In this controlled study, we found that exposure to ultraviolet-C (UV-C) radiation was able to arrest the growth of selected pathogenic enteric and nonfermenting Gram-negative rods. Further studies are needed to confirm the clinical efficacy and determine optimal implementation strategies for utilizing UV-C terminal disinfection.
An Exploratory Evaluation of Barriers to Study Leave Application Amongst Psychiatric Trainees in the West Midlands

Aims
Between 2018–2021 there was a downward trend in study leave applications and the total spending on trainee study leave activities. There was concern that trainees may not be maximising educational opportunities. The authors were aware of anecdotal evidence indicating barriers to applying for study leave and therefore sought to objectively explore this. We aimed to evaluate qualitative and quantitative responses amongst psychiatric trainees within the West Midlands deanery regarding study leave applications. We also endeavoured to identify positive aspects and barriers, to improve knowledge and confidence in the process and identify areas for improvement in making the system more accessible.
MethodsThe authors met with Health Education West Midlands to clarify current processes and gather objective data regarding study leave applications between 2018 and 2021. A survey was distributed to all Psychiatric trainees in the West Midlands in December 2021. The survey was open for three weeks and contained closed and open questions. Data were analysed and a thematic analysis was completed independently by the authors to allow for triangulation.
ResultsThere were 62 responses (response rate of 27%) from trainees ranging from CT1-ST7+. 55% were unclear about the study leave application process, and of these, 79% said this had prevented them from applying. Only 37% of trainees found the process ‘very’ or ‘moderately easy’, with 23% finding it ‘very difficult’. When exploring barriers, worryingly 69% of trainees did not know where to find the list of approved courses. Other themes included too many signatures being required, long delays in forms being returned and money being reimbursed.
ConclusionThe majority of respondents were unclear about the study leave process or found it difficult. This acts as a barrier to application in the majority of cases and may have a knock-on effect on the overall quality of training. By identifying these barriers, we are now able to address these more directly.
The results were presented at a deanery wide level leading to a better understanding of the reduction in spending of study leave funding. Phase two of the project will see the introduction of an electronic application system, aiming for an easier and shorter process, in addition to creating consistency across trusts. There may have been some confounding factors, such as COVID-19 that may have contributed to the decline in study budget being utilised.
The feasibility of a group stress management Liver SMART intervention for patients with end-stage liver disease: A pilot study

Objective
Structured, empirically supported psychological interventions are lacking for patients who require organ transplantation. This stage IA psychotherapy development project developed and tested the feasibility, acceptability, tolerability, and preliminary efficacy of an 8-week group cognitive behavioral stress management intervention adapted for patients with end-stage liver disease awaiting liver transplantation.
MethodTwenty-nine English-speaking United Network for Organ Sharing–registered patients with end-stage liver disease from a single transplantation center enrolled in 8-week, group cognitive-behavioral liver stress management and relaxation training intervention adapted for patients with end-stage liver disease. Patients completed pre- and postintervention surveys that included the Beck Depression Inventory II and the Beck Anxiety Inventory. Feasibility, acceptability, tolerability, and preliminary efficacy were assessed.
ResultAttendance rate was 69.40%. The intervention was rated as “good” to “excellent” by 100% of participants who completed the postintervention survey in teaching them new skills to relax and to cope with stress, and by 94.12% of participants in helping them feel supported while waiting for a liver transplant. No adverse events were recorded over the course of treatment. Attrition was 13.79%. Anxious and depressive symptoms were not statistically different after the intervention.
Significance of resultsThe liver stress management and relaxation training intervention is feasible, acceptable, and tolerable to end-stage liver disease patients within a transplant clinic setting. Anxious and depressive symptoms remained stable postintervention. Randomized controlled trials are needed to study the intervention's effectiveness in this population.

