Sociodemographic and clinical characteristics of the population with a first psychotic episode attended in the mental health services of area 5 of Madrid (Spain)

Introduction
Risk of functional impairment and progression to chronic illness in people with a first episode of psychosis (FEP) has motivated early intervention programs, showing promising results. Defining the characteristics of people with FEP at local level enables the clinicians to adjust interventional models to the reality of the population. The area 5 of Madrid (Spain) is referred to La Paz University Hospital and it serves a catchment area of roughly 527,000 people.
ObjectivesWe aim to identify sociodemographic and clinical characteristics of patients in the area 5 of Madrid (Spain) who meet the criteria of FEP.
MethodsA descriptive retrospective study including 179 people (age range 18-40 years) who were attended in mental health services of La Paz University Hospital (area 5 of Madrid, Spain), between January 2019 and May 2020, having suffered a psychotic episode in the last five years.
ResultsThe average age of people with FEP was 29.32 years, with a higher proportion of men (62%). The mean duration of untreated psychosis (DUP) was 3.64 months and 47% of patients consume cannabis. We found disparities in DUP among the different districts in the area and we also observed differences depending on the district for inclusion in rehabilitation programs or psychotherapy. The following averages were obtained for the aggregate sample: 1.01 hospitalization/year, 1.42 emergency room visits/year, 1.81 years of illness and a mean dosage equivalent to olanzapine 6.75 mg/day. The incidence of psychosis in our area has been 7.01 cases per 100000 inhabitants/year.
ConclusionsThe incidence of psychosis has been as expected according to data recorded at previous studies in Spain. The results obtained in our sample have included a lower DUP and a higher use of cannabis than those described in the literature. We have also found differences when observing the inclusion of patients in different treatments (psychotherapy, rehabilitation), which may be related to the differences in the DUP by districts. Further exploration in this field is needed to draw causal conclusions.

Background
Tobacco is a highly prevalent substance of abuse in patients with psychosis. Previous studies have reported an association between tobacco use and schizophrenia. The aim of this study was to analyze the relationship between tobacco use and first-episode psychosis (FEP), age at onset of psychosis, and specific diagnosis of psychosis.
MethodsThe sample consisted of 1105 FEP patients and 1355 controls from the European Network of National Schizophrenia Networks Studying Gene–Environment Interactions (EU-GEI) study. We assessed substance use with the Tobacco and Alcohol Questionnaire and performed a series of regression analyses using case-control status, age of onset of psychosis, and diagnosis as outcomes and tobacco use and frequency of tobacco use as predictors. Analyses were adjusted for sociodemographic characteristics, alcohol, and cannabis use.
ResultsAfter controlling for cannabis use, FEP patients were 2.6 times more likely to use tobacco [p ⩽ 0.001; adjusted odds ratio (AOR) 2.6; 95% confidence interval (CI) [2.1–3.2]] and 1.7 times more likely to smoke 20 or more cigarettes a day (p = 0.003; AOR 1.7; 95% CI [1.2–2.4]) than controls. Tobacco use was associated with an earlier age at psychosis onset (β = −2.3; p ⩽ 0.001; 95% CI [−3.7 to −0.9]) and was 1.3 times more frequent in FEP patients with a diagnosis of schizophrenia than in other diagnoses of psychosis (AOR 1.3; 95% CI [1.0–1.8]); however, these results were no longer significant after controlling for cannabis use.
ConclusionsTobacco and heavy-tobacco use are associated with increased odds of FEP. These findings further support the relevance of tobacco prevention in young populations.

Background
Social cognition has been associated with functional outcome in patients with first episode psychosis (FEP). Social cognition has also been associated with neurocognition and cognitive reserve. Although cognitive reserve, neurocognitive functioning, social cognition, and functional outcome are related, the direction of their associations is not clear. Therefore, the main aim of this study was to analyze the influence of social cognition as a mediator between cognitive reserve and cognitive domains on functioning in FEP both at baseline and at 2 years.
MethodsThe sample of the study was composed of 282 FEP patients followed up for 2 years. To analyze whether social cognition mediates the influence of cognitive reserve and cognitive domains on functioning, a path analysis was performed. The statistical significance of any mediation effects was evaluated by bootstrap analysis.
ResultsAt baseline, as neither cognitive reserve nor the cognitive domains studied were related to functioning, the conditions for mediation were not satisfied. Nevertheless, at 2 years of follow-up, social cognition acted as a mediator between cognitive reserve and functioning. Likewise, social cognition was a mediator between verbal memory and functional outcome. The results of the bootstrap analysis confirmed these significant mediations (95% bootstrapped CI (−10.215 to −0.337) and (−4.731 to −0.605) respectively).
ConclusionsCognitive reserve and neurocognition are related to functioning, and social cognition mediates in this relationship.

AGB stars are important contributors of processed matter to the ISM. However, the physical and chemical mechanisms involved in its ejection are still poorly known. This process is expected to have remarkable effects in the innermost envelope, where the dust grains are formed, the gas is accelerated, the chemistry is active, and the radiative excitation becomes important. A good tracer of this region in C-rich stars is SiS, an abundant refractory molecule that can display maser lines, very sensitive to changes in the physical conditions. We present high angular resolution interferometer observations (HPBW ≳0.″.25) of the v = 0 J = 14 – 13 and 15 – 14 SiS maser lines towards the archetypal AGB star IRC+10216, carried out with CARMA and ALMA to explore the inner 1” region around the central star. We also present an ambitious monitoring of these lines along one single pulsation period carried out with the IRAM 30 m telescope.

