Environmental and patient-care control measures were initiated in response to an outbreak of legionellosis in an intensive care unit in 1992. The measures included maintaining elevated potable hot-water temperatures following superheating and using sterile water for administrations through nasogastric tubes. Legionella species remained below detectable levels in the potable hot-water system upon reevaluation in 1999. Nosocomial cases of legionellosis have not been reported since the outbreak.

This is a report of six cases of Clostridium difficile-associated diarrhea (CDAD) that occurred among cancer patients undergoing bone marrow transplantation in a tertiary-care cancer hospital. Specific infection control measures that were taken to minimize the nosocomial spread of CDAD also are discussed.

During a bitterly cold Minnesota weekend, I holed up at home with two dogs and hot tea while reading and rereading the paper that “everyone is talking about.” The findings of Grief and coworkers now have received world-wide exposure. In consequence, anesthesiologists, surgeons, and infection control workers may be pondering how soon to start “putting everybody on 80% oxygen in the OR and recovery room.” The article is not perfect—it has some warts—yet it may stimulate timely reconsideration of four intertwined topics: the difficulty of detecting an incremental infection prevention effect in a clinical trial; the importance of designing such a trial so that the control-group treatment represents current best practice; the bittersweet vagaries of inductive inference that are inescapable whenever we generalize to our practices (non-research conditions) techniques reported from clinical trials (research conditions); and the absence of logical rules of engagement to follow at our own hospitals when a new infection control gimmick or gadget arrives on the scene.

Of course, I have already submitted my e-mail letter to the editor of the New England Journal of Medicine. What I could not cover within the 400-word limit of that forum inspired this essay.

The apparent effect of only a few hours' supplemental oxygen administration for colorectal surgical-incision infection prevention was astounding. The 50% reduction in infection risk seems almost too good to be true. There is a seductive charm when a single, simple, relatively cheap and safe process change gives that much outcome improvement.

Methicillin-resistant Staphylococcus aureus nasal colonization was investigated in patients arriving for elective cardiovascular surgery, renal patients admitted for arteriovenous graft surgery, and patients transferred to our hospital from other institutions. Renal patients were significantly more likely to be colonized and represent a potential source of MRSA to our institution.

Routine use of mupirocin to prevent staphylococcal infections is controversial. We assessed attitudes and practices of healthcare professionals attending the Fourth Decennial International Conference on Nosocomial and Healthcare-Associated Infections regarding mupirocin prophylaxis. Eighty percent of participants did not use mupirocin routinely. At the end of the session, 58% indicated they would consider increased use of mupirocin.

We evaluated the efficacy of a commercial washer-pasteurizer. Carriers were inoculated with 104 to 106 test organisms and pasteurized at 170°F for 30 minutes. Pasteurization eliminated all test organisms with the exception of Bacillus subtilis spores. Pasteurization appears efficacious for the disinfection of respiratory-care equipment and could result in a cost savings of approximately $30,000 per year.

We compared, in three intensive care units, colonization of hubs with hub protection boxes or hubs with needleless closed connectors; 137 central venous catheters and 451 hubs were randomized in two groups with similar characteristics. Catheter and hub colonization were not different between the two groups. Among 30 colonized catheters, the same isolate was found in only two hubs; hub contamination rarely is responsible for catheter colonization in short-term catheters. Further studies are required to evaluate the benefit of protected hubs compared with unprotected hubs.

First identified in the late 1980s as the main causative agent of non-A, non-B hepatitis, hepatitis C virus (HCV) infection is now the most common chronic bloodborne infection in the United States. It is likely that the number of deaths attributable to HCV-related chronic liver disease will increase substantially during the next 2 decades, and the potential economic and clinical burden related to HCV is staggering. Accordingly, it is essential to identify risk factors for transmission of HCV and implement appropriate precautions to decrease the prevalence of this emerging infection. The relative importance of the two most common types of exposures associated with transmission of HCV, ie, blood transfusion and injection drug use, has evolved over time. Strict blood-donor selection and the use of reliable serological tests have led to a significant reduction in transfusion-associated risk. Injection drug use is currently the single most important risk factor for HCV infection in the United States.

Transmission of HCV poses a serious threat to healthcare workers and patients if infection control techniques or disinfection procedures are inadequate. Potential exposures include contact of the eyes, mucous membranes, broken skin, or needlestick injury. Hospitalized patients may serve as a reservoir for transmission, and the prevalence of anti-HCV seropositivity among such patients can be as high as 20%.

The infected healthcare worker is a potential source of transmission. A recent molecular phylogenetic analysis demonstrated that a cardiac surgeon likely transmitted HCV to five of his patients during open heart surgery. In summary, nosocomial transmission has been unequivocally confirmed and underscores the need for strict infection control practices.

An outbreak of nosocomial ringworm involved five infants in a neonatal intensive care unit. The index case was a nurse infected with Microsporum canis by her cat. After standard infection control measures were initiated, the outbreak was resolved successfully by an interdisciplinary professional collaboration of physician and veterinary dermatologists and infection control personnel.
