Controversy continues over the relationship between calcium intake and bone density in women. Epidemiological evidence does not strongly support the conclusion that high levels of dietary calcium during the postmenopausal years influence the extent or rate of bone loss. Dietary calcium in the growing years may be a determinant of peak adult bone density attained at maturity. Experimental trials do not appear to support the contention that calcium supplementation will restore lost bone in healthy or osteoporotic postmenopausal women. However, an increased calcium intake to an optimal individual level may slow the rate of bone loss in postmenopausal women. The cross-sectional and controlled investigations regarding the relationship between calcium intake and bone density will be discussed in this review.

Background and Objective.
Patients undergoing orthopedic surgery are susceptible to methicillin-resistant Staphylococcus aureus (MRSA) infections, which can result in increased morbidity, hospital lengths of stay, and medical costs. We sought to estimate the economic value of routine preoperative MRSA screening and decolonization of orthopedic surgery patients.
Methods.A stochastic decision-analytic computer simulation model was used to evaluate the economic value of implementing this strategy (compared with no preoperative screening or decolonization) among orthopedic surgery patients from both the third-party payer and hospital perspectives. Sensitivity analyses explored the effects of varying MRSA colonization prevalence, the cost of screening and decolonization, and the probability of decolonization success.
Results.Preoperative MRSA screening and decolonization was strongly cost-effective (incremental cost-effectiveness ratio less than $6,000 per quality-adjusted life year) from the third-party payer perspective even when MRSA prevalence was as low as 1%, decolonization success was as low as 25%, and decolonization costs were as high as $300 per patient. In most scenarios this strategy was economically dominant (ie, less costly and more effective than no screening). From the hospital perspective, preoperative MRSA screening and decolonization was the economically dominant strategy for all scenarios explored.
Conclusions.Routine preoperative screening and decolonization of orthopedic surgery patients may under many circumstances save hospitals and third-party payers money while providing health benefits.

Summary
INTRODUCTION
The emerging field of community and ecosystem genetics has so far focused on how the genetic variation in one species can influence the composition of associated communities and ecosystem processes such as decomposition (see definitions in Table 3–1; reviews by Whitham et al. 2003, 2006; Johnson & Stinchcombe 2007; Hughes et al. 2008). A key component of this approach has been an emphasis on understanding how the genetics of foundation plant species influence a much larger community. It is reasoned that because foundation species structure their ecosystems by creating locally stable conditions and provide specific resources for diverse organisms (Dayton 1972; Ellison et al. 2005), the genetics of these species as “community drivers” are most important to understand and most likely to have cascading ecological and evolutionary effects throughout an ecosystem (Whitham et al. 2006). For example, when a foundation species’ genotype influences the relative fitness of other species, it constitutes an indirect genetic interaction (Shuster et al. 2006), and when these interactions change species composition and abundance among individual tree genotypes, they result in individual genotypes having distinct community and ecosystem phenotypes. Thus, in addition to an individual genotype having the “traditional” phenotype that population geneticists typically consider as the expression of a trait at the individual and population level, community geneticists must also consider higher-level phenotypes at the community and ecosystem level. The predictability of phenotypes at levels higher than the population can be quantified as community heritability (i.e., the tendency for related individuals to support similar communities of organisms and ecosystem processes; Whitham et al. 2003, 2006; Shuster et al. 2006).

