Objectives
To point out to the medical community who believes in, ‘Prevention is better than cure’ that the alarming report of cause of sudden increase in PTSD sufferers, especially in young war veterans is very clear. The memory of human suffering imprints an indelible legacy in the minds of the young people for the rest of their life ultimately voiding them into a life of social burden. As health workers like always and ironically we deal with the victims of various socio-political decisions made by world leaders, dictators and even by the terrorists. Now do we think it's a time to give a lonely message to all that ‘war should be stopped?’
MethodsThis is a personal audit on studying over 500 news reports and published papers written on PTSD incidence after a major war in the last decade.
ResultsRemove the cause PTSD will be eradicated. On the other PTSD victims i.e. abused in childhood, rape victims, road traffic accident victims or witnesses, fire fighters, policemen we hardly can manoeuvre in the field of prevention. However weak it is; we can give a message to the warring powers to save youngsters from future PTSD.
Conclusions• PTSD is preventable but has very bleak curative achievement.
• We should not deprive our youngsters of the peaceful life, most of us are enjoying in our 40s, 50s and 60s.
• European congress can raise a voice against the proven cause of PTSD.

Objectives
Chronic diabetics at some point gives up their vigilance in the sugar control. Many of them end up with intractable diabetic feet with limb losses incurring health budget, enormous morbidity and mortality.
Methods300 patients who presented with diabetic feet been interviewed and noticed that ‘few weeks or months prior to their start of symptoms a major change in the treatment mechanism happens which lead to catastrophic complications of Diabetes’. They do not qualify according to DSM IV classification to be called MDE. But can be called Dysthymia. Surgical, Medical, Vascular and Psychiatric units in an Australian Hospital have been involved in these interviews (a simple questionnaire). Personal audit in two countries (Fiji Islands & Bangladesh) where Diabetes is pretty alarming in number have been included in the data.
ResultsDiabetic patients over time develop Dysthymia when they feel hopeless about control of their illness. Often they are overwhelmed by thoughts of long-term complications like retinopathy, neuropathy that they give up hope that they could combat its complications. Diabetic educators should be involved during their early stage of their illness before complications set in and should be trained to pick up dysthymic signs and symptoms for prompt referral for early psychological and psychiatric intervention.
Conclusions:
• Diabetic feet are preventable.
• Early psychiatric assessment is helpful.
• Preventive steps are cost effective.
• Should Psychiatry Department have a role in the multidisciplinary management team of Diabetes?

A raw clay from Uruguay was modified with aluminium to obtain an aluminium pillared clay (Al-PILC). The solids were characterized by scanning electron microscopy, X-ray diffraction and nitrogen adsorption-desorption isotherms. The Al-PILC retained the typical laminar structure of montmorillonite. The specific surface area and the microporous volume of the Al-PILC, 235 m2 g-1 and 0.096 cm3 g-1, respectively, were much higher than those of the clay. The phosphate adsorption capacity of the Al-PILC was higher than those of the clay. The phosphate adsorption kinetic followed the pseudo-first-order model for both, the clay and the Al-PILC, and the phosphate adsorption isotherm for the Al-PILC fit the Freundlich model.

Epigenetic and inflammatory marker profiles associated with depression in a community-based epidemiologic sample

Background
Recent work suggests that epigenetic differences may be associated with psychiatric disorders. Here we investigate, in a community-based sample, whether methylation profiles distinguish between individuals with and without lifetime depression. We also investigate the physiologic consequences that may be associated with these profiles.
MethodUsing whole blood-derived genomic DNA from a subset of participants in the Detroit Neighborhood Health Study (DNHS), we applied methylation microarrays to assess genome-wide methylation profiles for over 14 000 genes in 33 persons who reported a lifetime history of depression and 67 non-depressed adults. Bioinformatic functional analyses were performed on the genes uniquely methylated and unmethylated in each group, and inflammatory biomarkers [interleukin (IL)-6 and C-reactive protein (CRP)] were measured to investigate the possible functional significance of the methylation profiles observed.
ResultsUniquely unmethylated gene sets distinguished between those with versus without lifetime depression. In particular, some processes (e.g. brain development, tryptophan metabolism) showed patterns suggestive of increased methylation among individuals with depression whereas others (e.g. lipoprotein) showed patterns suggestive of decreased methylation among individuals with depression. IL-6 and CRP levels were elevated among those with lifetime depression and, among those with depression only, IL-6 methylation showed an inverse correlation with circulating IL-6 and CRP.
ConclusionsGenome-wide methylation profiles distinguish individuals with versus without lifetime depression in a community-based setting, and show coordinated signals with pathophysiological mechanisms previously implicated in the etiology of this disorder. Examining epigenetic mechanisms in concert with other dynamic markers of physiologic functioning should improve our understanding of the neurobiology of depression.
