“It Is A Carrot-Stick Model”: A Qualitative Study of Rural-Serving Clinician and Rural-Residing Veteran Perceptions of Requirements to Quit Smoking prior to Elective Surgery

Introduction. Some medical centers and surgeons require patients to stop smoking cigarettes prior to elective orthopaedic surgeries in an effort to decrease surgical complications. Given higher rates of smoking among rural individuals, rural patients may be disproportionately impacted by these requirements. We assessed the perceptions and experiences of rural-residing Veterans and clinicians related to this requirement. Methods. We conducted qualitative semistructured one-on-one interviews of 26 rural-residing veterans, 10 VA orthopaedic surgery staff (from two Veterans Integrated Services Networks), 24 PCPs who serve rural veterans (14 VA; 10 non-VA), and 4 VA pharmacists. Using the knowledge, attitudes, and behavior framework, we performed conventional content analysis. Results. We found three primary themes across respondents: (1) knowledge of and the evidence base for the requirement varied widely; (2) strong personal attitudes toward the requirement; and (3) implementation and possible implications of this requirement. All surgery staff reported knowledge of requirements at their institution. VA PCPs reported knowledge of requirements but typically could not recall specifics. Most patients were unaware. The majority of respondents felt this requirement could increase motivation to quit smoking. Some PCPs felt a more thorough explanation of smoking-related complications would result in increased quit attempts. About half of all patients reported belief that the requirement was reasonable regardless of initial awareness. Respondents expressed little concern that the requirement might increase rural-urban disparities. Most PCPs and patients felt that there should be exceptions for allowing surgery, while surgical staff disagreed. Discussion. Most respondents thought elective surgery was a good motivator to quit smoking; but patients, PCPs, and surgical staff differed on whether there should be exceptions to the requirement that patients quit preoperatively. Future efforts to augment perioperative smoking cessation may benefit from improving coordination across services and educating patients more about the benefits of quitting.

Observations of massive, extended discs around both pre-main-sequence and main-sequence stellar systems indicate that protoplanetary discs larger than the observed planetary system are a common phenomenon, while the existence of large comets suggests that the total cometary mass is much greater than previous estimates. Both observations suggest that theories of the origin of the solar system are best approached from the perspective provided by theories of star formation, in particular that the protoplanetary disc may have extended up to ~103 AU. A model with a surface density distribution similar to a minimum-mass solar nebula, but extending further in radius, is derived by considering the gravitational collapse of a uniform, slowly rotating molecular cloud. The boundary of the planetary system is determined not by lack of mass, as in previous ‘mass-limited’ models (i.e. those with a sharp decrease in surface density Σ beyond the radius of the observed planetary system), but instead by the increasing collision time between the comets or planetesimals initially formed by gravitational instability beyond the planetary zone. Bodies formed beyond ~50 AU have sizes on the order of 102 km and represent a collisionally unevolved population; they are composed of relatively small, unaltered clumps of interstellar dust and ices with individual sizes estimated to range up to ~10 m. By contrast, bodies formed closer in, for example in the Uranus-Neptune zone, consist of larger agglomerations of dust and ices with individual sizes ranging up to ~1 km. Planetesimals formed by gravitational instability at smaller heliocentric distances r are typically much smaller than those formed further out, the masses mp being proportional to Σ3r6, but subsequent collisional aggregation in the planetary region is expected to produce bodies with sizes ranging up to 102 km or more. In both cases the first-formed solid objects may be identified with observed cometary nuclei; some accumulate to produce the outer planets, but the majority are ejected, either to interstellar space or into the Oort cloud. Observed comets represent a dynamically well-mixed group from various sources; they are expected to comprise a heterogeneous mix of both pristine and relatively altered material and to have a broad mass distribution ranging up to the size of the largest planetesimals.

Summary
Introduction
Mass lost from stars in the central regions of galaxies may flow inwards to form both massive molecular clouds and a young, new stellar population having the form of a thick, rapidly rotating nuclear disc. Angular momentum is transferred from the clouds to the stars in the original spheroid, thereby spinning up the old bulge population within the central ≃ 1 kpc and increasing the general ‘boxiness’ of the underlying stellar density distribution. New stars formed by the collapse of massive molecular clouds in the nuclear disc also contribute towards the general boxiness of the central nuclear bulge.
Bulge-disc interaction
Following previous investigations into the fate of stellar mass loss in the central regions of galaxies (e.g. Bailey & Clube 1978, Bailey 1980, 1982, 1985), we assume that material flows inwards ultimately to form a dense, cold nuclear disc of molecular gas. We assume a disc of radius Rd ≃ 500 pc and mass Md ≈ 108M⊙, embedded within a nuclear bulge of radius Rn ≃ 1 kpc and mass Mn ≃ 1010M⊙, adopting a flat rotation curve. Over a Hubble time, the total stellar mass loss exceeds 10% of the original bulge mass, i.e. ≳ 109M⊙.
We also assume that the nuclear disc fragments into clouds with masses Mc ≃ 106M⊙, by analogy with the clouds in the disc of our Galaxy. These massive clouds, moving at the local circular velocity, suffer dynamical friction against the surrounding bulge stars, which causes them to spiral slowly into the galactic centre.
