I support the major theme of Heyman's target article that behavioral choice theory can enhance our understanding of drug dependence, but I raise concerns about the critique of the operant model of drug dependence, the underscoring of melioration to the exclusion of other theories of choice, and assertions about the unique effects of drug reinforcement.

Summary
Contingency management (CM) interventions represent one treatment approach that has great potential to effectively motivate and facilitate change in this challenging clinical population. CM interventions are based on extensive basic-science and clinical-research evidence demonstrating that drug use and abuse are heavily influenced by learning and conditioning and are quite sensitive to systematically applied environmental consequences. Behavioral-analytic theory and the empirical literature on behavior change in general suggests that the efficacy of CM interventions is influenced by the schedule used to deliver consequences the magnitude of the consequence, the choice of the target behavior, the selection of the type of consequence, and the monitoring of the target behavior. The chapter uses examples from cannabis CM program to describe these basic principles and illustrate their application. Almost half of those seeking treatment for marijuana abuse have criminal justice involvement and are referred to treatment by the legal system.

We briefly discuss some potential contributions of behavioral momentum research to the study of the behavioral effects of abused drugs. Contributions to the study of the direct effects of drugs on operant responding and to the study of drugs as reinforcers are addressed. Too little empirical evidence is available to thoroughly evaluate the relevance of behavioral momentum concepts to the study of drugs and behavior, but we note several reasons for optimism regarding its potential to make positive contributions.

Summary
This chapter reviews positive results from a series of well-controlled clinical trials supporting the efficacy of Community Reinforcement Approach (CRA) treatments for cocaine and opioid dependence. It presents a brief description of the rationale for adding the voucher-based incentives to CRA. In developing this treatment for cocaine dependence in the late 1980s, the goal was to develop an intervention to treat cocaine dependence in outpatient settings. More substantially, the trial by Abbott and colleagues with opioid-dependent individuals did not involve vouchers and yet the CRA treatment resulted in greater reductions in opioid use than standard drug-abuse counseling. Delivered with or without vouchers, CRA represents an efficacious treatment that can enhance treatment outcomes with cocaine and opioid-dependent patients. Recognizing the importance of reinforcement to substance dependence and integrating systematic use of that principle into clinical efforts to reduce cocaine and opioid dependence has the potential to substantially improve treatment outcomes.
