Summary
Patients with diabetes mellitus suffer from increased cardiovascular morbidity and mortality. The vascular endothelium forms the inner lining of blood vessels and serves as an important autocrine and paracrine regulator of vascular function. Loss of its modulatory function is thought to play a central role in the pathogenesis of late diabetic vascular complications, in both type 1 and type 2 diabetes. The increased flux of glucose under diabetic conditions results in the activation of various pathways that metabolize the excess of glucose, yet promote endothelial dysfunction. Clinical studies have consistently shown that intensive glycemic control delays the onset and progression of vascular complications in type 1 and, to a lesser extent, type 2 diabetes. Thus, hyperglycemia is an important mediator of endothelial dysfunction. However, additional factors appear to contribute to endothelial dysfunction in type 2 diabetes, and in certain patients with type 1 diabetes (e.g., those with diabetic nephropathy or obesity). In this chapter, we focus primarily on the effect of glucose on endothelial cells (ECs), but also refer to glucose-independent alterations of EC function in type 1 and type 2 diabetes.
EFFECTS OF HIGH GLUCOSE ON ENDOTHELIAL CELLS
Hyperglycemia-induced cellular changes influence and amplify each other; therefore, activation of ECs and subsequent endothelial dysfunction is the result of interacting cellular pathways rather than a single cellular event. Accumulating data support the hypothesis that oxidative stress is a common link, because several pathways seem to contribute to the oxidative stress associated with acute or chronic hyperglycemia (1–5). The polyol pathway is one of the possible biochemical mechanisms by which hyperglycemia could impair the function and structure of ECs (6).