Radiocarbon accelerator mass spectrometric (AMS) dates on the acid-insoluble fraction from 38 core tops from the western Ross Sea, Antarctica, are used to address these questions: (1) What are the apparent ages of sediments at or close to the present sediment/water interface? (2) Is there a statistically significant pattern to the spatial distribution of core top ages? and (3) Is there a “correction factor” that can be applied to these age determinations to obtain the best possible Holocene (downcore) chronologies? Ages of core top sediments range from 2000 to 21,000 14C yr B.P. Some “old” core top dates are from piston cores and probably represent the loss of sediment during the coring process, but some core top samples >6000 14C yr B.P. may represent little or no Holocene deposition. Four possible sources of variability in dates ≤6000 14C yr B.P. (n = 28) are associated with (1) different sample preparation methods, (2) different sediment recovery systems, (3) different geographic regions, and (4) within-sample lateral age variability. Statistical analysis on an a posteriori design indicates that geographic area is the major cause of variability; there is a difference in mean surface sediment age of nearly 2000 yr between sites in the western Ross Sea and sites east of Ross Bank in south-central Ross Sea. The systematic variability in surface age between areas may be attributed to: (a) variable sediment accumulation rates (SAR) (surface age is inversely related to SAR), (b) differences in the percentage of reworked (dead) carbon between each area, and/or (c) differences in the CO2 exchange between the ocean and the atmosphere.
Factors contributing to economic burden in lung cancer spousal caregivers

Objective:
The determinates of economic burden in lung cancer caregivers are poorly understood. Of particular interest is the role patient symptoms play in caregiver economic burden. Guided by a stress process conceptual framework, this study examined the predictors of economic burden reported by lung cancer spousal caregivers. Our study focused on the pathway of contextual and stressor variables leading to economic burden in lung cancer caregivers.
Method:Relying on survey data from 138 spouses, structural equation modeling was employed to examine the determinants of economic burden measured using the Family Impact Survey. Contextual variables included age, gender, education, and income; and stressor variables included patient physical and mental symptoms, as well as number of children in the home.
Results:A significant indirect path between age and economic distress through patient symptoms (p = 0.05) indicates younger spouses providing care for patients with more symptoms and reporting greater economic burden. Direct effects between contextual variables and economic burden revealed that caregivers with less education (p = 0.02) and those with more children at home (p = 0.01) reported more adverse economic outcomes.
Significance of Results:Numerous factors impact spousal caregivers' economic burden, including the presence of children at home, being a younger caregiver, and lower educational attainment by caregivers. Moreover, the direct effects between age and economic burden were not significant, supporting the clear role patient symptoms play in the path to economic burden in spousal caregivers. These results underscore the need for healthcare providers to address psychosocial factors when dealing with patients and families with lung cancer. Specifically, the results highlight the importance of addressing patient symptoms early before they threaten the family's economic well-being.

Diatom assemblages in surface sediments of the Ross Sea: relationship to present oceanographic conditions

Fifty four surface sediment samples from the western and central Ross Sea were examined to determine relationships between modern oceanographic conditions and surface sediment diatom assemblages. A centered R mode principal components analysis demonstrates four geographically distinct assemblages. The assemblage just north of the Ross Ice Shelf in the central Ross Sea is most closely associated with Stephanopyxis spp. (a heavily silicified diatom abundant during the Pliocene), and may result from a combination of winnowing/reworking, and modern flux of primarily non-siliceous algae. The algal assemblage in the western part of the central Ross Sea is most closely associated with Thalassiosira gracilis (an open water diatom), and reflects early seasonal pack ice break up during the late spring inception of the Ross Sea polynya. The algal assemblage north of Drygalski Ice Tongue, in the western Ross Sea, is most closely associated with Fragilariopsis curta (a diatom common in stratified ice edge zones), suggesting that water column seeding by species melting out of coastal sea ice is important in this area. The assemblage south of Drygalski Ice Tongue is most closely associated with resting spores of Thalassiosira antarctica (a diatom associated with coastal waters). Although the habitat of T. antarctica requires future research, we speculate that sea ice conditions unique to area B support an autumnal T. antarctica bloom.
