A Systematic Review of the Risks and Benefits of Venous Thromboembolism Prophylaxis in Traumatic Brain Injury

Background: Patients suffering from traumatic brain injury (TBI) are at increased risk of venous thromboembolism (VTE). However, initiation of pharmacological venous thromboprophylaxis (VTEp) may cause further intracranial hemorrhage. We reviewed the literature to determine the postinjury time interval at which VTEp can be administered without risk of TBI evolution and hematoma expansion. Methods: MEDLINE and EMBASE databases were searched. Inclusion criteria were studies investigating timing and safety of VTEp in TBI patients not previously on oral anticoagulation. Two investigators extracted data and graded the papers’ levels of evidence. Randomized controlled trials were assessed for bias according to the Cochrane Collaboration Tool and Cohort studies were evaluated for bias using the Newcastle-Ottawa Scale. We performed univariate meta-regression analysis in an attempt to identify a relationship between VTEp timing and hemorrhagic progression and assess study heterogeneity using an I2 statistic. Results: Twenty-one studies were included in the systematic review. Eighteen total studies demonstrated that VTEp postinjury in patients with stable head computed tomography scan does not lead to TBI progression. Fourteen studies demonstrated that VTEp administration 24 to 72 hours postinjury is safe in patients with stable injury. Four studies suggested that administering VTEp within 24 hours of injury in patients with stable TBI does not lead to progressive intracranial hemorrhage. Overall, meta-regression analysis demonstrated that there was no relationship between rate of hemorrhagic progression and VTEp timing. Conclusions: Literature suggests that administering VTEp 24 to 48 hours postinjury may be safe for patients with low-hemorrhagic-risk TBIs and stable injury on repeat imaging.

The Australian Square Kilometre Array Pathfinder (ASKAP) will give us an unprecedented opportunity to investigate the transient sky at radio wavelengths. In this paper we present VAST, an ASKAP survey for Variables and Slow Transients. VAST will exploit the wide-field survey capabilities of ASKAP to enable the discovery and investigation of variable and transient phenomena from the local to the cosmological, including flare stars, intermittent pulsars, X-ray binaries, magnetars, extreme scattering events, interstellar scintillation, radio supernovae, and orphan afterglows of gamma-ray bursts. In addition, it will allow us to probe unexplored regions of parameter space where new classes of transient sources may be detected. In this paper we review the known radio transient and variable populations and the current results from blind radio surveys. We outline a comprehensive program based on a multi-tiered survey strategy to characterise the radio transient sky through detection and monitoring of transient and variable sources on the ASKAP imaging timescales of 5 s and greater. We also present an analysis of the expected source populations that we will be able to detect with VAST.

The Lowland Maya “Protoclassic”: A reconsideration of its nature and significance

The term “Protoclassic,” employed regularly but inexplicitly in the literature of lowland Maya archaeology, has become increasingly nebulous and ambiguous in both meaning and usage. This paper reviews the history and use of the term and presents a formal redefinition of the Protoclassic as a ceramic stage based explicitly and exclusively on ceramic criteria. Some suggestions regarding future use of the term also are offered. The paper further addresses and resolves a number of persisting questions regarding Protoclassic orange wares, including problems concerning the actual existence of the “Aguacate ceramic group.” and the relationships of Aguacate-group pottery to other emergent orange wares of the terminal Late Preclassic and initial Early Classic periods. The nature and significance of the “Holmul I Style,” the “Floral Park Ceramic Sphere.” and the relationships of the two to each other and the larger, redefined “protoclassic” ceramic stage also are examined. A spatial distribution for protoclassic ceramics considerably expanded over what has ever been reported previously is described, and Chronometric data are presented to support a revised chronology for the protoclassic ceramic stage. Finally, ceramic data are offered that suggest a real subdivision of the protoclassic ceramic stage into an early, emergent facet originating entirely within Late Preclassic lowland traditions, and a later, fully “Classic” facet corresponding to the early Tzakol (Tzakol 1) ceramic horizon.

Antimicrobial resistance results in increased morbidity, mortality, and costs of health care. Prevention of the emergence of resistance and the dissemination of resistant microorganisms will reduce these adverse effects and their attendant costs. Appropriate antimicrobial stewardship that includes optimal selection, dose, and duration of treatment, as well as control of antibiotic use, will prevent or slow the emergence of resistance among microorganisms. A comprehensively applied infection control program will interdict the dissemination of resistant strains

Readers of Joyce's Ulysses are familiar with his references to the modality of the visible and the audible as well as with the vast range and importance of the audible in the work. But commentators apparently have not seen that the key both to these references and to Joyce's use of the audible in Ulysses is Aristotle's idea of perception and modality as pondered by Stephen at the beginning of the third or “Proteus” chapter. This key opens a new door into Joyce's world of ineluctable modality and reveals the importance of the modality of the audible both in the physical world without and the stream of consciousness within. Joyce's conception of modality and his treatment of sound are closely related. His use of the audible provides one of the best illustrations of his basic concept of modality, while this idea of modality is itself the best guide to an understanding of his experimentation with the audible.

A great deal of attention has been given to the surge of critical interest in metaphysical poetry that followed the publication of Herbert Grierson's edition of John Donne's poems in 1912. Theodore Spencer and Mark Van Doren, for instance, have examined the revival of the seventeenth-century metaphysical poets from 1912 to 1938. The achievement of these and of other recent critics has tended to obscure the earlier phase of the metaphysical revival. It has been assumed frequently that the revival began with Grierson's edition and that current theories about the sensibility reflected in metaphysical poetry were first presented in some essays by T. S. Eliot that appeared in the early 1920's. In reality, however, Grierson's edition and the reviews that acclaimed it marked the end of the first stage of the metaphysical revival. His edition was no doubt in part the cause of the enthusiasm about Donne that reached a scholarly climax in 1931 with the observance of the tercentenary of the poet's death. It was also the result of the increased interest in Donne's poetry and personality that began during the later decades of the nineteenth century. Similarly, Eliot's essays were not so much a new note as a sensitive formulation of ideas that had become familiar by 1912.
