Phase 3 Safety and Tolerability Results of the Combination Olanzapine and Samidorphan in Patients with Schizophrenia: The 1 Year ENLIGHTEN-2-Extension

Objective
Combination olanzapine and samidorphan (OLZ/SAM) is in development for treatment of schizophrenia and bipolar I disorder and is intended to provide the antipsychotic efficacy of olanzapine while mitigating olanzapine-associated weight gain. This 52-week open-label extension study (NCT02873208; ENLIGHTEN-2-EXT) in schizophrenia assessed the safety and tolerability of OLZ/SAM. Methods: Patients completing the 24-week, randomized, double-blind, phase 3 ENLIGHTEN−2 study comparing weight gain with OLZ/SAM vs olanzapine were eligible for ENLIGHTEN-2-EXT enrollment. Initial OLZ/SAM doses were based on olanzapine dose (10 or 20 mg) received at the conclusion of ENLIGHTEN-2; subsequent olanzapine dose adjustments were allowed. The samidorphan dose (10 mg) remained fixed throughout. Assessments included adverse events (AEs), weight, waist circumference, metabolic laboratory parameters, and Positive and Negative Syndrome Scale (PANSS) scores. Analyses were based on observed results using descriptive statistics. Baseline was relative to the first OLZ/SAM dose in the extension study.
Results265 patients received OLZ/SAM; 167 (63.0%) completed the extension study. Common AEs (= 5%) were weight decreased (n=23; 8.7%), extra dose administered (n=21; 7.9%), headache (n=18; 6.8%), and weight increased (n=16; 6.0%). At week 52, mean (SD) change from baseline for weight and waist circumference was −0.03 (6.216) kg and −0.35 (6.115) cm, respectively. Changes in fasting lipid and glycemic parameters were generally small and remained stable over 52 weeks. PANSS total scores remained stable during the extension.
ConclusionsOLZ/SAM was generally well tolerated over 52 weeks. Weight, waist circumference, metabolic laboratory parameters, and schizophrenia symptoms remained stable throughout the study.

In recent years, a variety of efforts have been made in political science to enable, encourage, or require scholars to be more open and explicit about the bases of their empirical claims and, in turn, make those claims more readily evaluable by others. While qualitative scholars have long taken an interest in making their research open, reflexive, and systematic, the recent push for overarching transparency norms and requirements has provoked serious concern within qualitative research communities and raised fundamental questions about the meaning, value, costs, and intellectual relevance of transparency for qualitative inquiry. In this Perspectives Reflection, we crystallize the central findings of a three-year deliberative process—the Qualitative Transparency Deliberations (QTD)—involving hundreds of political scientists in a broad discussion of these issues. Following an overview of the process and the key insights that emerged, we present summaries of the QTD Working Groups’ final reports. Drawing on a series of public, online conversations that unfolded at www.qualtd.net, the reports unpack transparency’s promise, practicalities, risks, and limitations in relation to different qualitative methodologies, forms of evidence, and research contexts. Taken as a whole, these reports—the full versions of which can be found in the Supplementary Materials—offer practical guidance to scholars designing and implementing qualitative research, and to editors, reviewers, and funders seeking to develop criteria of evaluation that are appropriate—as understood by relevant research communities—to the forms of inquiry being assessed. We dedicate this Reflection to the memory of our coauthor and QTD working group leader Kendra Koivu.1
100 Olanzapine/Samidorphan for Schizophrenia: Weight Gain and Metabolic Outcomes in Phase 3 ENLIGHTEN-2 and Subsequent Long-Term, Open-Label Safety Study

Background:
Opioid antagonists may mitigate medication-associated weight gain and/or metabolic dysregulation. ENLIGHTEN-2 evaluated a combination of olanzapine and the opioid antagonist samidorphan (OLZ/SAM) vs olanzapine for effects on weight gain and metabolic parameters over 24 weeks in adults with stable schizophrenia.
METHODS:This phase 3, double-blind study (ClinicalTrials.gov: NCT02694328) enrolled adults 18–55 yo with stable schizophrenia, randomized 1:1 to once-daily OLZ/SAM or olanzapine. Co-primary endpoints were percent change from baseline in body weight and proportion of patients with ≥10% weight gain at week 24. Waist circumference and fasting metabolic parameters were also measured. Completers could enter a 52-week open-label safety extension.
RESULTS:561 patients were randomized: 550 were dosed, 538 had ≥1 post-baseline weight assessment, and 352 (64%) completed; 10.9% discontinued due to AEs. At week 24, least squares mean (SE) percent weight change from baseline was 4.21 (0.68)% with OLZ/SAM and 6.59 (0.67)% with olanzapine (difference, −2.38 [0.76]%; P=0.003). Fewer patients treated with OLZ/SAM (17.8%) had ≥10% weight gain vs olanzapine (29.8%; odds ratio=0.50; P=0.003). The change from baseline in waist circumference was significantly smaller with OLZ/SAM (P<0.001). Common AEs (≥10%) with OLZ/SAM and olanzapine were weight increased (24.8%, 36.2%), somnolence (21.2%, 18.1%), dry mouth (12.8%, 8.0%), and increased appetite (10.9%, 12.3%), respectively. Metabolic parameter changes were generally small and remained stable with long-term OLZ/SAM treatment.
DISCUSSION:OLZ/SAM treatment limited weight gain associated with olanzapine. Metabolic parameter changes were generally small, similar between groups over 24 weeks, and remained stable over an additional 52 weeks of open-label OLZ/SAM treatment.

