Importance of Identifying Cognitive Impairment in Multiple Sclerosis

This article aims to highlight the impact of cognitive impairment on outcomes and quality of life for people with multiple sclerosis (MS) and to review current evidence for the efficacy of disease-modifying therapies (DMTs) and other interventions. In addition, we provide clinical practice insights regarding screening and management of cognitive impairment in people with MS. Evidence suggests that cognitive deterioration often accompanies magnetic resonance imaging changes. Neocortical volume and deep grey matter atrophy correlate with cognitive impairment. Similarly, cognitive decline is predictive of a higher lesion burden. Cognitive impairment is an important clinical measure of disability and negatively impacts quality of life. Phase 3 studies suggest that DMTs such as natalizumab, ozanimod and fingolimod may provide long-lasting, clinically meaningful effects on cognition in people with MS. Further data are needed to support the use of adjunct cognitive behavioural and exercise interventions for people with MS who have cognitive impairment. More data are needed to define appropriate management strategies for cognitive impairment in people with MS. Baseline and periodic screening for cognitive impairment and inclusion of cognitive impairment as a clinical trial endpoint will help to inform efforts to manage this important aspect of MS.
Barriers and facilitators to implementing changes in opioid prescribing in rural primary care clinics

Background:
Opioids are more commonly prescribed for chronic pain in rural settings in the USA, yet little is known about how the rural context influences efforts to improve opioid medication management.
Methods:The Six Building Blocks is an evidence-based program that guides primary care practices in making system-based improvements in managing patients using long-term opioid therapy. It was implemented at 6 rural and rural-serving organizations with 20 clinic locations over a 15-month period. To gain further insight about their experience with implementing the program, interviews and focus groups were conducted with staff and clinicians at the six organizations at the end of the 15 months and transcribed. Team members used a template analysis approach, a form of qualitative thematic analysis, to code these data for barriers, facilitators, and corresponding subcodes.
Results:Facilitators to making systems-based changes in opioid management within a rural practice context included a desire to help patients and their community, external pressures to make changes in opioid management, a desire to reduce workplace stress, external support for the clinic, supportive clinic leadership, and receptivity of patients. Barriers to making changes included competing demands on clinicians and staff, a culture of clinician autonomy, inadequate data systems, and a lack of patient resources in rural areas.
Discussion:The barriers and facilitators identified here point to potentially unique determinants of practice that should be considered when addressing opioid prescribing for chronic pain in the rural setting.

Summary
Introduction
‘Child protection is everyone's job’ is a familiar sentiment, heard from statutory child protection services throughout the UK. In Scotland, this has been expressed by the mantra ‘It's everyone's job to make sure I’m all right’ (Scottish Executive, 2002), and in England and Wales by ‘Every child matters’ (DoE, 2003). Yet this sense of shared concern and responsibility is often accompanied by feelings of helplessness and inadequacy, in personal and professional discussions about keeping children safe from sexual harm.
This chapter builds upon our belief that protecting children from sexual exploitation and abuse cannot be considered in isolation, but is one key element in an integrated approach to supporting children and families within their communities. It cannot be separated from the range of social factors which influence children's development and life experience. That is an international approach to children's wellbeing, and to the comprehensive way in which it needs to be secured. It is reemphasised in the United Nations’ Millenium Goals (UN, 2006; Stahl et al, 2006).
Such an approach means acknowledging the realities of poverty, inequalities and deprivation, alongside culturally harmful attitudes to gender, race and diversity, as well as the predatory and brutalising behaviour of some individuals and groups. Thus joined-up strategic planning, which takes account of all available evidence of connections among personal, community and societal influences, is needed. That vitally includes fostering and supporting genuinely informed communities in protecting children and young people from abuse and exploitation, and involving young people actively in their own safety.
Contrast with individual approaches
This approach contrasts with the individually focused, narrowly professionalised approaches to child protection which have long dominated public policy. We analyse this critically in the first part of this chapter. Such systems have been unable to make substantial progress in preventing harm and abuse. While investigation will still be needed in individual cases, and must be of the highest quality we can achieve, systems based almost entirely on a case-by-case approach can never tackle the root causes of the ‘bottomless pit of need’ (Baldwin and Carruthers, 1998). Yet even where locally based, community-wide protective services are implemented, they find it hard to maintain funding stability – usually being the first to be cut during hard economic times. Witness, for example, the ‘roller-coaster’ existence of family centres, and the struggles of local Sure Start projects to maintain their outreach work.

