Although chemical and radiological agents cause toxicity through different mechanisms, the multiorgan injuries caused by these threats share similarities that convene on the level of basic biological responses. This publication will discuss these areas of convergence and explore “multi-utility” approaches that could be leveraged to address common injury mechanisms underlying actions of chemical and radiological agents in a threat-agnostic manner. In addition, we will provide an overview of the current state of radiological and chemical threat research, discuss the US Government’s efforts toward medical preparedness, and identify potential areas for collaboration geared toward enhancing preparedness and response against radiological and chemical threats. We also will discuss previous regulatory experience to provide insight on how to navigate regulatory paths for US Food and Drug Administration (FDA) approval/licensure/clearance for products addressing chemical or radiological/nuclear threats. This publication follows a 2022 trans-agency meeting titled, “Overlapping Science in Radiation and Sulfur Mustard Exposures of Skin and Lung: Consideration of Models, Mechanisms, Organ Systems, and Medical Countermeasures,” sponsored by the National Institute of Allergy and Infectious Diseases (NIAID), a part of the National Institutes of Health (NIH). Discussions from this meeting explored the overlapping nature of radiation and chemical injury and spurred increased interest in how preparedness for one threat leads to preparedness for the other. Herein, subject matter experts from the NIAID and the Biomedical Advanced Research and Development Authority (BARDA), a part of the Administration for Strategic Preparedness and Response (ASPR), summarize the knowledge gained from recently funded biomedical research, as well as insights from the 2022 meeting. These topics include identification of common areas for collaboration, potential use of biomarkers of injury to identify injuries caused by both hazards, and common and widely available treatments that could treat damage caused by radiological or chemical threats.

Purpose:
To summarize presentations and discussions from the 2022 trans-agency workshop titled “Overlapping science in radiation and sulfur mustard (SM) exposures of skin and lung: Consideration of models, mechanisms, organ systems, and medical countermeasures.”
Methods:Summary on topics includes: (1) an overview of the radiation and chemical countermeasure development programs and missions; (2) regulatory and industry perspectives for drugs and devices; 3) pathophysiology of skin and lung following radiation or SM exposure; 4) mechanisms of action/targets, biomarkers of injury; and 5) animal models that simulate anticipated clinical responses.
Results:There are striking similarities between injuries caused by radiation and SM exposures. Primary outcomes from both types of exposure include acute injuries, while late complications comprise chronic inflammation, oxidative stress, and vascular dysfunction, which can culminate in fibrosis in both skin and lung organ systems. This workshop brought together academic and industrial researchers, medical practitioners, US Government program officials, and regulators to discuss lung-, and skin- specific animal models and biomarkers, novel pathways of injury and recovery, and paths to licensure for products to address radiation or SM injuries.
Conclusions:Regular communications between the radiological and chemical injury research communities can enhance the state-of-the-science, provide a unique perspective on novel therapeutic strategies, and improve overall US Government emergency preparedness.

A national need is to prepare for and respond to accidental or intentional disasters categorized as chemical, biological, radiological, nuclear, or explosive (CBRNE). These incidents require specific subject-matter expertise, yet have commonalities. We identify 7 core elements comprising CBRNE science that require integration for effective preparedness planning and public health and medical response and recovery. These core elements are (1) basic and clinical sciences, (2) modeling and systems management, (3) planning, (4) response and incident management, (5) recovery and resilience, (6) lessons learned, and (7) continuous improvement. A key feature is the ability of relevant subject matter experts to integrate information into response operations. We propose the CBRNE medical operations science support expert as a professional who (1) understands that CBRNE incidents require an integrated systems approach, (2) understands the key functions and contributions of CBRNE science practitioners, (3) helps direct strategic and tactical CBRNE planning and responses through first-hand experience, and (4) provides advice to senior decision-makers managing response activities. Recognition of both CBRNE science as a distinct competency and the establishment of the CBRNE medical operations science support expert informs the public of the enormous progress made, broadcasts opportunities for new talent, and enhances the sophistication and analytic expertise of senior managers planning for and responding to CBRNE incidents.