Summary
Tumor angiogenesis is a key regulator of tumor growth and metastasis. Assays allowing the analysis of tumor angiogenesis are an essential tool to elucidate the role played by the tumor microenvironment in regulating tumor angiogenesis. The assays should also be capable of systematically investigating the effects of physiologically relevant, mechanical and chemical stimuli and their synergistic interactions. The high optical resolution of microfluidic assays facilitates three-dimensional studies of cellular morphogenesis. Their versatility can be applied to study the multi-parameter control of angiogenic factors.

Summary
Dynamic mechanical forces play a critical role in modulating cellular function, and inclusion of these extracellular stimuli in culture systems may improve the relevance and utility of biological results. In this work, we discuss recent advances made by our research group in applying dynamically controlled mechanical stimuli to cells cultured in 2-D and 3-D arrayed environments. Advantages in throughput and precision arising from microengineering such systems are demonstrated, with illustrative examples of potential biological applications. Engineering challenges associated with building these culture systems are explored, and the design and fabrication strategies that we have developed are discussed. Finally, the ability to incorporate additional sensing technologies into these dynamic screening platforms is explored.

Summary
Live cells can sense the mechanical characteristics of the microenvironment and translate the mechanical cues to intracellular biochemical signals in physiology and disease. To investigate intracellular signaling transduction during mechanosensing, nanotechnologies, and FRET live-cell imaging technologies have been developed to visualize the output signals in real time, such as intracellular molecular activity. Meanwhile, micropatterned technologies have been applied to modulate the physical and mechanical environment surrounding the cell to fine-tune the input signals in cellular mechanosensing. These advanced technologies can join forces and shed new light into the molecular networks that control mechanotransduction in normal conditions and disease.

The discovery of Neolithic houses at Durrington Walls that are contemporary with the main construction phase of Stonehenge raised questions as to their interrelationship. Was Durrington Walls the residence of the builders of Stonehenge? Were the activities there more significant than simply domestic subsistence? Using lipid residue analysis, this paper identifies the preferential use of certain pottery types for the preparation of particular food groups and differential consumption of dairy and meat products between monumental and domestic areas of the site. Supported by the analysis of faunal remains, the results suggest seasonal feasting and perhaps organised culinary unification of a diverse community.

Continuous fiber reinforced composites (RFC) are hierarchal and complex at multiple scales. In this work, tools are developed to automate the 3D characterization and quantification of the overall microstructure. Structure quantification enables accurate representation for material simulation and property prediction for the integrated computation materials engineering (ICME) of RFC based components. Relationships are developed to describe the key attributes of the microstructure at multiple scales including the individual fibers, tows, weave, porosity, and secondary matrix phases, which are treated as 'gestalts' of the structure. Here gestalt refers to the essence of shape or complete form of key features of the microstructure such as those of the tow architecture of the textile. Visualization tools are developed based on an artificial color scheme that allow the visual recognition of whole tows instead of just the collection of simple lines and curves representative of the fibers, which provides means whereby the gestalt of the microstructure can be visualized at the tow scale. These tools are demonstrated using a 3D dataset of the SiNC/SiC S200 ceramic matrix composite material (CMC) obtained via automated serial sectioning. Methods are then demonstrated to generate microstructure models representative of the characterized material for finite element analyses (FEA).
Maternal enhancement of cytotype regulation in Drosophila melanogaster by genetic interactions between telomeric P elements and non-telomeric transgenic P elements

The X-linked telomeric P elements (TPs) TP5 and TP6 regulate the activity of the entire P element family because they are inserted in a major locus for the production of Piwi-interacting RNAs (piRNAs). The potential for this cytotype regulation is significantly strengthened when either TP5 or TP6 is combined with a non-telomeric X-linked or autosomal transgene that contains a P element. By themselves, none of the transgenic P elements have any regulatory ability. Synergism between the telomeric and transgenic P elements is much greater when the TP is derived from a female. Once an enhanced regulatory state is established in a female, it is transmitted to her offspring independently of either the telomeric or transgenic P elements – that is, it works through a strictly maternal effect. Synergistic regulation collapses when either the telomeric or the transgenic P element is removed from the maternal genotype, and it is significantly impaired when the TPs come from stocks heterozygous for mutations in the genes aubergine, piwi or Su(var)205. The synergism between telomeric and transgenic P elements is consistent with a model in which P piRNAs are amplified by alternating, or ping-pong, targeting of primary piRNAs to sense and antisense P transcripts, with the sense transcripts being derived from the transgenic P element and the antisense transcripts being derived from the TP.

Summary
For some years, we have taught an introductory course in biomechanics within the Department of Mechanical and Industrial Engineering at the University of Toronto. We have been unable to find a textbook suitable for the purpose of introducing engineers and others having a “hard science” background to the field of biomechanics. That is not to say that excellent books on biomechanics do not exist; in fact, there are many. However, they are typically at a level that is too advanced for an introductory course, or they cover too limited a subset of topics for purposes of an introductory course.
This book represents an attempt to fill this void. It is not meant to be an extensive treatise on any particular branch of biomechanics, but rather to be an introduction to a wide selection of biomechanics-related topics. Our hope is that it will aid the student in his or her introduction to the fascinating world of bioengineering, and will lead some to pursue the topic in greater detail.
In writing this book, we have assumed that the reader has a background in engineering and mathematics, which includes introductory courses in dynamics, statics, fluid mechanics, thermodynamics, and solid mechanics. No prior knowledge of biology, anatomy, or physiology is assumed, and in fact every section begins with a review of the relevant biological background. Each chapter then emphasizes identification and description of the essential aspects of the related biomechanics problems.