Background.
Methicillin-resistant Staphylococcus aureus (MRSA) transmission and infections are a continuing problem in hospitals. Although some have recommended universal surveillance for MRSA at hospital admission to identify and to isolate MRSA-colonized patients, there is a need for formal economic studies to determine the cost-effectiveness of such a strategy.
Methods.We developed a stochastic computer simulation model to determine the potential economic impact of performing MRSA surveillance (ie, single culture of an anterior nares specimen) for all hospital admissions at different MRSA prevalences and basic reproductive rate thresholds from the societal and third party-payor perspectives. Patients with positive surveillance culture results were placed under isolation precautions to prevent transmission by way of respiratory droplets. MRSA-colonized patients who were not isolated could transmit MRSA to other hospital patients.
Results.The performance of universal MRSA surveillance was cost-effective (defined as an incremental cost-effectiveness ratio of less than $50,000 per quality-adjusted life-year) when the basic reproductive rate was 0.25 or greater and the prevalence was 1% or greater. In fact, surveillance was the dominant strategy when the basic reproductive rate was 1.5 or greater and the prevalence was 15% or greater, the basic reproductive rate was 2.0 or greater and the prevalence was 10% or greater, and the basic reproductive rate was 2.5 or greater and the prevalence was 5% or greater.
Conclusions.Universal MRSA surveillance of adults at hospital admission appears to be cost-effective at a wide range of prevalence and basic reproductive rate values. Individual hospitals and healthcare systems could compare their prevailing conditions (eg, the prevalence of MRSA colonization and MRSA transmission dynamics) with the benchmarks in our model to help determine their optimal local strategies.

Background.
Methicillin-resistant Staphylococcus aureus (MRSA) can cause severe infection in patients who are undergoing vascular surgical operations. Testing all vascular surgery patients preoperatively for MRSA and attempting to decolonize those who have positive results may be a strategy to prevent MRSA infection. The economic value of such a strategy has not yet been determined.
Methods.We developed a decision-analytic computer simulation model to determine the economic value of using such a strategy before all vascular surgical procedures from the societal and third-party payer perspectives at different MRSA prevalence and decolonization success rates.
Results.The model showed preoperative MRSA testing to be cost-effective (incremental cost-effectiveness ratio, <$50,000 per quality-adjusted life year) when the MRSA prevalence is ≥0.01 and the decolonization success rate is ≥0.25. In fact, this strategy was dominant (ie, less costly and more effective) at the following thresholds: MRSA prevalence ≥0.01 and decolonization success rate ≥0.5, and MRSA prevalence ≥0.025 and decolonization success rate ≥0.25.
Conclusion.Testing and decolonizing patients for MRSA before vascular surgery may be a cost-effective strategy over a wide range of MRSA prevalence and decolonization success rates.

An international project is proposed to create a map showing the world subdivided into macroecosystem regions within each of which ecological conditions are relatively uniform but which show certain natural potentials and limitations. The map should tend to supplement the Dasmann-Udvardy system of biogeographical provinces, being of higher resolution and greater ecological relevance. The primary purpose of the map will be to serve as a reporting structure for information about global resources and environment, though it will be based largely on published information.
Maps based on classification of climatic types, vegetation formations, and soil groups, will be synthesized and generalized to delineate the areas to be shown on the ecoregion map. Its delineations will be refined through consultations with local experts, and through the interpretation of low-resolution remote-sensing imagery. The usefulness of the map is considered favourably in relation to national-level policy analysis, environmental monitoring, transfer of agricultural technology, compatibility with remote-sensing systems for monitoring environmental conditions, and agricultural activities, biomass estimation, macroreserve selection, and land management.

This explanatory note is intended to accompany the map included in this issue. The map is on a scale of 1:30,000,000 (1 cm = 300 km) and shows regional-scale ‘ecosystem’ units, or ecoregions, differentiated according to a scheme modified from J.M. Crowley (1967), and using climate and vegetation as indicators of the extent of each unit. The units are similar in concept to ‘ecobiomes’ as proposed by Polunin (1984). The map was developed following a proposal by Bailey & Hogg (1986) to supplement the Udvardy (1975) system of biogeographical provinces with a treatment of higher resolution. Three levels or categories of this hierarchy are shown. Of these the broadest, domains, and within them divisions, are based largely on the broad ecological climate zones following the Koppen system as modified by Trewartha (1968), and summarized in our Tables I and II. Climate is emphasized at the broadest levels because of its overriding effect on the composition and productivity of ecosystems etc. from region to region.
