Acculturation, Hispanic ethnicity, and trust: Verifying and explaining racial/ethnic differences in trust in health providers in North Carolina Medicaid

Three North Carolina Medicaid surveys conducted from 2000 to 2012 reported increasing numbers of Hispanic children enrolled in Medicaid and much lower trust in providers expressed by their adult caregiver respondents compared with responses for non-Hispanic Black and White children. To verify and explain this apparent trust chasm, we used bivariate and regression analyses. The variables employed included trust (dependent variable); child’s race/ethnicity, age, and sex; satisfaction and health status scales; two utilization measures; respondent’s age, sex, and education; geographical region; and population density of county of residence. Race/ethnicity was strongly associated with trust (p < .001), controlling for other independent variables. Access, satisfaction, and respondent’s age and education were also significant. Our results fit the Behavioral Model for Vulnerable Populations, which maps the role of significant variables in health-seeking behavior. After analyzing the concept of trust, we argue that lower acculturation explains lower Hispanic trust compared with non-Hispanic Blacks. We suggest policies to improve acculturation.

ABSTRACT IMPACT: Identifying factors associated with opioid overdoses will enable better resource allocation in communities most impacted by the overdose epidemic. OBJECTIVES/GOALS: Opioid overdoses often occur in hotspots identified by geographic and temporal trends. This study uses principles of community engaged research to identify neighborhood and community-level factors associated with opioid overdose within overdose hotspots which can be targets for novel intervention design. METHODS/STUDY POPULATION: We conducted an environmental scan in three overdose hotspots’‘ two in an urban center and one in a small city’‘ identified by the Rhode Island Department of Health as having the highest opioid overdose burden in Rhode Island. We engaged hotspot community stakeholders to identify neighborhood factors to map within each hotspot. Locations of addiction treatment, public transportation, harm reduction programs, public facilities (i.e., libraries, parks), first responders, and social services agencies were converted to latitude and longitude and mapped in ArcGIS. Using Esri Service Areas, we will evaluate the service areas of stationary services. We will overlay overdose events and use logistic regression identify neighborhood factors associated with overdose by comparing hotspot and non-hotspot neighborhoods. RESULTS/ANTICIPATED RESULTS: We anticipate that there will be differing neighborhood characteristics associated with overdose events in the densely populated urban area and those in the smaller city. The urban area hotspots will have overlapping social services, addiction treatment, and transportation service areas, while the small city will have fewer community resources without overlapping service areas and reduced public transportation access. We anticipate that overdoses will occur during times of the day when services are not available. Overall, overdose hotspots will be associated with increased census block level unemployment, homelessness, vacant housing, and low food security. DISCUSSION/SIGNIFICANCE OF FINDINGS: Identifying factors associated with opioid overdoses will enable better resource allocation in communities most impacted by the overdose epidemic. Study results will be used for novel intervention design to prevent opioid overdose deaths in communities with high burden of opioid overdose.

Adequate income is a social determinant of health. In the United States, only Social Security beneficiaries receive inflation-protected guaranteed income. Social Security needs another 1983 compromise in which stakeholders accepted “shared pain” to avoid insolvency. We propose indexing the benefit using the chained consumer price index (CPI) for all urban consumers and providing a one-time bonus of 8% to 10% for beneficiaries in their mid-80s, when needs become greater. The chained CPI has little impact when beneficiaries start receiving benefits, but older beneficiaries need protection. The estimated 75-year savings from this restructured benefit amount to 14.2% to 18% of Social Security deficits. Modest increases in payroll taxes and maximum earnings taxed should make up most of the shortfall. Including unearned income with wages and salaries subject to the 6.2% individual tax would produce much more revenue. The discussion explores the proposal’s political feasibility, grounding in current policy and political science literature, and the role of income as a social determinant of health.

Twenty-five years ago, in November 1976, a physician misunderstood a cassette-tape providing continuing education for family practitioners to say that the rare neurological complication called Guillain-Barré syndrome could be a side effect of flu vaccines. When a recently vaccinated patient developed the syndrome, the physician alerted public officials and thereby started the process that ultimately ended the government campaign to immunize all Americans against swine flu. The physician was right, but for the wrong reasons, as Neustadt and Fineberg point out in the introduction to the 1983 edition of their classic case study of the swine flu episode (1983:xxv).

Analyses of American public opinion about abortion rarely consider what light other cases might shed on the U.S. controversy. This paper argues that there are striking parallels between the politics of abortion in the U.S. and Canada. These parallels extend beyond courts and interest groups; they also apply to public opinion. Gallup data indicate aggregate similarities in how both publics view the legalization of abortion in the last fifteen years, and World Values Survey data show that Americans and Canadians hold similar views about abortion even when the controversy is lifted out of its legal context. Probit analysis demonstrates that the same variables similarly predict where these publics stand on abortion when health and discretionary aspects of the debate are taken into account. Postmaterialist value orientations may provide important clues about what future direction the abortion debate in the U.S. may take.

This essay explores efforts to establish interdisciplinary research associations by comparing two organizations that were founded in the early 1980s. One has focused on the field of politics and the life sciences and the other on health services research. Both are involved in securing recognition for a research area—or “field of research”—that had not previously been conceptualized as a coherent academic or professional enterprise. The motivation for this paper is my interest in politics and the life sciences (the field), the organization—the Association for Politics and the Life Sciences that was established in 1980 to foster scholarly study of the field—and its journal Politics and the Life Sciences. (For the sake of clarity I adopt the convention of signifying a field entirely in lower-case orthography, beginning an organizational name with capital letters and naming the related journals in italics.)

The time between electronic-medical-record reporting of a positive result of a test for Clostridium difficile toxin in stool and the ordering of antimicrobial therapy was compared during consecutive periods when results were not telephoned (n = 274) and when results were telephoned (n = 90) to the clinical service. The mean times to the ordering of antimicrobial therapy were 11.9 and 3.6 hours, respectively (P < .001).

Our course differs from most comparable courses in ethics and public policy because we attract students from widely disparate programs. at the university of rochester, we put into the same small seminar master's students in a very quantitative public policy program dominated by microeconomics, community medicine students from the preventive medicine department's master's program in the medical school, selected undergraduate upperclassmen, and nursing students in nurse-clinician master's programs.
The faculty also have different backgrounds. One has taught politica philosophy in the Political Science Department for many years; the other (who had a primary appointment in the Preventive Medicine Department and a joint appointment in Political Science) taught health politics and policy, but had previously completed considerable training in philosophy and political theory.
