166 Predicting 30 Day Return Hospital Admissions in Patients with COVID-19 Discharged from the Emergency Department: A national retrospective cohort study

OBJECTIVES/GOALS: Identification of COVID-19 patients at risk for deterioration following discharge from the emergency department (ED) remains a clinical challenge. Our objective was to develop a prediction model that identifies COVID-19 patients at risk for return and hospital admission within 30 days of ED discharge. METHODS/STUDY POPULATION: We performed a retrospective cohort study of discharged adult ED patients (n = 7,529) with SARS-CoV-2 infection from 116 unique hospitals contributing to the national REgistry of suspected COVID-19 in EmeRgency care (RECOVER). The primary outcome was return hospital admission within 30 days. Models were developed using Classification and Regression Tree (CART), Gradient Boosted Machine (GBM), Random Forest (RF), and least absolute shrinkage and selection (LASSO) approaches. RESULTS/ANTICIPATED RESULTS: Among COVID-19 patients discharged from the ED on their index encounter, 571 (7.6%) returned for hospital admission within 30 days. The machine learning (ML) models (GBM, RF,: and LASSO) performed similarly. The RF model yielded a test AUC of 0.74 (95% confidence interval [CI] 0.71–0.78) with a sensitivity of 0.46 (0.39-0.54) and specificity of 0.84 (0.82-0.85). Predictive variables including: lowest oxygen saturation, temperature; or history of hypertension,: diabetes, hyperlipidemia, or obesity, were common to all ML models. DISCUSSION/SIGNIFICANCE: A predictive model identifying adult ED patients with COVID-19 at risk for return hospital admission within 30 days is feasible. Ensemble/boot-strapped classification methods outperform the single tree CART method. Future efforts may focus on the application of ML models in the hospital setting to optimize allocation of follow up resources.
Understanding public perception of coronavirus disease 2019 (COVID-19) social distancing on Twitter

Objective:
Social distancing policies are key in curtailing severe acute respiratory coronavirus virus 2 (SARS-CoV-2) spread, but their effectiveness is heavily contingent on public understanding and collective adherence. We studied public perception of social distancing through organic, large-scale discussion on Twitter.
Design:Retrospective cross-sectional study.
Methods:Between March 27 and April 10, 2020, we retrieved English-only tweets matching two trending social distancing hashtags, #socialdistancing and #stayathome. We analyzed the tweets using natural language processing and machine-learning models, and we conducted a sentiment analysis to identify emotions and polarity. We evaluated the subjectivity of tweets and estimated the frequency of discussion of social distancing rules. We then identified clusters of discussion using topic modeling and associated sentiments.
Results:We studied a sample of 574,903 tweets. For both hashtags, polarity was positive (mean, 0.148; SD, 0.290); only 15% of tweets had negative polarity. Tweets were more likely to be objective (median, 0.40; IQR, 0–0.6) with ~30% of tweets labeled as completely objective (labeled as 0 in range from 0 to 1). Approximately half of tweets (50.4%) primarily expressed joy and one-fifth expressed fear and surprise. Each correlated well with topic clusters identified by frequency including leisure and community support (ie, joy), concerns about food insecurity and quarantine effects (ie, fear), and unpredictability of coronavirus disease 2019 (COVID-19) and its implications (ie, surprise).
Conclusions:Considering the positive sentiment, preponderance of objective tweets, and topics supporting coping mechanisms, we concluded that Twitter users generally supported social distancing in the early stages of their implementation.

Background
Lithium (Li) is the gold standard treatment for bipolar disorder (BD). However, its mechanisms of action remain unknown but include neurotrophic effects. We here investigated the influence of Li on cortical and local grey matter (GM) volumes in a large international sample of patients with BD and healthy controls (HC).
MethodsWe analyzed high-resolution T1-weighted structural magnetic resonance imaging scans of 271 patients with BD type I (120 undergoing Li) and 316 HC. Cortical and local GM volumes were compared using voxel-wise approaches with voxel-based morphometry and SIENAX using FSL. We used multiple linear regression models to test the influence of Li on cortical and local GM volumes, taking into account potential confounding factors such as a history of alcohol misuse.
ResultsPatients taking Li had greater cortical GM volume than patients without. Patients undergoing Li had greater regional GM volumes in the right middle frontal gyrus, the right anterior cingulate gyrus, and the left fusiform gyrus in comparison with patients not taking Li.
ConclusionsOur results in a large multicentric sample support the hypothesis that Li could exert neurotrophic and neuroprotective effects limiting pathological GM atrophy in key brain regions associated with BD.

Characterisation of Collagen Scaffolds using X-ray Microtomography

Collagen scaffolds have been produced that incorporate predefined internal channels. The scaffolds were obtained with the aid of sacrificial moulds that have been manufactured using a rapid prototyping technique. A computer aided design file of the mould was created and then realised using an ink-jet printer. A dispersion of collagen was then cast into the mould and frozen. Ethanol was used to dissolve the mould leaving the collagen, which was then freeze dried to produce the final product. The scaffold was then analysed using X-ray microtomography (XMT) to determine whether the desired internal structure was obtained. It was found necessary to add saturated potassium iodide (KI) solution to the scaffold in order to analyse it satisfactorily by XMT. The resultant images indicate that the desired internal structure was obtained.

The apparent decline in voter participation in national elections since 1972 is an illusion created by using the Bureau of the Census estimate of the voting-age population as the denominator of the turnout rate. We construct a more accurate estimate of those eligible to vote, from 1948–2000, using government statistical series to adjust for ineligible but included groups, such as noncitizens and felons, and eligible but excluded groups, such as overseas citizens. We show that the ineligible population, not the nonvoting, has been increasing since 1972. During the 1960s the turnout rate trended downward both nationally and outside the South. Although the average turnout rates for presidential and congressional elections are lower since 1972 than during 1948–70, the only pattern since 1972 is an increased turnout rate in southern congressional elections. While the voting age was lowered to 18 in 1971, the lower turnout rate of young voters accounts for less than one-fourth of reduced voter participation.

Down through the years the Constitution has not always included all Americans — especially Blacks and women. No such charges have been leveled against the Religious Liberty clauses, though there have been occasions when their protections have been overridden or imperfectly applied. To remind us — as the Framers knew well — that “nothing human can be perfect,” we hear remarks from a distinguished Baptist pastor, an accomplished lawyer, and an eminent civil rights leader, Dr. Benjamin Hooks.
