Automated hand hygiene compliance system’s audible alert reminder increases healthcare worker hand hygiene compliance

Objective:
To evaluate the effectiveness of an automated hand hygiene compliance system (AHHCS) audible alert and vibration for increasing hand hygiene compliance.
Design:A nonrandomized, before-and-after, quasi-experimental study of an AHHCS was implemented in several inpatient units. Over a 51-day period, the system’s real-time audible alert was turned on, off, and back on. Overall, hand hygiene compliance was compared between days with activated and deactivated alerts and vibration.
Setting:This study was conducted at a level 1 trauma center, a regional academic health system with 1,564 beds.
Participants:The AHHCS was implemented in 9 inpatient units: 3 adult medical-surgical step-down units, and 6 adult intensive care units. The AHHCS badges were assigned to patient care assistants, registered nurses, physical therapists, occupational therapists, speech therapists, respiratory therapists, and physicians.
Intervention:In the 9 inpatient units, selected healthcare staff were issued wearable badges that detected entry into and exit from a patient room. The audible alert was turned on for 16 days, turned off for 17 days, and then turned back on for 18 days, for a total of 51 days.
Results:Utilization of the AHHCS real-time audible alert reminder resulted in sustained HH compliance ≥90%. When the alert and vibration were deactivated, HH compliance dropped to an average of 74% (range, 62%–78%). Once the alert resumed, HH compliance returned to ≥90%.
Conclusion:Utilization of an AHHCS with real-time reminder audible alerts may be an effective method to increase healthcare worker HH compliance to ≥90%. Users of AHHCSs should consider the use of real-time reminders to improve HH compliance.
Screening for Major Depression in the Early Stages of Multiple Sclerosis

Background
Multiple Sclerosis (MS) is associated with a high risk of developing major depression, but depression in MS patients frequently goes undetected and untreated. The current study examined the clinical utility of the Beck Depression Inventory (BDI) as a screening measure for major depression in newly diagnosed MS patients.
MethodsForty-six new referrals to an MS clinic completed the BDI and participated in a structured interview for major depression, within 2 months of the diagnosis of MS.
ResultsAccording to DSM-III-R criteria, 40% of patients were diagnosed with major depression, 22% had adjustment disorder with depressed mood, and 37% showed no evidence of mood disorder. Sensitivity and specificity values, and positive and negative predictive values are reported for every BDI cut-off score between 9 and 21.
ConclusionsA BDI cut-off score of 13 (sensitivity = .71, specificity = .79) is recommended as optimal for use in screening for major depression in newly diagnosed MS patients. The use of the BDI as a screening measure for major depression must proceed with caution given that a cut-off score of 13 still yielded a false-negative rate of 30%.

Progress in achieving improved performance in the generation and utilization of hydrogen depends on our ability to identify materials with optimized electrical and (photo)- electrochemical performance. Given their high volume fraction of interfaces, high chemical stability and versatility (ionic, electronic, optical property control), nanocrystalline electroceramic materials are of growing interest for advanced energy conversion and storage technologies. As grain size decreases towards the Debye length and grain boundaries come in closer proximity, space charge properties begin to dominate, resulting in modified charge transport. Through systematic variation of grain boundary properties by heterogeneous indiffusion of cations, the electronic and ionic carrier profiles in the space charge region may be altered. The relationships between space charge potential and defect profiles in the space charge regions are quantitatively analyzed, and implications for nano-ionic materials in thin film solid oxide fuel cells are discussed. From the standpoint of photoelectrochemical water splitting for hydrogen generation, optimizing the band gap, band alignments, and transport properties while retaining stability has remained a challenging objective. Novel nanocrystalline composite structures are discussed which exhibit features amenable to optimization of required properties and electrical measurements to determine key transport properties of titanium dioxide nanopowder, a photoanode material are introduced.

A study of the content of water soluble vitamins in the 12 baby milk formulae available in the UK in April 1983 showed that levels of vitamins in the individual brands were generally in excess of those declared by the manufacturer. For cows' milk based formulae the mean excess of vitamins over the declared level was 94%. Vitamin levels in prepared formulae averaged three times the minimum recommended level. The average level of B vitamins in prepared cows' milk based formulae was six times higher than in mature human milk. For the two soya based products, the levels of vitamins were generally in excess of the declared level and levels of vitamin B12, biotin and, in one sample, vitamin B6 were considerably in excess of those found in mature human milk.

A survey was undertaken to update and extend available information on the vitamin content of pasteurized milk as produced at processing dairies in mainland UK and to investigate regional, seasonal and breed effects. The concentration of total retinol in milk from non-Channel Island (NCI) breeds averaged 61·9 βg/100g in summer and 41·2 βg/100g in winter. Concentrations of β-carotene were 31·5 and 10·5 βg/100g in summer and winter respectively. Concentrations of retinol in milk from Channel Island (CI) breeds were similar, but concentrations of β-carotene were on average 3 times higher. The concentration of vitamin D3 in milk from NCI breeds was 0·033 βg/100g in summer and 0·026 βg/100g in winter. There was no marked seasonal variation in the mean concentration of total vitamin C (14·5 βg/ml). Values for the concentration of B vitamins (βg/ml) were: folic acid 0·060, vitamin B12 0·0042, riboflavin 1·78, nicotinic acid 0·71, pantothenic acid 3·60, biotin 0·020, thiamin 0·46 and vitamin B6 0·61. Seasonal variation in the concentration was most marked for folic acid (c.v. 17·4%) and to a lesser extent for vitamin B12 (c.v. 10·3%). The only breed differences in the B vitamin content were for riboflavin and folic acid, the mean values obtained for milk from CI breeds being respectively 20 and 10 % higher than those from NCI breeds.

Concentrations were measured of different vitamins in milk as delivered to the home and after storage in the domestic refrigerator, during summer and winter periods. Vitamin C concentrations varied greatly, ranging from 0·1 to 18·4 εg/ml. Most of the lower vitamin C values were associated with known exposure of the milk to daylight and there was a clear tendency for the concentrations to fall with increased time of refrigerated storage especially when associated with prior exposure of the milk to daylight. Riboflavin values were mostly in the range 1·35–1·92εg/ml, but occasional lower values were associated with low content of vitamin C and in some instances with prolonged exposure to daylight on the doorstep. Concentrations of the other vitamins (εg/m1) were: folic acid 0·041–0·074, thiamin 0·38–0·48, vitamin B12 0·0032–0·0044 and vitamin B6 0·56–0·71. All fell within the normal ranges and were not significantly influenced by exposure of the milk to daylight or subsequent storage in the refrigerator.
