. trifolia as the closest relative of V. bicolor. The identified variations in the plastomes can be utilized as markers that could distinguish the three closely related genotypes which can help the Philippine herbal industry provide a more stable source of quality herbal medicines.

Depression and anxiety disorders are the leading cause of sickness absence and long-term work incapacity in most developed countries. The present study aimed to carry out a systematic meta-review examining the effectiveness of workplace mental health interventions, defined as any intervention that a workplace may either initiate or facilitate that aims to prevent, treat or rehabilitate a worker with a diagnosis of depression, anxiety or both. Relevant reviews were identified via a detailed systematic search of academic and grey literature databases. All articles were subjected to a rigorous quality appraisal using the AMSTAR assessment. Of the 5179 articles identified, 140 studies met the inclusion criteria, of which 20 were deemed to be of moderate or high quality. Together, these reviews analysed 481 primary research studies. Moderate evidence was identified for two primary prevention interventions; enhancing employee control and promoting physical activity. Stronger evidence was found for CBT-based stress management although less evidence was found for other secondary prevention interventions, such as counselling. Strong evidence was also found against the routine use of debriefing following trauma. Tertiary interventions with a specific focus on work, such as exposure therapy and CBT-based and problem-focused return-to-work programmes, had a strong evidence base for improving symptomology and a moderate evidence base for improving occupational outcomes. Overall, these findings demonstrate there are empirically supported interventions that workplaces can utilize to aid in the prevention of common mental illness as well as facilitating the recovery of employees diagnosed with depression and/or anxiety.

Background
Cannabis use has been reported to be associated with an earlier onset of symptoms in patients with first-episode psychosis, and a worse outcome in those who continue to take cannabis. In general, studies have concentrated on symptoms of psychosis rather than mania. In this study, using a longitudinal design in a large naturalistic cohort of patients with first-episode psychosis, we investigated the relationship between cannabis use, age of presentation to services, daily functioning, and positive, negative and manic symptoms.
MethodClinical data on 502 patients with first-episode psychosis were collected using the MiData audit database from seven London-based Early Intervention in psychosis teams. Individuals were assessed at two time points – at entry to the service and after 1 year. On each occasion, the Positive and Negative Syndrome Scale, Young Mania Rating Scale and Global Assessment of Functioning Scale disability subscale were rated. At both time points, the use of cannabis and other drugs of abuse in the 6 months preceding each assessment was recorded.
ResultsLevel of cannabis use was associated with a younger age at presentation, and manic symptoms and conceptual disorganization, but not with delusions, hallucinations, negative symptoms or daily functioning. Cannabis users who reduced or stopped their use following contact with services had the greatest improvement in symptoms at 1 year compared with continued users and non-users. Continued users remained more symptomatic than non-users at follow-up.
ConclusionsEffective interventions for reducing cannabis use may yield significant health benefits for patients with first-episode psychosis.

Background
In first-episode schizophrenia, longer duration of untreated psychosis (DUP) predicts poorer outcomes.
AimsTo address whether the relationship between DUP and outcome is a direct causal one or the result of association between symptoms and/or cognitive functioning and social functioning at the same time point.
MethodSymptoms, social function and cognitive function were assessed in 98 patients with first-episode schizphrenia at presentation and 1 year later.
ResultsThere was no significant clinical difference between participants with short and long DUP at presentation. Linear regression analyses revealed that longer DUP significantly predicted more severe positive and negative symptoms and poorer social function at 1 year, independent of scores at presentation. Path analyses revealed independent direct relationships between DUP and social function, core negative symptoms and positive symptoms. There was no significant association between DUP and cognition.
ConclusionsLonger DUP predicts poor social function independently of symptoms. The findings underline the importance of taking account of the phenomenological overlap between measures of negative symptoms and social function when investigating the effects of DUP.

Background
The impact of co-morbid substance use in first-episode schizophrenia has not been fully explored.
MethodThis naturalistic follow-up of a cohort of 152 people with first-episode schizophrenia examined substance use and clinical outcome in terms of symptoms and social and neuropsychological function.
ResultsData were collected on 85 (56%) of the patient cohort after a median period of 14 months. Over the follow-up period, the proportion of smokers rose from 60% at baseline to 64%. While 30% reported lifetime problem drinking of alcohol at baseline, only 15% had problem drinking at follow-up. Furthermore, while at baseline 63% reported lifetime cannabis use and 32% were currently using the drug, by the follow-up assessment the latter figure had fallen to 18.5%. At follow-up, persistent substance users had significantly more severe positive and depressive symptoms and greater overall severity of illness. A report of no lifetime substance use at baseline was associated with greater improvement in spatial working memory (SWM) at follow-up.
ConclusionsPast substance use may impede recovery of SWM performance in people with schizophrenia in the year or so following first presentation to psychiatric services. The prevalence of substance use other than tobacco tends to diminish over this period, in the absence of specific interventions. Persistent substance use in first-episode schizophrenia is associated with more severe positive and depressive symptoms but not negative symptoms, and should be a target for specific treatment intervention.

