Impact of specialty on the self-reported practice of using oral antibiotic therapy for definitive treatment of bloodstream infections

Background:
No established guidelines exist regarding the role of oral antibiotic therapy (OAT) to treat bloodstream infections (BSIs), and practices may vary depending on clinician specialty and experience.
Objective:To assess practice patterns regarding oral antibiotic use for treatment of bacteremia in infectious diseases clinicians (IDCs, including physicians and pharmacists and trainees in these groups) and non–infectious diseases clinicians (NIDCs).
Design:Open-access survey.
Participants:Clinicians caring for hospitalized patients receiving antibiotics.
Methods:An open-access, web-based survey was distributed to clinicians at a Midwestern academic medical center using e-mail and to clinicians outside the medical center using social media. Respondents answered questions regarding confidence prescribing OAT for BSI in different scenarios. We used χ2 analysis for categorical data evaluated association between responses and demographic groups.
Results:Of 282 survey responses, 82.6% of respondents were physicians, 17.4% pharmacists, and IDCs represented 69.2% of all respondents. IDCs were more likely to select routine use of OAT for BSI due to gram-negative anaerobes (84.6% vs 59.8%; P < .0001), Klebsiella spp (84.5% vs 69.0%; P < .009), Proteus spp (83.6% vs 71.3%; P < .027), and other Enterobacterales (79.5% vs 60.9%; P < .004). Our survey results revealed significant differences in selected treatment of Staphylococcus aureus syndromes. Fewer IDCs than NIDCs selected OAT to complete treatment for methicillin-resistant S. aureus (MRSA) BSI due to gluteal abscess (11.9% vs 25.6%; P = .012) and methicillin-susceptible S. aureus (MSSA) BSI due to septic arthritis (13.9% vs 20.9%; P = .219).
Conclusions:Practice variation and discordance with evidence for the use of OAT for BSIs exists among IDCs versus NIDCs, highlighting opportunities for education in both clinician groups.

Biological invasions are one of the grand challenges facing society, as exotic species introductions continue to rise and can result in dramatic changes to native ecosystems and economies. The scale of the “biological invasions crisis” spans from hyperlocal to international, involving a myriad of actors focused on mitigating and preventing biological invasions. However, the level of engagement among stakeholders and opportunities to collaboratively solve invasives issues in transdisciplinary ways is poorly understood. The Biological Invasions: Confronting a Crisis workshop engaged a broad group of actors working on various aspects of biological invasions in Virginia, USA—researchers, Extension personnel, educators, local, state, and federal agencies, nongovernmental organizations, and land managers—to discuss their respective roles and how they interact with other groups. Through a series of activities, it became clear that despite shared goals, most groups are not engaging with one another, and that enhanced communication and collaboration among groups is key to designing effective solutions. There is strong support for a multistakeholder coalition to affect change in policy, public education/engagement, and solution design. Confronting the biological invasions crisis will increasingly require engagement among stakeholders.

The Texas red beds represent one of the richest series of early Permian deposits in the world. In particular, the Clear Fork Group has produced a diverse assemblage of temnospondyls, early reptiles, and synapsids. However, most of this material has been sourced from the oldest member, the Arroyo Formation, and the understanding of the paleoecosystem of the younger Vale and Choza formations is less well resolved. Here we present a previously undescribed Vale locality, the first vertebrate-bearing locality from the formation to be described in detail in several decades, from near Abilene, Texas with juvenile diplocaulids, captorhinids, abundant material of rare taxa such as Varanops and diadectids, and the first report of a recumbirostran ‘microsaur’ from the formation. This assemblage is atypical of early Permian deposits in the taxonomic and size distribution of the vertebrate fauna in comparison to other localities from the Vale Formation that preserve a greater abundance of aquatic taxa (e.g., fishes, Trimerorhachis) and synapsids (e.g., Dimetrodon). Minimal abrasion of the elements, relative articulation and association of the specimen of Varanops, and the paucity of aquatic taxa suggest an ephemeral pond deposit in which organisms were preserved essentially in situ. Our characterization of the locality also permits a revision and discussion of the vertebrate faunal assemblage of the Vale Formation.
The Composition of Poly(Ethylene Terephthalate) (PET) Surface Precipitates Determined at High Resolving Power by Tandem Mass Spectrometry Imaging

We present the first demonstration of a general method for the chemical characterization of small surface features at high magnification via simultaneous collection of mass spectrometry (MS) imaging and tandem MS imaging data. High lateral resolution tandem secondary ion MS imaging is employed to determine the composition of surface features on poly(ethylene terephthalate) (PET) that precipitate during heat treatment. The surface features, probed at a lateral resolving power of<200 nm using a surface-sensitive ion beam, are found to be comprised of ethylene terephthalate trimer at a greater abundance than is observed in the surrounding polymer matrix. This is the first chemical identification of PET surface precipitates made without either an extraction step or the use of a reference material. The new capability employed for this study achieves the highest practical lateral resolution ever reported for tandem MS imaging.

Analysis of the two largest southern Maya lowland metal assemblages, from Lamanai (n = 187) and Tipu (n = 99), Belize reveals that Mesoamerican and European technologies were negotiated through the processes of recycling objects to create new forms and juxtaposing objects of different provenances for bodily ornamentation. Lamanai's occupants began acquiring metal as early as a.d. 1100 and then engaged in on-site metallurgy as early as a.d. 1450, continuing into the early seventeenth century. Tipu was a nexus for metals between the late sixteenth and early seventeenth centuries. At both sites, metal objects were recovered primarily from human burials and midden deposits. A program of typological study and compositional analysis revealed forms shared between the sites but evidence of on-site metallurgy is supported only at Lamanai. Metals from these Maya communities, both centers of Spanish reducción, demonstrate that the southern Maya lowlands was by no means a “refuge” from Spanish aggression.

The surface layers on coal fly ash particles are of special environmental interest in that concentration enrichments of trace elements may occur [1], thereby enhancing the potential bioavailability of toxic species. Little research, however, has been devoted to the analytical characterization of intraparticle and interparticle distributions of trace elements. The high detection sensitivity, spatial resolution, and depth profiling capabilities of secondary ion mass spectrometry (SIMS), coupled to digital image acquisition and processing [2], permit three-dimensional (3D) compositional maps for collections of individual micron-sized particles. The 3D analysis of trace element distributions in coal fly ash particles is the subject of this SIMS investigation