Monocrystals and bicrystals of ice have been creep tested at temperatures between 4 and — 30°C. The bicrystals had a single grain boundary running parallel to the tensile axis; this configuration inhibited grain-boundary slip between the two grains. The creep tests, which were carried out at constant stress σ and temperature T, yielded data of strain ϵ for time elapsed since the start of the test. These data showed accelerating creep for both monocrystals and bicrystals at all strain levels. Strain-rates were derived at strains of 0.01, 0.05. and 0.10, and these rates were fitted to the expression
k is Boltzmann’s constant and E is the creep activation energy. Derived values of n were 1.9 for monocrystals and 2.9 for bicrystals. The creep activation energy was found to be 78 kJ/mol for monocrystals and 75 kJ/mol for bicrystals. The processes of creep deformation in mono-, bi- and polycrystals are discussed.

High-resolution (or “cross-correlation”) electron backscatter diffraction analysis (HR-EBSD) utilizes cross-correlation techniques to determine relative orientation and distortion of an experimental electron backscatter diffraction pattern with respect to a reference pattern. The integrity of absolute strain and tetragonality measurements of a standard Si/SiGe material have previously been analyzed using reference patterns produced by kinematical simulation. Although the results were promising, the noise levels were significantly higher for kinematically produced patterns, compared with real patterns taken from the Si region of the sample. This paper applies HR-EBSD techniques to analyze lattice distortion in an Si/SiGe sample, using recently developed dynamically simulated patterns. The results are compared with those from experimental and kinematically simulated patterns. Dynamical patterns provide significantly more precision than kinematical patterns. Dynamical patterns also provide better estimates of tetragonality at low levels of distortion relative to the reference pattern; kinematical patterns can perform better at large values of relative tetragonality due to the ability to rapidly generate patterns relating to a distorted lattice. A library of dynamically generated patterns with different lattice parameters might be used to achieve a similar advantage. The convergence of the cross-correlation approach is also assessed for the different reference pattern types.

Background:
The subcranial approach is a modification of traditional craniofacial resection. It provides similar broad access to the anterior skull base, but with lower mortality and morbidity. It has been the surgical technique of choice at our institution since 2006 for treating advanced stage sinonasal tumours (American Joint Committee on Cancer stage III or above). This paper reports our experience and outcomes.
Method and results:Eighteen patients underwent subcranial craniofacial resection over a seven-year period, this being combined with a second adjunctive procedure in 89 per cent of cases. Forty per cent of patients required reconstruction of the primary defect. No peri-operative deaths occurred. One patient had a transient cerebrospinal fluid leak. The major complication rate was 33 per cent, of which 67 per cent were directly related to soft tissue reconstruction. Tumour recurrence rate was 17 per cent and the five-year disease-free survival estimate was 40 per cent.
Conclusion:The subcranial approach is a safe and effective technique that may be used to successfully treat advanced sinonasal malignancies with anterior skull base extension.

This is the official guideline endorsed by the specialty associations involved in the care of head and neck cancer patients in the UK. In the absence of high-level evidence base for follow-up practices, the duration and frequency are often at the discretion of local centres. By reviewing the existing literature and collating experience from varying practices across the UK, this paper provides recommendations on the work up and management of lateral skull base cancer based on the existing evidence base for this rare condition.
Recommendations• Patients should be followed up to a minimum of five years with a prolonged follow-up for selected patients. (G)
• Patients should be followed up at least two monthly in the first two years and three to six monthly in the subsequent years. (G)
• Patients should be seen in dedicated multidisciplinary head and neck oncology clinics. (G)
• Patients should be followed up by dedicated multidisciplinary clinical teams. (G)
• The multidisciplinary follow-up team should include clinical nurse specialists, speech and language therapists, dietitians and other allied health professionals in the role of key workers. (G)
• Clinical assessment should include adequate clinical examination including fibre-optic rigid or flexible nasopharyngolaryngoscopy. (R)
• Magnetic resonance imaging and positron emission tomography combined with computed tomography imaging should be used when recurrence is suspected. (R)
• Narrow band imaging can be used in the follow-up in selected sites. (R)
• Second primary tumours should be part of rationale of follow-up and therefore adequate screening strategies should be used to detect them. (G)
• Patients should be educated with regard to the appearance and detection of recurrences. (G)
• Patients with persistent pain should be investigated to exclude recurrent disease. (R)
• Patients should be offered support with tobacco and alcohol cessation services. (R)

