How to improve clinical practice on involuntary hospital admissions of psychiatric patients: Suggestions from the EUNOMIA study

Number and procedures of involuntary hospital admissions vary in Europe according to the different socio-cultural contexts. The European Commission has funded the EUNOMIA study in 12 European countries in order to develop European recommendations for good clinical practice in involuntary hospital admissions. The recommendations have been developed with the direct and active involvement of national leaders and key professionals, who worked out national recommendations, subsequently summarized into a European document, through the use of specific categories. The need for standardizing the involuntary hospital admission has been highlighted by all centers. In the final recommendations, it has been stressed the need to: providing information to patients about the reasons for hospitalization and its presumable duration; protecting patients’ rights during hospitalization; encouraging the involvement of family members; improving the communication between community and hospital teams; organizing meetings, seminars and focus-groups with users; developing training courses for involved professionals on the management of aggressive behaviors, clinical aspects of major mental disorders, the legal and administrative aspects of involuntary hospital admissions, on communication skills. The results showed the huge variation of involuntary hospital admissions in Europe and the importance of developing guidelines on this procedure.

Estimates of heterosis and association of genetic distance with heterosis in durum wheat under different moisture regimes

The objectives of the present study were to evaluate heterosis for grain yield and yield components in durum wheat, and to assess the prediction potential of amplified fragment length polymorphism (AFLP) based and agronomic trait based genetic distances (GD and MD, respectively) to F1 performance, mid parent heterosis (MPH), and specific combining ability effects (SCA) under well-watered and moisture stress conditions. Six parental genotypes with different responses to moisture stress and their 15 F1 crosses were evaluated for their responses to moisture stress conditions in a glasshouse. Some cross combinations showed significant MPH for grain yield and yield components. The expression of heterosis for grain yield was greater under moisture stress conditions than under well-watered conditions. Cluster analysis of the parental lines based on agronomic performance under stress conditions was similar to cluster analysis result based on AFLP marker profiles. F1 performance was strongly correlated to both SCA effects and MPH under both stress and well water conditions. The correlation between SCA and MPH was very high under both treatment conditions for all traits. Correlation between GD and MD was significant only under stress conditions. Positive correlation was found only for the association between GD v. F1 performance and GD v. SCA effects for harvest index (HI) under well-watered conditions. None of the correlations between MD and SCA effects were significant. The absence of association between GD and heterosis for yield and most agronomic traits implied that heterozygosity per se diversity is not a good predictor of heterosis or F1 performance under both well-watered and stressed conditions.

Durum wheat genotypes with different responses to moisture stress were studied in a glasshouse under moisture stress and non-stress conditions to investigate differences in water use and transpiration efficiency and interrelationships among water use and transpiration efficiency and associated traits. Significantly high genotypic variability in the cumulative amount of water used before (ETba) and after (ETpa) anthesis was observed. Susceptible genotypes used higher amounts of water before anthesis and lower amounts after anthesis. In contrast, tolerant genotypes used a higher proportion of water during the post-anthesis period. Significantly high variability among the genotypes was observed for various measures of water use and transpiration efficiency, total dry matter and harvest index. Ranking of cultivars for water use efficiency based on grain yield (WUEG) and transpiration efficiency based on grain yield was consistent with ranking of cultivars for drought susceptibility indices. Drought susceptibility index was significantly but negatively correlated with harvest index, WUEG and grain yield. However, it was positively and significantly correlated with the ETba[ratio ]ETpa ratio. A high positive correlation of WUEG with harvest index and grain yield with harvest index was found. It was concluded that selection for lower ETba[ratio ]ETpa ratios up to about 0·8 could indirectly lead to improved WUEG and HI, hence improved grain yield. Selection for increased WUEG and/or grain yield would increase yield in water-limited environments.

Introduction
During the last ten years, there has been an increased interest in investigating the basic mechanisms of epilepsy in the immature brain. It was already known that the behavioral manifestations of seizures were age dependent but it was widely believed that in the immature central nervous system (CNS) inhibitory events predominated (Purpura & Housepian, 1961; Purpura, 1969, 1972). Therefore, it was assumed that the immature brain was not prone to seizures. However, the preponderance of recently acquired data indicates that the developing CNS is more susceptible to seizures than is the adult CNS. This concept is based on both epidemiological (Gibbs & Gibbs, 1963; Vernadakis & Woodbury, 1969; Hauser & Kurland, 1975; Woodbury, 1977) and experimental (Moshé et al., 1983; Albala et al., 1984; Schwartzkroin, 1984; Swann & Brady, 1984; Cavalheiro et al., 1987; Sperber & Moshé, 1988; Moshé, 1989) studies. Additional data have revealed that epileptogenesis during development may not be a linear event. There appear to be periods of increased seizure susceptibility, followed by relatively more resistant ‘developmental windows’ (Schwartzkroin, 1984; Moshé 1989; Moshé et al., 1992). Furthermore, since brain areas do not mature at the same time, epileptogenesis may differ from site to site.
