After almost two decades of dealing with asymmetric threats, America's strategic focus on technological superiority carefully honed during the Cold War, began to fracture, eroding its military and economic advantage. But China's recent emergence as a credible geopolitical rival has reinvigorated US efforts to sustain high-tech leadership as the basis of its military primacy. At the center of these efforts are the defense and defense-related agencies of the national security state (NSS) whose mission is to dominate the new technological frontiers of military power and achieve future competitive advantage. Alongside the quest for breakthroughs in foundational technologies, NSS agencies are seeking to correct long neglected deficiencies in advanced manufacturing, by rebuilding the industrial ecosystem's supply chains depleted after decades of offshoring. With this suite of initiatives, the US is following a course of action more consistent with the exercise of (economic) statecraft than industrial policy. However, unlike its authoritarian rival, the US faces the challenge of having to balance security imperatives with commercial interests and, not least, having to contend with a dysfunctional and maladapted presidency.
Variability in Antimicrobial Use Among Hospitals Participating in the Canadian Nosocomial Infection Surveillance Program

Background: The association between antimicrobial use (AMU) and emergence of antimicrobial resistance is well documented. The Canadian Nosocomial Infection Surveillance Program (CNISP) has conducted sentinel surveillance of AMU at participating Canadian hospitals since 2009 resulting in the largest pan-Canadian hospital database of dispensed antimicrobials. Objectives: Describe interhospital variability of AMU across Canada. Methods: Hospitals submit annual AMU data based on patient days (PD). Antimicrobials were measured in defined daily doses (DDD) for adults using the WHO Anatomical Therapeutic Chemical (ATC) system. The AMU data among pediatric patients have been available since 2017 using days of therapy (DOT). Surveillance includes systemic antibacterial agents (J01 ATC codes), oral metronidazole, and oral vancomycin. AMU was assessed using quintiles, interquartile ranges (IQR), and relative IQRs (upper- and lower-quartile values divided by the median). Results: Between 2009 and 2018, 20–26 hospitals participated in adult surveillance each year (35 teaching hospitals and 3 nonteaching hospitals participated in ≥1 year). Over this period, overall AMU decreased by 13% at participating adult hospitals from 645 to 560 DDD per 1,000 PD. AMU varied substantially between hospitals, but this variability decreased over time (Fig. 1). In 2009, the IQRs for overall AMU spanned 309 DDD per 1,000 PD, and in 2018 it spanned only 103 DDD per 1,000 PD. This decrease in variability was due to large decreases in use among hospitals with high use in 2009–2010. Among hospitals in the highest use quintile in 2009–2010, AMU decreased, on average, 44 DDD per 1,000 PD each year. Among hospitals in the lowest use quintile in 2009–2010, AMU increased, on average, 6 DDD per 1,000 PD each year. In 2018, antibiotics with the largest absolute IQR variability were cefazolin (61–113 DDD per 1,000 PD), piperacillin-tazobactam (32–64 DDD per 1,000 PD), and vancomycin (24–49 DDD per 1,000 PD). Among antibiotics with ≥1 DDD per 1,000 PD, antibiotics with the largest relative IQR variability were tobramycin (0.3–6 DDD per 1,000 PD), cefadroxil (0.08–9 DDD per 1,000 PD), and linezolid (0.2–3 DDD per 1,000 PD). In 2018, the IQR for overall pediatric AMU (n = 7 teaching hospitals) was 426–581 DOT per 1,000 PD. Antibiotics with the largest IQRs were vancomycin (0.6–58 DOT per 1,000 PD), cefazolin (33–88 DOT per 1,000 PD), and tobramycin (3–57 DOT per 1,000 PD). Among antibiotics with ≥1 DOT per 1,000 PD in 2018, antibiotics with the largest relative IQRs were tobramycin (3–57 DOT per 1,000 PD), cefuroxime (1–6 DOT per 1,000 PD), and amoxicillin (8–42 DOT per 1,000 PD). Conclusions: There is wide variation in overall antibiotic use across hospitals. Variation between AMU at adult hospitals has decreased between 2009 and 2018; in 2018, antibiotics with the largest IQRs were cefazolin and piperacillin-tazobactam. Benchmarking AMU is crucial for informing antimicrobial stewardship efforts.