This is the official guideline endorsed by the specialty associations involved in the care of head and neck cancer patients in the UK. Surgery is one of the key modalities used in head and neck cancer treatment. Recent advances and a greater awareness of the short- and long-term toxicities associated with non-surgical modalities and newer technologies that permit minimal access resections have led to a resurgence in surgery. This paper provides an overview of the role of surgery in head and neck cancer practice.

This is the official guideline endorsed by the specialty associations involved in the care of head and neck cancer patients in the UK. There has been significant debate in the management of oropharyngeal cancer in the last decade, especially in light of the increased incidence, clarity on the role of the human papilloma virus in this disease and the treatment responsiveness of the human papilloma virus positive cancers. This paper discusses the evidence base pertaining to the management of oropharyngeal cancer and provides recommendations on management for this group of patients receiving cancer care.
Recommendations• Cross-sectional imaging is required in all cases to complete assessment and staging. (R)
• Magnetic resonance imaging is recommended for primary site and computed tomography scan for neck and chest. (R)
• Positron emission tomography combined with computed tomography scanning is recommended for the assessment of response after chemoradiotherapy, and has a role in assessing recurrence. (R)
• Examination under anaesthetic is strongly recommended, but not mandatory. (R)
• Histological diagnosis is mandatory in most cases, especially for patients receiving treatment with curative intent. (R)
• Oropharyngeal carcinoma histopathology reports should be prepared according to The Royal College of Pathologists Guidelines. (G)
• Human papilloma virus (HPV) testing should be carried out for all oropharyngeal squamous cell carcinomas as recommended in The Royal College of Pathologists Guidelines. (R)
• Human papilloma virus testing for oropharyngeal cancer should be performed within a diagnostic service where the laboratory procedures and reporting standards are quality assured. (G)
• Treatment options for T1–T2 N0 oropharyngeal squamous cell carcinoma include radical radiotherapy or transoral surgery and neck dissection (with post-operative (chemo)radiotherapy if there are adverse pathological features on histological examination). (R)
• Transoral surgery is preferable to open techniques and is associated with good functional outcomes in retrospective series. (R)
• If treated surgically, neck dissection should include levels II–IV and possibly level I. Level IIb can be omitted if there is no disease in level IIa. (R)
• If treated with radiotherapy, levels II–IV should be included, and possibly level Ib in selected cases. (R)
• Altering the modalities of treatment according to HPV status is currently controversial and should be undertaken only in clinical trials. (R)
• Where possible, patients should be offered the opportunity to enrol in clinical trials in the field. (G)

Background:
Previous literature reviews comparing transoral laser surgery versus radiotherapy for glottic carcinoma treatment have analysed ‘early stage’ disease as one group. The current review aimed to assess local control outcomes, comparing these two treatment modalities, specifically for either tumour stage 1a or stage 1b lesions.
Methods:The three authors conducted independent, structured literature searches. Simple weighted means were calculated.
Results:Thirty-six publications were analysed. Three-year local control rates for tumour stage 1a tumours were 88.9 per cent for transoral laser surgery (n = 1308) and 89.3 per cent for radiotherapy (n = 2405). For tumour stage 1b tumours, the local control rates were 76.8 per cent for transoral laser surgery (n = 194) and 86.2 per cent for radiotherapy (n = 492).
Conclusion:From this analysis of level four evidence, there was no demonstrable difference in local control rates for tumour stage 1a glottic squamous cell carcinoma treated by transoral laser surgery or radiotherapy. There was a trend towards improved local control of tumour stage 1b tumours treated with radiotherapy, but this finding was based on a limited number of published outcomes (n = 194).

