Improvement in Depression Symptoms Measured by Montgomery-Åsberg Depression Rating Scale and Quick Inventory of Depressive Symptomatology-Self Rated Items after Randomised Double-blind COMP360 Psilocybin Therapy for Treatment-resistant Depression

Introduction
COMP360 is a synthetic, proprietary, purified form of psilocybin in development for treatment-resistant depression (TRD) with FDA Breakthrough Therapy designation. In a recent phase IIb study, COMP360 psilocybin 25mg was superior to 1mg on change from baseline (CFB) to Week 3 on the Montgomery-Åsberg Depression Rating Scale (MADRS) total score (primary efficacy endpoint), when administered alongside psychological support. Quick Inventory of Depressive Symptomatology-Self Rated (QIDS-SR16) total score (exploratory efficacy endpoint) showed similar results.
ObjectivesTo analyse changes in specific depression symptoms after psilocybin treatment in the aforementioned study, as measured by individual item scores on the MADRS and QIDS-SR16 (range 0-6 and 0-3).
MethodsParticipants with TRD were randomised to single doses of psilocybin 25mg (n=79), 10mg (n=75), or 1mg (n=79). A remote, blinded rater assessed the MADRS at Baseline, Day 2 (the day post-psilocybin), and Weeks 1, 3, 6, 9, and 12. The QIDS-SR16 was self-rated at Baseline, Day 1, Day 2, and Weeks 1, 2, 3, 6, 9, and 12. At each time point, descriptive statistics were calculated for each MADRS and QIDS-SR16 individual item score.
ResultsAt Week 3, MADRS items with the largest differences in mean CFB in the 25mg arm were Inability to Feel, Apparent Sadness, Lassitude, and Reported Sadness. Greater improvement in the 25mg arm was apparent from Day 2 and remained to Week 12 (Lassitude remained to Week 6 only). On the QIDS-SR16, the item with the largest difference in mean CFB at Week 3 in the 25mg arm was in Feeling Sad and remained evident to Week 12 (Table 1).

As anhedonia is predictive of poorer treatment response, and improvements in anhedonia correlate with improvements in functioning, it is important to understand the impact of treatments on this symptom.

Bacterial superinfection and antibiotic prescribing in the setting of the current mpox outbreak are not well described in the literature. This retrospective observational study revealed low prevalence (11%) of outpatient antibiotic prescribing for bacterial superinfection of mpox lesions; at least 3 prescriptions (23%) were unnecessary.

OBJECTIVES/GOALS: 1) Characterize racial differences in congestive heart failure care delivery. 2) Examine the extent to which specific clinical roles were associated with improved care outcomes (i.e., hospitalizations, readmissions, days between readmissions, and charges) of African Americans (AA) with CHF. METHODS/STUDY POPULATION: EMR data was extracted from the Arkansas Clinical Data Repository (AR-CDR) on patients (ages 18-105) who received care between January 1, 2014 and December 31, 2021. Variables included age, sex, race, ethnicity, rurality, clinical diagnosis, morbidities, medical history, medications, heart failure phenotypes, and care delivery team composition. Binomial logistic regression ascertained the effects of these variables on patient’s care outcomes. A Mann Whitney-U test identified racial differences in outcomes. Psychometrically, classical test theory and item response theory assessed items for the risk surveillance tool. RESULTS/ANTICIPATED RESULTS: The study identified 5,962 CHF patients who generated 80,921 care encounters. The results revealed the disproportionate impact of CHF prevalence, hospitalizations, and readmissions on AAs. AAs had a significantly higher number of hospitalizations (i.e., 50% more) than Caucasians. Specific clinical roles (i.e., MDs, RNs, Care Managers) were consistently associated with 30% or greater decrease in odds of hospitalization and readmission, even when stratified by heart failure phenotype. Classical test theory results (e.g., Cronbach’s alpha; 0.88) indicated the set of items on the risk surveillance tool accurately reflect a patient risk for improved outcomes. DISCUSSION/SIGNIFICANCE: The findings stimulate the need for 1) EHR-based tools that manage care delivery equity and 2) investigations of specific clinical roles in risk stratifying and operationalizing the care plans of AAs, advancing formal access-to-care frameworks by ensuring access to clinical roles that are associated with improved outcomes.