Processes governing the evolution of planetesimals are critical to understanding how rocky planets are formed, how water is delivered to them, the origin of planetary atmospheres, how cores and magnetic dynamos develop, and ultimately, which planets have the potential to be habitable. Theoretical advances and new data from asteroid and meteorite observations, coupled with spacecraft missions such as Rosetta and Dawn, have led to major advances in this field over the last decade. This transdisciplinary volume presents an authoritative overview of the latest in our understanding of the processes of planet formation. Combining meteorite, asteroid and icy body observations with theory and modelling of accretion and orbital dynamics, this text also provides insights into the exoplanetary system and the search for habitable worlds. This is an essential reference for those interested in planetary formation, solar system dynamics, exoplanets and planetary habitability.

BACKGROUND
Bloodstream infection (BSI) due to methicillin-resistant Staphylococcus aureus (MRSA) is associated with considerable morbidity and mortality.
OBJECTIVETo determine the incidence of MRSA BSI in Canadian hospitals and to identify variables associated with increased mortality.
METHODSProspective surveillance for MRSA BSI conducted in 53 Canadian hospitals from January 1, 2008, through December 31, 2012. Thirty-day all-cause mortality was determined, and logistic regression analysis was used to identify variables associated with mortality.
RESULTSA total of 1,753 patients with MRSA BSI were identified (incidence, 0.45 per 1,000 admissions). The most common sites presumed to be the source of infection were skin/soft tissue (26.6%) and an intravascular catheter (22.0%). The most common spa types causing MRSA BSI were t002 (USA100/800; 55%) and t008 (USA300; 29%). Thirty-day all-cause mortality was 23.8%. Mortality was associated with increasing age (odds ratio, 1.03 per year [95% CI, 1.02–1.04]), the presence of pleuropulmonary infection (2.3 [1.4–3.7]), transfer to an intensive care unit (3.2 [2.1–5.0]), and failure to receive appropriate antimicrobial therapy within 24 hours of MRSA identification (3.2 [2.1–5.0]); a skin/soft-tissue source of BSI was associated with decreased mortality (0.5 [0.3–0.9]). MRSA genotype and reduced susceptibility to vancomycin were not associated with risk of death.
CONCLUSIONSThis study provides additional insight into the relative impact of various host and microbial factors associated with mortality in patients with MRSA BSI. The results emphasize the importance of ensuring timely receipt of appropriate antimicrobial agents to reduce the risk of an adverse outcome.

Despite the certainty with which our theories of industrial society chorused the demise of the small-scale economy, Italy'ssmall producers have not merely refused to disappear but have continued to nourish, multiply and predominate in the postwar economy. Most explanations for this phenomenon have centred oneconomic factors—whether the old linear notions of delayed development and backwardness (Fuà 1976; Sereni 1966), or the more fashionable centre-periphery model which attributes the survival of small-scale production to Italy's ‘latecomer’ status within the international division of labour (see, e.g., Paci 1979; Bagnasco 1977). In this paper I shall attempt to provide a non-economic explanation for the persistence and reproduction of Italy's small business stratum. The weight and importance of small enterprise in a given economy is not entirely explained by economic factors, since comparative statistics tell us that in many advanced economies petty production is by no means a marginal organisational form. Indeed they indicate that the processes of industrialism, far from generating homogenous and purgative effects, produce a diversity of conditions in which small firms can survive. On these and other empirical grounds, the paper argues that the differences observable in the Italian case are the product of particular social and institutional factors.

Summary
This chapter attempts two things. One is to show how Mann's theory of state power continues to generate new insights that advance the debates on state capacity, economic transformation, and globalization. The other is to suggest how these insights are at odds with some aspects of Mann's recent work on globalization.
Mann's theoretical and historical work on state power, as even the most cursory survey would reveal, has influenced scholars young and old across the social science disciplines. His theory of state power and the distinction between infrastructural and despotic power have offered particularly fertile soil for scholars of comparative politics and political economy. Many scholars of comparative politics have applied his concepts to explain failed or weak states in the developing world (e.g. Lucas 1998; Centeno 1997), to account for developmental blockages or breakthroughs in transition economies (Stoner-Weiss 2002; Zhu 2002), to revise conventional explanations for the rise of the West (Hall 1985), and to explain why some states, whether European or Asian, have been more effective than others at economic development (Weiss and Hobson 1995). In recent years, young sociologists have also sought to extend Mann's idea of the modern state's infrastructural power (IP) to symbolic and social infrastructures (e.g. Loveman). As the range of applications attests, Mann's theoretical and conceptual innovations continue to bear fruit.

In general, Mann's theory of state power has been most fruitfully applied to explain broad historical differences in types of state and contrasts in state autonomy.