A recently introduced hybrid Potts-phase field method has demonstrated the ability to evolve microstructures in conjunction with compositional fields tied to different phases. In this approach, Monte Carlo Potts methods are used to evolve the microstructure while phase field methods are used to evolve the composition, and the two fields are coupled through free energy functionals. Recent developments of the model allow different multi-component alloy systems to be simulated by using thermodynamic databases and kinetic quantities to dictate the behavior. An example of the method using the aluminum-silicon binary system is demonstrated.

Summary
Introduction
Malignancy of unknown origin, also referred to as occult primary or cancer with unknown primary (CUP), presents a diagnostic and therapeutic challenge. Approximately 10–15% of patients present with metastatic disease without an obvious primary tumor, and in many cases a primary tumor is not found even at autopsy (1). These include the paraneoplastic syndromes, where a patient presents with the humoral effects of an occult primary. In the absence of a known primary tissue of origin, the therapy is empiric and the outcome is generally poor. However, there are several favorable presentations that should be considered, and there are advances in personalized therapy that may improve outcomes. The role of FDG-PET/CT in cancer with an unknown primary is still being defined, but it is likely to play an increased role not only in the initial evaluation of these patients, but also in the assessment of response and in the direction of subsequent therapy.
Patients with an occult primary frequently present with general symptoms such as anorexia, weight loss, and fatigue, or with a localizing finding, such as lymphadenopathy, pathologic fracture, or a large fluid collection. The tumors in these patients are heterogeneous in histological appearance, but are characterized by their aggressiveness, propensity for early dissemination, and atypical metastatic pattern. The existence of metastatic disease at presentation is associated with a poor prognosis, and with a median survival of less than a year (2, 3). However, patients with only loco-regional disease have a better prognosis (4).

Summary
Homer
The life and times of the poet who created the Homeric epics are shrouded in mystery, as they have been since antiquity. He himself is partly to blame for this, in that he never mentions his name or gives any other personal information. The name Homer at some point in the seventh or sixth century bc came to be connected to the poems that are called Iliad and Odyssey (the titles are found for the first time we know of in Herodotus Histories 2.116), and more than one place in Ionia, most prominently Smyrna and Chios, claimed Homer as its native son. He was supposed to have lived at any time between the fall of Troy (traditionally placed in the twelfth century bc) and the seventh century. Some Lives of Homer are known from Roman imperial times, but they are worthless as historical sources because they are largely composed out of elements taken from the poems themselves (the boy Homer is taught by Phemius, a name suspiciously similar to that of the singer in Odysseus’ palace, and travels together with someone called Mentes, recalling Odysseus’ old friend and advisor of Telemachus, etc.). More than once it has even been suggested that Homer never existed; a recent proponent of this view argues that he was the creation of a group of professional performers called ‘the descendants of Homer (Homeridai)’, who thus endowed themselves with a mythical forefather. The name Homer, not common in Greek, would be their reinterpretation of the designation ὁμηρίδαι, which originally referred to professionals singing at a *ὅμαρος, ‘assembly of the people’. Conversely, some think there may have been two ‘Homers’, one composing the Iliad, the other the Odyssey.
Modern scholarship concurs with antiquity in placing Homer in Ionia, on account of the predominance of Ionian forms in his language; however, his dates remain contested. Can archaeology perhaps be of help? Here we must distinguish between the world created by Homer in his poems and the world in which Homer himself lived. As for the first, modern opinions vary between considering the setting of the Homeric epics by and large Mycenaean (1600–1200 bc), ‘dark age’ (1200–900 bc), eighth- or early seventh-century, or an amalgam.

Book XXII recounts the climax of the Iliad: the fatal encounter between the main defender of Troy and the greatest warrior of the Greeks, which results in the death of Hector and Achilles' revenge for the death of his friend Patroclus. At the same time it adumbrates Achilles' own death and the fall of Troy. This edition will help students and scholars better appreciate this key part of the epic poem. The introduction summarises central debates in Homeric scholarship, such as the circumstances of composition and the literary interpretation of an oral poem, and offers synoptic discussions of the structure of the Iliad, the role of the narrator, similes and epithets. There is a separate section on language, which provides a compact list of the most frequent Homeric characteristics. The commentary offers up-to-date linguistic guidance, and elucidates narrative techniques, typical elements and central themes.