Eggletonite is a new mineral, the Na analogue of ganophyllite. Type material has composition
(Na0.82K0.40Ca0.39□0.39)Σ2.00 (Mn6.61Zn0.08Mg0.16Fe0.61Al0.56)Σ8.02 (Si10.33Al1.67)Σ12.00[(O28.92(OH)3.08]Σ32.00 (OH)4.00·10.66H2O.
It occurs as prismatic crystals elongated along [100] in miarolitic cavities in nepheline syenite at the Big Rock Quarry, Little Rock, Arkansas. Single-crystal X-ray diffraction data are consistent with space group I2/a or Ia; a = 5.554, b = 13.72, c = 25.00Å, β = 93.95° Z = 2; this is equivalent to the substructure of ganophyllite. Physical properties include cleavage: {001} perfect; colour: dark brown; translucent; streak: light brown; hardness (Mohs): 3–4; brittle; observed density 2.76, calculated density 2.76 g/cm3. Optically, eggletonite is biaxial negative, with optical parameters 2Vα = 9(3)° r < ν, weak; indices of refraction α = 1.566(2), β = 1.606(2), γ = 1.606(2); pleochroism is very weak, γ ≃ β > α; α = pale brown to colourless, β ≃ γ = pale yellow brown. Principal lines in the X-ray powder diffraction pattern are (d, I/Io): 12.4, 100; 3.45, 15; 3.13, 30; 2.854, 15; 2.691, 25; 2.600, 20; 2.462, 20; 2.387, 15. The name is in honour of Dr Richard A. Eggleton, Australian National University.

Retzian-(La), ideally, Mn2La(AsO4)(OH)4, is a new rare-earth analogue of retzian and retzian-(Nd). It was found associated with willemite, calcite, and franklinite, in the Stealing Hill Mine, Ogdensburg, Sussex County, New Jersey, USA. Retzian-(La) is orthorhombic, space group Pban, with a = 5.670(7), b = 12.01(1), and c = 4.869(8) Å. The strongest lines in the X-ray powder diffraction pattern are (d,I/I0,hkl): 3.51 80 111, 2.715 100 131, 1.848 50 061, 1.456 40. Microprobe analysis yielded MnO 25.2, MgO 3.7, ZnO 1.2, As2O5 26.5, La2O3 10.6, Ce2O3 8.8, Nd2O3 8.2, Pr2O3 5.1, Sm2O3 2.2, Y2O3 2.5, H2O [7.7], sum = 101.7 wt. %, corresponding to an La-dominant member of the retzian series.
Retzian-(La) forms 0.5 mm euhedral crystals composed of the forms {001}, {010}, {110}, and {150}; the crystals have pseudohexagonal habit. It is reddish-brown, with vitreous luster, no cleavage, density > 4.2 g/cm3. Optically, it is biaxial, positive, 2V = 82° with α = 1.766, β = 1.773, and γ = 1.788. Dispersion is strong, r > v; pleochroism is very weak; orientation is X = c, Y= b, Z = a; absorption is Z > Y > X.

Frictional Behavior of C60 Microparticle-Coated Steel

The frictional behaviors of 304 stainless steel disks coated with C60 microparticles, both containing benzene and free of benzene, have been studied under different loads and sliding speeds with a pin-on-disk configuration in ambient air atmosphere at room temperature. The results indicated that the coating containing benzene, benzene-solvated C60 microparticles (C60·4C6H6), reduced friction as well as wear. The coated samples showed a 50–70% reduction in friction coefficient in comparison to uncoated samples. Neither the coated nor the uncoated sample showed significant change in friction coefficient for different sliding speeds. Under different loads, the uncoated sample had almost the same friction coefficient. However, with the increase of load, the friction coefficient of C60·4C6H6-coated disk showed a minimum value of 0.25 at 25 g load and then reached the uncoated values beyond 50 g load. The coefficient of friction of the disk coated with benzene-free C60 showed a slight increase with load, reaching the value of uncoated 304 stainless steel disk at about 40 g. The reduced friction of the solvated-C60 coated 304 stainless steel is probably due to the lowered shear strength of the hcp structure of C60·4C6H6 molecular crystal in which the benzene molecules are intercalated. The results of this study suggest the importance of the presence of second component, in addition to C60, in the coating materials in order for them to form a preferred crystal structure with low shear strength as far as using C60 as a solid lubricant is concerned.

Hospitals, insurance companies, and federal and state governments are increasingly concerned about reducing patient cost expenditures while maintaining high quality patient care. One method of reducing expenditures has been to tie hospital reimbursement with a prospective payment system based on diagnosis-related groups (DRGs). However, reimbursement under the DRG system is not acceptable for all patients in all hospitals because it is neither an accurate predictor of costs nor of clinical outcome. This deficiency poses significant problems for hospitals because DRGs are used nationwide as the prospective payment system for inpatients covered by Medicare. Several case-mix adjusters have been proposed to modify DRGs to improve their accuracy in predicting costs and outcome. We reviewed five of the most widely available indices: Acute Physiologic and Chronic Health Evaluation (APACHE II), Coded Disease Staging, Computerized Severity Index (CSI), Medical Illness Severity Group System (MEDISGROUPS), and Patient Management Categories (PMC). Recommendations for the use of a single case-mix adjuster cannot be made at this time because all indices have not been compared in sufficiently diverse settings and because some are better predictors of costs while others are better predictors of clinical outcome. Hospital epidemiologists and other infection control practitioners should be informed about these indices and their potential applications as they expand their role beyond infection control problems to issues concerning cost containment, quality assurance, and reimbursement.
