Internet cognitive behavioural therapy for mixed anxiety and depression: a randomized controlled trial and evidence of effectiveness in primary care

Background
Major depressive disorder (MDD) and generalized anxiety disorder (GAD) have the highest co-morbidity rates within the internalizing disorders cluster, yet no Internet-based cognitive behavioural therapy (iCBT) programme exists for their combined treatment.
MethodWe designed a six-lesson therapist-assisted iCBT programme for mixed anxiety and depression. Study 1 was a randomized controlled trial (RCT) comparing the iCBT programme (n = 46) versus wait-list control (WLC; n = 53) for patients diagnosed by structured clinical interview with MDD, GAD or co-morbid GAD/MDD. Primary outcome measures were the Patient Health Questionnaire nine-item scale (depression), Generalized Anxiety Disorder seven-item scale (generalized anxiety), Kessler 10-item Psychological Distress scale (distress) and 12-item World Health Organization Disability Assessment Schedule II (disability). The iCBT group was followed up at 3 months post-treatment. In study 2, we investigated the adherence to, and efficacy of the same programme in a primary care setting, where patients (n = 136) completed the programme under the supervision of primary care clinicians.
ResultsThe RCT showed that the iCBT programme was more effective than WLC, with large within- and between-groups effect sizes found (>0.8). Adherence was also high (89%), and gains were maintained at 3-month follow-up. In study 2 in primary care, adherence to the iCBT programme was low (41%), yet effect sizes were large (>0.8). Of the non-completers, 30% experienced benefit.
ConclusionsTogether, the results show that iCBT is effective and adherence is high in research settings, but there is a problem of adherence when translated into the ‘real world’. Future efforts need to be placed on developing improved adherence to iCBT in primary care settings.

Imploding indirect-drive double shell targets may provide an alternative, non-cryogenic path to ignition at the National Ignition Facility (NIF). Experiments are being pursued at OMEGA to understand the hydrodynamics of these implosions and the possibility of scaling it to the NIF design. We have used 40 beams from the OMEGA laser to directly drive the capsules, and we have used the remaining 20 beams to backlight the imploding shells from two different directions at multiple times. We will review the recent experiments to measure the hydrodynamics of the targets using two-view X-ray radiography of the capsules. We will present data on measured yields from the targets. We will present a measured time history of the hydrodynamics of the implosion. Experiments were pursued using direct drive in which the M-band effect (experienced in the indirect drive experiments) could be eliminated or controlled. It was learned in the direct drive experiments that the best performing capsules were those that had a thin outer layer of gold. This effectively causes M-band pre-heat effects giving implosion hydrodynamics and performance closer to the indirect drive case. We will review the methods used to radiograph the targets and the techniques used to extract useful information to compare with calculations. The effect of imperfections in the target construction will be shown to be minimal during the initial stage of implosion. The yields from the targets were observed to be uniformly low compared to indirect-drive.

Nonlinear or multiphoton fluorescence microscopy utilizes the simultaneous absorption of two (or three) longer wavelength photons to excite fluorophores (Denk et.al., 1990). For example, UV absorbing fluorophores such as DAPI or lndo-1 are excited using 700 nm near infrared light rather than 350 nm UV excitation. This relatively new form of laser scanning fluorescence microscopy has excellent optical sectioning capabilities in thick, highly scattering tissues. The high degree of intrinsic optical sectioning arises from the spatial nature of the excitation dependence, rather than from a confocal aperture or deconvolution algorithm. The fluorescence arising from any point in the specimen depends on the second or third power of the intensity (i.e. two and three photon excitation, respectively). This squaring (or cubing) of the illumination point spread function effectively confines the excitation to a tenth of a femtoliter optical volume when using a high NA lens. 680 to 1100 nm mode-locked lasers producing pulses of 100 femtosecond duration at a repetition rate of 80 Mhz are used to efficiently produce fluorescence excitation at average powers that are usually in the 0.5 to 10 mW range at the sample.

Considerable obscurity surrounds the introduction into Islamic thought of the orthodox doctrine that it is God who “creates” the acts of man, whereas man merely “acquires” them (kasaba, iktasaba). The doctrine is sometimes attributed to al-Ash'arī, but in his Maqālāt al-Islamīyīn he himself frequently uses the term in his accounts of the views of other writers. This paper examines the various uses of kasaba and iktasaba in that work to see what light is thrown on the origin and development of the conception.