Summary
Patients with comorbid major depression and generalized anxiety disorder (GAD) tend to have a more severe and prolonged course of illness and greater functional impairment. GAD is one of the most common mental disorders in primary medical-care settings, and is associated with increased use of health services. Current theories on pathological anxiety fit a multifactorial epigenetic model that integrates early stressors, inherited and acquired vulnerabilities, and the risks of developing interrelated or coincidental somatic diseases. The tolerability profile of prescribed medication is an important consideration, particularly when long-term treatment is recommended. Distressing symptoms on stopping treatment are common with many classes of psychotropic drug, including selective serotonin reuptake inhibitor (SSRIs), serotonin-norepinephrine reuptake inhibitors (SNRIs), and benzodiazepines. There is clearly room for improvement in the development of more efficacious and more acceptable pharmacological approaches to the management of this common, distressing, typically disabling, and often persistent anxiety disorder.

One hundred and seventy children attending a hospital accident and emergency department following everyday trauma were interviewed and completed the Post-traumatic Stress Disorder (PTSD) screening battery suggested by Yule and Udwin (1991). Diagnostic interviews (CAPS-C) confirmed that 39 (22.9%) fulfilled the DSM-IV criteria for PTSD. There were significant differences between children with and without PTSD on each individual component of the screening battery. Various criteria for caseness were evaluated and at 6 weeks post trauma the screen identified up to 90% of children diagnosed with PTSD and 73% with borderline conditions. A subset of 36 children were reassessed 8 months post trauma and all children with persistent PTSD were correctly identified by initial screen scores. The limitations of the study and the role of screening for PTSD in the absence of proven psychological interventions are discussed.

This is the first compendious study of the influence of Platonism on the English literary tradition, showing how English writers used Platonic themes and images within their own imaginative work. Source texts include Plato's Dialogues, and the writings of Neoplatonists and the early Christians who were largely responsible for assimilating Platonic ideas into a Christian culture; and there are essays on more than thirty English authors from the Middle Ages to the twentieth century, including Shakespeare, Milton, Blake, Wordsworth, Yeats, Pound and Iris Murdoch. Each chronological section has its own introduction to highlight how every age has reconstructed Platonism to suit its own understanding of the world, and there is a bibliographical guide to further reading. Established experts and new writers over a range of disciplines have worked together to produce the first comprehensive overview of Platonism in English literature.

Summary
This is a study of the way that Platonism and Neoplatonism run like a changing thread through the web of English literature. Not all of the authors here represented are English, and some did not even write in that language. But they have all contributed to a tradition which can be defined and which still continues. Our main aid to that definition has been to confine ourselves to writers who knew at least part of the corpus of Platonic texts at first hand. As can be seen from the bibliography, these texts include not only Plato's Dialogues but also the writings of the Neoplatonists, especially Plotinus, through whom Plato was read for so long. Our choice of writers has necessarily had to be selective but our chronological range from antiquity to the present offers, for the first time, a comprehensive overview of the influence of Platonism on English literature.
That very comprehensiveness is, of course, a source of danger, and our account must of necessity be both partial and personal. But this is also true of the readings of Plato and his followers by the writers who are discussed here. Plato himself was a rich and diverse writer, and every age has rediscovered Plato in a different way, and reinterpreted Platonism to suit its different understandings of the world. This is not the story of a nexus of mummified ideas carried forward by authors with a nostalgia for antiquity, but of particular people who read within a related group of philosophical texts and responded to them individually in very different contexts from the pagan culture in which Plato and Plotinus lived.