Background
Substance use may be a risk factor for the onset of schizophrenia.
AimsTo examine the association between substance use and age at onset in a UK, inner-city sample of people with recent-onset schizophrenia.
MethodThe study sample consisted of 152 people recruited to the West London First-Episode Schizophrenia Study. Self-reported data on drug and alcohol use, as well as information on age at onset of psychosis, were collected. Mental state, cognition (IQ, memory and executive function) and social function were also assessed.
ResultsIn total, 60% of the participants were smokers, 27% reported a history of problems with alcohol use, 35% reported current substance use (not including alcohol), and 68% reported lifetime substance use (cannabis and psychostimulants were most commonly used). Cannabis use and gender had independent effects on age at onset of psychosis, after adjusting for alcohol misuse and use of other drugs.
ConclusionsThe strong association between self-reported cannabis use and earlier onset of psychosis provides further evidence that schizophrenia may be precipitated by cannabis use and/or that the early onset of symptoms is a risk factor for cannabis use.

The 2005 articles by Stoltman et al. and Flannery et al. to which Neff et al. (this issue) have responded are not an indictment of instrumental neutron activation analysis (INAA) but, rather, of the way Blomster et al. (2005) misuse it and of the hyperbolic culture-historical claims they have made from their INAA results. It has long been acknowledged that INAA leads not to sources but to chemical composition groups. Based on composition groups derived from an extremely unsystematic collection of sherds from only seven localities, Blomster et al. claim that the Olmec received no carved gray or kaolin white pottery from other regions; they also claim that neighboring valleys in the Mexican highlands did not exchange such pottery with each other. Not only can one not leap directly from the elements in potsherds to such sweeping culture-historical conclusions, it is also the case that other lines of evidence (including petrographic analysis) have for 40+ years produced empirical evidence to the contrary. In the end, it was their commitment to an unfalsifiable model of Olmec superiority that led Blomster et al. to bypass the logic of archaeological inference.

Background
Studies of chronic schizophrenia suggest that there are subgroups with different profiles of cognitive impairment.
AimsTo determine whether such heterogeneity is present at illness onset and any relationship to clinical variables.
MethodNinety-three community patients with first-episode schizophrenia and 50 healthy volunteers were assessed for premorbid (Revised National Adult Reading Test) and current IQ, memory and executive function.
ResultsHalf of those with schizophrenia had preserved IQ in the normal range but there was evidence of a specific impairment in spatial working memory even in those with high/average IQ; 37 out of 93 (40%) had generalised cognitive decline. Those with low premorbid IQ were significantly younger at illness onset. For the entire group, age at onset correlated positively with premorbid but not current IQ.
ConclusionsAt illness onset, cognitive heterogeneity is present in people with schizophrenia, with a high proportion having undergone general cognitive decline. However, working memory impairment may be a common feature. Lower premorbid IQ is a risk factor for an earlier onset.

1. The flow of digesta through the duodenum and the concurrent secretion of the pancreas were studied in four Friesian calves given four milk-substitute diets. The diets were: reconstituted, ‘mildly’ pre-heated, spray-dried skim-milk powder with (SKF) or without (SK) margarine fat or with 50% of the skim-milk powder in diet SKF replaced by soya-bean flour (ASKF) or fish-protein concentrate (BSKF), together with dried whey. The diets were given ad lib. twice daily from 13 to 37 d of age, each diet being given for 6 consecutive days. Collections of duodenal digesta and pancreatic secretions, from cannulas, were made for 12 h after feeding the 6th and 12th meals (‘experimental’ meals) for each diet
2. The diets fed as ‘experimental’ meals contained polyethylene glycol (PEG) as a fluid (whey) marker and goat's milk containing [3H]lysine as a marker for total protein; β-carotene was added as a lipid marker to the three diets containing margarine fat
3. Over the 12 h postprandial period, the patterns of duodenal digesta flow and secretion of pancreatic fluid did not differ markedly between the four diets. The abomasal outflow of both nitrogen and lipid in a 12 h postprandial period was related to their intakes from the ‘penultimate’(5th and 11th) meals for diets SKF and SK but to their intakes at the ‘experimental meals’ for diets ASKF and BSKF. Secretion of pancreatic enzyme activity was highest during the 1st hour after feeding but the main outflow from the abomasum of total N and lipid occurred 5–10 h after feeding
4. The time required for all the whey marker (PEG) to pass through the duodenum was similar for diets SKF and SK, but only 53 and 42% respectively of the ingested protein marker passed through the duodenum in the 12 h after feeding. More acid appeared to be secreted by the abomasum when diet SK was given; also less undigested protein passed out of the abomasum after giving this diet. It is concluded that the physical absence of fat globules in the abomasal clot increases the degree of proteolysis
5. The secretions of pancreatic fluid and pancreatic enzyme activity were all markedly lower for diet SK than for diet SKF
6. With diets containing non-milk proteins (ASKF and BSKF), abomasal proteolysis was less efficient and the ingested protein passed out of the abomasum more rapidly than for diet SKF. There was no difference in the rate of abomasal outflow of the whey fluids between diets SKF, ASKF and BSKF
7. In comparison with diet SKF, diets ASKF and BSKF tended to induce less pancreatic enzyme secretion over a 12 h postprandial period, with the exception of lipase
8. There appeared to be no direct relationship between the quantities of any of the pancreatic enzymes secreted during a postprandial period and either the concurrent flow of duodenal digesta or the total quantities of dietary constituents passing through the duodenum.
