The purpose of this document is to highlight practical recommendations to assist acute-care hospitals in prioritization and implementation of strategies to prevent healthcare-associated infections through hand hygiene. This document updates the Strategies to Prevent Healthcare-Associated Infections in Acute Care Hospitals through Hand Hygiene, published in 2014. This expert guidance document is sponsored by the Society for Healthcare Epidemiology (SHEA). It is the product of a collaborative effort led by SHEA, the Infectious Diseases Society of America, the Association for Professionals in Infection Control and Epidemiology, the American Hospital Association, and The Joint Commission, with major contributions from representatives of a number of organizations and societies with content expertise.
Association of anticholinergic burden with adverse effects in older people with intellectual disabilities: an observational cross-sectional study

Background
No studies to date have investigated cumulative anticholinergic exposure and its effects in adults with intellectual disabilities.
AimsTo determine the cumulative exposure to anticholinergics and the factors associated with high exposure.
MethodA modified Anticholinergic Cognitive Burden (ACB) scale score was calculated for a representative cohort of 736 people over 40 years old with intellectual disabilities, and associations with demographic and clinical factors assessed.
ResultsAge over 65 years was associated with higher exposure (ACB 1–4 odds ratio (OR) = 3.28, 95% CI 1.49–7.28, ACB 5+ OR = 3.08, 95% CI 1.20–7.63), as was a mental health condition (ACB 1–4 OR = 9.79, 95% CI 5.63–17.02, ACB 5+ OR = 23.74, 95% CI 12.29–45.83). Daytime drowsiness was associated with higher ACB (P<0.001) and chronic constipation reported more frequently (26.6% ACB 5+ v. 7.5% ACB 0, P<0.001).
ConclusionsOlder people with intellectual disabilities and with mental health conditions were exposed to high anticholinergic burden. This was associated with daytime dozing and constipation.

Approximately half of the variation in wellbeing measures overlaps with variation in personality traits. Studies of non-human primate pedigrees and human twins suggest that this is due to common genetic influences. We tested whether personality polygenic scores for the NEO Five-Factor Inventory (NEO-FFI) domains and for item response theory (IRT) derived extraversion and neuroticism scores predict variance in wellbeing measures. Polygenic scores were based on published genome-wide association (GWA) results in over 17,000 individuals for the NEO-FFI and in over 63,000 for the IRT extraversion and neuroticism traits. The NEO-FFI polygenic scores were used to predict life satisfaction in 7 cohorts, positive affect in 12 cohorts, and general wellbeing in 1 cohort (maximal N = 46,508). Meta-analysis of these results showed no significant association between NEO-FFI personality polygenic scores and the wellbeing measures. IRT extraversion and neuroticism polygenic scores were used to predict life satisfaction and positive affect in almost 37,000 individuals from UK Biobank. Significant positive associations (effect sizes <0.05%) were observed between the extraversion polygenic score and wellbeing measures, and a negative association was observed between the polygenic neuroticism score and life satisfaction. Furthermore, using GWA data, genetic correlations of -0.49 and -0.55 were estimated between neuroticism with life satisfaction and positive affect, respectively. The moderate genetic correlation between neuroticism and wellbeing is in line with twin research showing that genetic influences on wellbeing are also shared with other independent personality domains.

Metabolism and performance during extended high-intensity intermittent exercise after consumption of low- and high-glycaemic index pre-exercise meals

The metabolic and performance benefits of prior consumption of low-glycaemic index (GI) meals v. high-GI meals were determined in extended high-intensity intermittent exercise. Participants (ten males and four females, aged 25·8 (sd 7·3) years) completed two testing days (each consisting of back-to-back 90-min intermittent high-intensity treadmill running protocols separated by 3 h) spaced by at least 7 d. Using a randomised counterbalanced cross-over design, low-GI, lentil-based meals (GI about 42) or high-GI, potato-based meals (GI about 78) matched for energy value were consumed 2 h before, and within 1 h after, the first exercise session. Performance was measured by the distance covered during five 1-min sprints (separated by 2·5 min walking) at the end of each exercise session. Peak postprandial blood glucose was higher by 30·8 % in the high-GI trial compared with the low-GI trial, as was insulin (P = 0·039 and P = 0·003, respectively). Carbohydrate oxidation was lower by 5·5 % during the low-GI trials compared with the high-GI trials at the start of the first exercise session (P < 0·05). Blood lactate was significantly higher (6·1 v. 2·6 mmol/l; P = 0·019) and blood glucose significantly lower (4·8 v. 5·4 mmol/l; P = 0·039) at the end of the second exercise session during the high-GI trial compared with the low-GI trial. Sprint distance was not significantly different between conditions. A low-GI meal improved the metabolic profile before and during extended high-intensity intermittent exercise, but did not affect performance. Improvements in metabolic responses when consuming low-GI meals before exercise may be beneficial to the long-term health of athletes.

The mechanism responsible for driving the ubiquitous winds of cool giant and supergiant stars remains to be established. To this end, we are constructing semi-empirical models of the extended outer atmospheres (‘chromospheres’) and winds of selected red supergiants. These models are constrained by analyses of the UV line spectra of single stars, and of red supergiants in binaries that eclipse their main-sequence companions: the ζ Aur and VV Cep stars. These detached binaries are well-separated, with no evidence of mass transfer. The C II] 2325 Å line profiles of the binaries are similar to those of comparable single stars, suggesting that the chromospheres remain relatively unperturbed by binarity. However, it is unclear how much binarity disturbs the wind: binary observations suggest a gradual acceleration (β ∼ 3), but line profile analyses of single red supergiants imply a rapid acceleration (β < 1). To date, we have obtained extensive series of HST/GHRS and STIS observations of three eclipsing red supergiant binaries: ζ Aur, HR 2554 and VV Cep. In this paper, we focus on ζ Aur, and present observations and modelling results for this eclipsing binary.
