Efficacy and safety of a 4-week course of repeated subcutaneous ketamine injections for treatment-resistant depression (KADS study): randomised double-blind active-controlled trial

Background
Prior trials suggest that intravenous racemic ketamine is a highly effective for treatment-resistant depression (TRD), but phase 3 trials of racemic ketamine are needed.
AimsTo assess the acute efficacy and safety of a 4-week course of subcutaneous racemic ketamine in participants with TRD. Trial registration: ACTRN12616001096448 at www.anzctr.org.au.
MethodThis phase 3, double-blind, randomised, active-controlled multicentre trial was conducted at seven mood disorders centres in Australia and New Zealand. Participants received twice-weekly subcutaneous racemic ketamine or midazolam for 4 weeks. Initially, the trial tested fixed-dose ketamine 0.5 mg/kg versus midazolam 0.025 mg/kg (cohort 1). Dosing was revised, after a Data Safety Monitoring Board recommendation, to flexible-dose ketamine 0.5–0.9 mg/kg or midazolam 0.025–0.045 mg/kg, with response-guided dosing increments (cohort 2). The primary outcome was remission (Montgomery-Åsberg Rating Scale for Depression score ≤10) at the end of week 4.
ResultsThe final analysis (those who received at least one treatment) comprised 68 in cohort 1 (fixed-dose), 106 in cohort 2 (flexible-dose). Ketamine was more efficacious than midazolam in cohort 2 (remission rate 19.6% v. 2.0%; OR = 12.1, 95% CI 2.1–69.2, P = 0.005), but not different in cohort 1 (remission rate 6.3% v. 8.8%; OR = 1.3, 95% CI 0.2–8.2, P = 0.76). Ketamine was well tolerated. Acute adverse effects (psychotomimetic, blood pressure increases) resolved within 2 h.
ConclusionsAdequately dosed subcutaneous racemic ketamine was efficacious and safe in treating TRD over a 4-week treatment period. The subcutaneous route is practical and feasible.

Objective:
The purpose of this study was to describe the recent trends of invasive and noninvasive β-hemolytic Streptococcus cultures in the Veterans’ Affairs (VA) cohort from 2009 to 2018.
Design:Retrospective cohort study from January 1, 2009, to January 1, 2019.
Setting:Veterans’ Affairs medical centers.
Patients or participants:All patients aged 18 years and older with cultures positive for β-hemolytic Streptococcus at a VA facility were included in the study.
Intervention(s):Data were retrieved from the VA Corporate Data Warehouse using structure query language through the SQL Server Management Studio software.
Results:Between 2009 and 2018, there were 40,625 patients with cultures with β-hemolytic Streptococcus. The median age was 64 years (interquartile range [IQR], 55–71) and the median Charlson comorbidity index was 4 (IQR, 2–7). Distributions for each type of β-hemolytic Streptococcus based on site of culture are provided. The 30-day all-cause mortality rate from all invasive β-hemolytic Streptococcus cases was 2.3%, and the 90-day all-cause mortality rate was 4.4%. The 30- and 90-day all-cause mortality rates for Streptococcus cases were higher for group A (3.9% and 6.1% respectively) and for groups C and G combined (3.2% and 6.1%, respectively) than for group B (2.0% and 4.0%, respectively).
Conclusions:Trends of cultures for invasive and noninvasive β-hemolytic Streptococcus suggest an association with disease and mortality. The burden associated with β-hemolytic Streptococcus infections should not be underestimated.

In Canada, recreational use of cannabis was legalized in October 2018. This policy change along with recent publications evaluating the efficacy of cannabis for the medical treatment of epilepsy and media awareness about its use have increased the public interest about this agent. The Canadian League Against Epilepsy Medical Therapeutics Committee, along with a multidisciplinary group of experts and Canadian Epilepsy Alliance representatives, has developed a position statement about the use of medical cannabis for epilepsy. This article addresses the current Canadian legal framework, recent publications about its efficacy and safety profile, and our understanding of the clinical issues that should be considered when contemplating cannabis use for medical purposes.

