Doctors’ and nurses’ views on patient care for type 2 diabetes: an interview study in primary health care in Oman

Aim
This study aimed at exploring the experiences of primary health-care providers of their encounters with patients with type 2 diabetes, and their preferences and suggestions for future improvement of diabetes care.
BackgroundBarriers to good diabetes care could be related to problems from health-care providers’ side, patients’ side or the health-care system of the country. Treatment of patients with type 2 diabetes has become a huge challenge in Oman, where the prevalence has increased to high levels.
MethodSemi-structured interviews were conducted with 26 health-care professionals, 19 doctors and seven nurses, who worked in primary health care in Oman. Qualitative content analysis was applied.
FindingsOrganizational barriers and barriers related to patients and health-care providers were identified. These included workload and lack of teamwork approach. Poor patients’ management adherence and influence of culture on their attitudes towards illness were identified. From the providers’ side, language barriers, providers’ frustration and aggressive attitudes towards the patients were reflected. Decreasing the workload, availability of competent teams with diabetes specialist nurses and continuity of care were suggested. Furthermore, changing professional behaviours towards a more patient-centred approach and need for health education to the patients, especially on self-management, were addressed. Appropriate training for health-care providers in communication skills with emphasis on self-care education and individualization of care according to each patient's needs are important for improvement of diabetes care in Oman.

A total of 12902 neonatal samples collected on absorbent paper for routine metabolic screening were tested anonymously for antibodies to toxoplasma. Seroprevalence varied from 19.5% in inner London, to 11.6% in suburban London, and 7.6% in non-metropolitan districts. Much of this variation appeared to be associated with the proportions of livebirths in each district to women born outside the UK. However, additional geographical variation remained and seroprevalence in UK-born women was estimated to be 12.7% in inner London, 7.5% in suburban London, and 5.5% in non-metropolitan areas. These estimates are considerably lower than any previously reported in antenatal sera in the UK. The wide geographical variation highlights a need for further research to determine the relative importance of different routes of transmission.

Small-angle neutron scattering (SANS) and the method of contrast variation were used to measure porosity and crystallite surface area in the energetic system octahydro-1,3,5,7-tetranitro-1,3,5,7-tetrazocine (HMX) and to gauge the effects of mechanical deformation on the pore-size distribution and crystallite surface area. The crystallite surface area and the presence of voids (pores) in a high explosive system are known to affect its behavior and overall performance. Measures of these two quantities after an insult, resulting from various process and accident scenarios, can be used to predict the performance of an explosive system after process- and accident-related mechanical deformation. The contrast variation technique allows us to discriminate between internal pores and features that are on or contiguous with the crystallite surface. Measurements were conducted on loose powders of HMX (261 and 10 mm, volume averaged mean particle diameters) and pellets made by uniaxial consolidation to 7 and 10 vol% porosity, respectively. Analysis of the SANS data indicates significant alteration of the intragranular pore structure and systematic shifts in the surface area that are dependent upon mechanical deformation.
Full Band Monte Carlo Simulation of Short Channel MOSFETs in 4H and 6H-SiC

This is a presentation of a full band Monte Carlo (MC) study, which compares electron transport and device performance for 4H and 6H-SiC 100 nm n-channel MOSFETs. The model used for the electrons is based on data from a full potential band structure calculation using the Local Density Approximation (LDA) to the Density Functional Theory (DFT). For the holes the transport is based on a three band k-p model including spin orbit interaction. The two polytypes are compared regarding surface mobilities obtained with the program, as well as transconductance, unit current gain frequency, carrier velocity, I-V characteristics and energy distribution in the channel for the MOSFETs.

NGC 1365 is a barred Seyfert 1.5 galaxy. Fig 1a is a contour plot of an [OIII]λ5007 image (Jörsäter & Lindblad 1989), showing a plume-like structure pointing towards SE in the direction of the minor axis of the galaxy, which also is the direction of the steepest velocity gradient in [OIII]λ5007. The plume is roughly aligned with a radio feature in the 6 and 20 cm continuum (see Sandqvist et al., this volume). The [OIII]-emission on the NW side is weaker. This plume-like structure is not seen in Hα.