Silicon carbide together with amorphous carbon are the main components of dust grains in the atmospheres of C-rich AGB stars. Small gaseous Si-C bearing molecules (such as SiC, SiCSi, and SiC2) are efficiently formed close to the stellar photosphere. They likely condense onto dust seeds owing to their highly refractory nature at the lower temperatures (i.e., below about 2500 K) in the dust growth zone which extends a few stellar radii from the photosphere. Beyond this region, the abundances of Si-C bearing molecules are expected to decrease until they are eventually reformed in the outer shells of the circumstellar envelope, owing to the interaction between the gas and the interstellar UV radiation field. Our goal is to understand the time-dependent chemical evolution of Si-C bond carriers probed by molecular spectral line emission in the circumstellar envelope of IRC+10216 at millimeter wavelengths.
Blogging in Infectious Diseases and Clinical Microbiology: Assessment of ‘Blogosphere’ Content

OBJECTIVE
To analyze influential infectious diseases, antimicrobial stewardship, infection control, or medical microbiology blogs and bloggers
SETTINGWorld wide web
DESIGNWe conducted a systematic search for blogs in accordance with the PRISMA guidelines in September 2015.
METHODSA snowball sampling approach was applied to identify blogs using various search engines. Blogs were eligible if they (1) focused on infectious diseases, antimicrobial stewardship, infection control, or medical microbiology; (2) were intended for health professionals; and (3) were written in English and (4) were updated regularly. We mapped blog and blogger characteristics and used an innovative tool to assess the architecture and content of the included blogs. The motivations and perceptions of bloggers and readers were also assessed.
RESULTSA total of 88 blogs were identified. Moreover, 28 blogs (32%) focused on infectious diseases, 46 (52%) focused on medical microbiology, and 14 (16%) focused on infection control or antimicrobial stewardship. Bloggers were mainly male with medical doctorates and/or PhDs; 32 bloggers (36%) posted at least weekly; and 51 (58%) had a research purpose. The aims were considered clear for 23 blogs (26%), and the field covered was considered broad for 25 blogs (28%). Presentation was considered good for 22 blogs (25%), 51 blogs (58%) were easy to read, and 46 blogs (52%) included expert interpretation. Among the top 10 blogs, 3 focused on infectious diseases, 6 focused on medical microbiology, and 2 focused on infection control (2 were equally ranked). The bloggers we questioned were motivated to share their independent expertise and opinions. Readers appreciated the concise messages on scientific topics and practical updates.
CONCLUSIONSThis study describes high-level blogs in the fields of infectious diseases, infection control, and medical microbiology. Our findings suggest ways in which bloggers should build/orientate blogs for readers, and we have highlighted current gaps in blog topics such as antimicrobial stewardship.

LOFT (Large Observatory For X-ray Timing) is one of the four candidate missions currently under assessment study for the M3 mission in ESAs Cosmic Vision program to be launched in 2024. LOFT will carry two instruments with prime sensitivity in the 2–30 keV range: a 10 m2 class large area detector (LAD) with a <1° collimated field of view and a wide field monitor (WFM) instrument. The WFM is based on the coded mask principle, and 5 camera units will provide coverage of more than 1/3 of the sky. The prime goal of the WFM is to detect transient sources to be observed by the LAD. With its wide field of view and good energy resolution of <500 eV, the WFM will be an excellent instrument for detecting and studying GRBs and X-ray flashes. The WFM will be able to detect ~150 gamma ray bursts per year, and a burst alert system will enable the distribution of ~100 GRB positions per year with a ~1 arcmin location accuracy within 30 s of the burst.

The Burst Observer and Optical Transient Exploring System (BOOTES), is a global robotic observatory network, which started in 1998 with Spanish leadership devoted to study optical emissions from gamma ray bursts (GRBs) that occur in the Universe. We present shot history and current status of BOOTES network. The Network philosophy, science and some details of 117 GRBs followed-up are discussed.

Since the early 90s Gamma Ray Bursts have been accepted to be of extra-galactic origin thanks to the isotropic distribution observed by BATSE and the redshifts observed in some of their optical or infrared counterparts. Nevertheless, there have been a few cases that upon further examination have turned out to be of galactic origin. Several of these galactic sources have presented a Fast Rise, Exponential Decay structure which leads us to believe that there could be an underlying correlation. In this work we do several statistical analyses to determine the degree of contamination by galactic sources that certain subsample of known FREDs have. And find that certain subsamples have a most probable contamination between 27% and 34%.

Multiferroics are currently of great interest for applications in microelectronics namely in future data storage and spintronic devices. These materials couple simultaneously ferroelectric and ferromagnetic properties and have potentially different applications resulting from the coupling between their dual order parameters. A true multiferroic material is single phase. However, the known true multiferroic materials possess insufficient coupling between the two phenomena or their magnetoelectric response occurs at temperatures too low to be useful in practical applications. But a tremendous progress in the field of microelectronics can be expected if one is able to design an effective multiferroic material with ideal coupling of the ferromagnetic and ferroelectric properties to suit a particular application. Within this context composite structures are gaining considerable interest and different strategies in terms of materials microstructure have been proposed including horizontal multilayers and vertical heterostructures. In the horizontal multilayer heterostructures, the alternating layers of conventional ferro/ferrimagnetic and ferroelectric phases are grown, while in the vertical heterostructures nanopillars of the ferro/ferrimagnetic phase are embedded in a ferroelectric matrix. The later structures show advantages over the first ones because promote larger interfacial surface area and are intrinsically heteroepitaxial in three dimensions; which is expected to allow a stronger coupling between ferroelectric and ferromagnetic components.