Summary
Wildlife conservation in the Anthropocene means there is a pressing need to find ways for wildlife and humans to share landscapes. However, this is challenging due to the complex interactions that occur within social-ecological systems (SES). This challenge is exemplified by grey wolf management in the American West, where human governance systems influence where and at what densities carnivores persist, thereby regulating and limiting the impacts of carnivores on both human and ecological communities. Here, we build a SES conceptual framework to disentangle the interdependencies between wolves and humans, including the ecological impacts of wolves and people in anthropogenic landscapes and the socio-economic forces shaping human–wolf interactions now and in the future. A key lesson is that coexistence rests not only on the biophysical capacity of a landscape to be shared by humans and wolves, but also on the capacity for human societies to adjust to and accept some level of conflict with wolves. As such, a holistic view that recognizes humans, our social systems and institutions as key actors and attributes of ecological systems can advance the theory and practice of coexistence.

Analysis of genetic diversity in an invasive population of Asian long-horned beetles in Ontario, Canada

Adult Asian long-horned beetles, Anoplophora glabripennis (Motschulsky) (Coleoptera: Cerambycidae: Lamiinae), were discovered in Ontario, Canada, in 2003 in the vicinity of a commercial warehouse. Trees were heavily scarred with signs of attack and larvae and adult beetles were common, suggesting that there had been multiple generations at the site. We amplified 16 microsatellite loci from 326 beetles to examine genetic diversity in this population. Based on Hardy – Weinberg equilibrium, 6 of 16 loci were monomorphic and 8 were not, indicating nonrandom mating. Measures of microsatellite genetic diversity and mitochondrial DNA haplotype diversity were significantly lower than those in A. glabripennis from China and Korea but were not significantly different from those in the New York City population. The proportion of different multilocus genotypes in the Ontario population was lower than in the populations in New York City and Linden, New Jersey. These results suggest that limited genetic diversity in the Ontario population has not hampered reproduction of this invasive insect. This genetic signature is common in other invasive species, likely because a population is founded by a few closely related individuals, or a large founding population suffers subsequent genetic bottlenecks.

The formal commissioning of the IRWG occurred at the 1991 Buenos Aires General Assembly, following a Joint Commission meeting at the IAU GA in Baltimore in 1988 that identified the problems with ground-based infrared photometry. The meeting justification, papers, and conclusions, can be found in Milone (1989). In summary, the challenges involved how to explain the failure to achieve the milli-magnitude precision expected of infrared photometry and an apparent 3% limit on system transformability. The proposed solution was to redefine the broadband Johnson system, the passbands of which had proven so unsatisfactory that over time effectively different systems proliferated, although bearing the same “JHKLMNQ” designations; the new system needed to be better positioned and centered in the spectral windows of the Earth's atmosphere, and the variable water vapour content of the atmosphere needed to be measured in real time to better correct for atmospheric extinction.

Psychiatric bed utilization: 1896 and 1996 compared

Background. The 1896 and 1996 populations of North West Wales are similar in number, ethnic and social mix and rurality, enabling a study of the comparative incidence and prevalence of service utilization over the course of a century.
Methods. We collected records on all psychiatric admissions from North-West Wales in 1894–1896 and 1996. These were assessed and diagnosed by the responsible sector area consultant.
Results. The data reveal substantially more patients admitted for all diagnoses in 1996, even when comparisons are restricted to detained patients. The incidence of hospitalization by detention for schizophrenia is slightly lower 1996 than in 1896 but the incidence of hospitalization is higher now than in 1996. The incidence of hospitalization by detention for non-affective disorder psychoses is the same in both 1896 and 1996 but there is a doubling of incidence of hospitalization. The incidence of hospitalization for bipolar disorders is similar in the two periods. Modern mental health services admit large numbers of personality disordered patients, where none were admitted 100 years ago.
Conclusions. Factors general to changing health care and expectations and others specific to mental health would appear to have led to the increase in rates of admissions observed in the modern period.
