Comparative dosimetric analysis of whole abdominal radiotherapy with VMAT versus 3D conformal in patients with Wilms tumour

Introduction
In this study, we undertake a dosimetric comparison of whole abdominal treatment plans of patients diagnosed with stage 3 Wilms tumour, to assess the benefits of treating these patients with volumetric arch therapy (VMAT) versus 3D conformal radiotherapy.
Material and methodsA retrospective study was undertaken on 23 patients receiving either VMAT or 3D conformal radiotherapy during 2013–2017. A dosimetric comparison was undertaken for both techniques, measuring planning target volume (PTV), conformity index (CI), homogeneity index (HI) and organs at risk (OAR).
ResultsThe dosimetric parameters for the PTV dose in the VMAT and 3D conformal technique showed no statistical difference (1,289·17 cGy versus 1,357·13 cGy, respectively, p=0·404). However, the VMAT technique had a better CI (1·04 VMAT versus 1·26 3D, p=0·004), and there was little difference in the HI (1·13 VMAT versus 1·15 3D, p=0·1606). In the statistical analysis, the decrease in dose to OAR for the VMAT technique is statistically significant for doses to lung and kidney (p=0·011 and p=0·002, respectively). Between the two techniques, there was no statistical significance in dose difference to the other OAR.
ConclusionThis work proposes using the VMAT technique in whole abdominal irradiation to improve conformity, without affecting the quality of the PTV coverage, when compared with the 3D conformal technique. In addition, VMAT reduces the doses to OAR such as the remaining kidney and lungs that are important to preserve to reduce the probability of radiation toxicity in these patients.

INTRODUCTION:
Anemia is a major problem, frequently resulting from iron deficiency (1). Guidelines recommend the administration of intravenous (IV) iron, leaving blood transfusions for critical patients due to the potential impact in length of stay (LOS) and mortality (2,3). We aimed to characterize IV iron sucrose utilization and health resource utilization in anemic patients.
METHODS:This is a retrospective ongoing cohort study. Patient records from a general Portuguese Hospital with an administration of iron sucrose in 2014–2015 were reviewed. Adult anemic patients with at least one hemoglobin (Hb) evaluation before and after the administration of IV iron were included. Endpoints assessed were: Hb level (baseline, 4 and 8 weeks after), anemia correction rate at weeks 4 and 8, blood transfusions, length of stay (LOS), rate of readmissions (<30 days) and inpatient mortality. Statistical analysis included non-parametric and chi-square tests to assess differences between groups and a logistic regression model, using a 5 percent significance level.
RESULTS:Data was collected for 401 patients (63.1 percent female; mean age Standard Deviation, SD: 62.6 (21.7) years) and 431 IV iron sucrose administration episodes. Mean cumulative iron dose was 679.5 mg. Baseline Hb level was 84.5 g/l and increased to 94.3 g/l (week 4) and to 103.0 g/l (week 8). Blood transfusions were performed in 53.8 percent of the episodes. Overall 157 (36.4 percent) episodes had a >20 g/l increase in Hb level. Blood transfusions were associated with a higher proportion of Hb level increase >20 g/l (44.0 percent versus 27.9 percent, p<.001). The overall mean LOS was 15.3 days, although episodes with transfusions had a significantly longer duration (17.5 days versus 12.7 days; p<.001). Overall readmission rate was 25.8 percent, with a higher proportion in episodes with blood transfusions (29.3 percent versus 21.6 percent). A total of 36 patients (9.0 percent) died at the hospital before discharge. Transfusions performed during or after IV iron administration increased 3.1 times the risk of in-hospital death (95 percent Confidence Interval, CI: 1.3-7.0; p = .008), after adjusting for age and sex.
CONCLUSIONS:We observed a high rate of blood transfusions in this cohort treated with intravenous iron sucrose for anemia. Transfusions were associated with substantial burden of resource consumption and in-hospital mortality.

Using ultrafast angle-resolved light scattering technique, we were able to trigger photoinduced phase transition processes in V2O3 film grown on a glass substrate. The phase transition is caused by photoacoustic wave in the film and appears as coherent oscillations of scattering signal at various time scales. These processes strongly depend on the size of microstructures constituting the V2O3 film. One of the key findings of our study is the presence of a size dependent phase transition threshold for V2O3 microstructures, where small size structures ( $\tilde <$ 0.5μm) have lowest contribution to the phase transition. The presence of this threshold can be well described by considering uneven internal strain in the films which is one of the key parameters controlling phase transition dynamics in various vanadium oxides.

Possible participation of calmodulin in the decondensation of nuclei isolated from guinea pig spermatozoa

The guinea pig spermatozoid nucleus contains actin, myosin, spectrin and cytokeratin. Also, it has been reported that phalloidin and/or 2,3-butanedione monoxime retard the sperm nuclear decondensation caused by heparin, suggesting a role for F-actin and myosin in nuclear stability. The presence of an F-actin/myosin dynamic system in these nuclei led us to search for proteins usually related to this system. In guinea pig sperm nuclei we detected calmodulin, F-actin, the myosin light chain and an actin-myosin complex. To define whether calmodulin participates in nuclear-dynamics, the effect of the calmodulin antagonists W5, W7 and calmidazolium was tested on the decondensation of nuclei promoted by either heparin or by a Xenopus laevis egg extract. All antagonists inhibited both the heparin- and the X. laevis egg extract-mediated nuclear decondensation. Heparin-mediated decondensation was faster and led to loss of nuclei. The X. laevis egg extract-promoted decondensation was slower and did not result in loss of the decondensed nuclei. It is suggested that in guinea pig sperm calmodulin participates in the nuclear decondensation process.

Laser induced insulator-to-metal phase transition in VO2 thin films was explored using femtosecond optical pump-probe spectroscopy. The transient reflection and relaxation processes in VO2 were observed during phase transition on 10-11 - 10-9 temporal-scale. Optical generation of carriers in VO2 initiates extremely fast changes in reflectivity which is strongly dependent on pump fluence. Analysis of transient optical properties allows suggesting the excitonic-controlled mechanism responsible for the light-induced phase transition in VO2.

Insulator-to-metal phase transition (PT) and relaxation dynamics in VO2 were studied with ultrafast transient reflection spectroscopy and degenerate four wave mixing technique. Optical pumping of free carriers in VO2 initiates PT on a femtosecond time scale. The relaxation dynamics is strongly dependent on pump energy. Spectral reflectivity behaviour at PT due to thermal heating and upon laser excitation of VO2 demonstrates close proximity in the relative change. PT induced by light was interpreted by exciton-controlled mechanism. Excited state dynamics of metallic VO2 is assigned to formations of electronic and hole polaronic states.
