Exploring the experience of a cognitive rehabilitation intervention for cancer-related cognitive change in people living with cancer: An interpretative phenomenological analysis

Objectives
Some cancer patients experience cancer-related cognitive change (CRCC). Cognitive rehabilitation interventions (CRIs) have recently been developed to help mitigate the impact of CRCC, which, untreated, can impact resumption of daily life post-cancer treatment. The experience of participants is important to understand but largely absent within research literature. This study aimed to explore how those with CRCC experience the phenomenon following completion of a CRI.
MethodsThis study comprised a qualitative phenomenological approach. This involved conducting in-depth, semi-structured interviews with 6 self-referred participants from one CRI. Participants were invited to discuss their experience of CRCC and what the CRI therefore meant to them. Interviews were analyzed using interpretative phenomenological analysis.
ResultsAnalysis of the findings revealed 4 key themes. (1) “Experiencing and addressing isolation” comprises reflections on posttreatment perceived abandonment and consequent feelings of belonging through CRI participation. (2) “Identity” explores participants’ reflections around perceived loss-of-self and feelings of empowerment from the intervention. (3) “Cognitive and physical balance” comprises the planning and choices participants make, supported by both their own and CRI coping strategies as they seek acceptance of cognitive change. (4) “Course reflections” explore reflections on intervention structure, format, and delivery, focusing on 2 subthemes of accessibility, flexibility and inclusivity, and communication. All participants reflected positively on their experience.
Significance of resultsResults support further dissemination among health professionals and implementation of this CRI to better support self-reported CRCC concerns within this population. Future qualitative research should explore the long-term impact of CRI interventions.
Predicting psychosis risk using a specific measure of cognitive control: a 12-month longitudinal study

Background
Identifying risk factors of individuals in a clinical-high-risk state for psychosis are vital to prevention and early intervention efforts. Among prodromal abnormalities, cognitive functioning has shown intermediate levels of impairment in CHR relative to first-episode psychosis and healthy controls, highlighting a potential role as a risk factor for transition to psychosis and other negative clinical outcomes. The current study used the AX-CPT, a brief 15-min computerized task, to determine whether cognitive control impairments in CHR at baseline could predict clinical status at 12-month follow-up.
MethodsBaseline AX-CPT data were obtained from 117 CHR individuals participating in two studies, the Early Detection, Intervention, and Prevention of Psychosis Program (EDIPPP) and the Understanding Early Psychosis Programs (EP) and used to predict clinical status at 12-month follow-up. At 12 months, 19 individuals converted to a first episode of psychosis (CHR-C), 52 remitted (CHR-R), and 46 had persistent sub-threshold symptoms (CHR-P). Binary logistic regression and multinomial logistic regression were used to test prediction models.
ResultsBaseline AX-CPT performance (d-prime context) was less impaired in CHR-R compared to CHR-P and CHR-C patient groups. AX-CPT predictive validity was robust (0.723) for discriminating converters v. non-converters, and even greater (0.771) when predicting CHR three subgroups.
ConclusionsThese longitudinal outcome data indicate that cognitive control deficits as measured by AX-CPT d-prime context are a strong predictor of clinical outcome in CHR individuals. The AX-CPT is brief, easily implemented and cost-effective measure that may be valuable for large-scale prediction efforts.

A near complete shell from the Hemphillian 4 (Miocene/Pliocene boundary) Buis Ranch local fauna of Beaver County, Oklahoma, represents a fossil box turtle. An anterior contact of neural III and neural V with costal III and costal V only, respectively, presence of a small contact between the suprapygal and eleventh peripherals, development of a thin peripheral lip for articulation with the posterior plastral lobe, placement of the vertebral III/IV sulcus on neural VII, presence of two anterior musk duct glands, a rounded posterior plastral lobe, an elongate shell outline, and a complete neural series diagnose the fossil as a new species, Terrapene parornata n. sp. A phylogenetic analysis of fossil box turtles places T. parornata along the phylogenetic stem of the extant taxon T. ornata. The holotype of ‘Terrapene longinsulae’ cannot be distinguished from Terrapene ornata and is therefore synonymized. Finally, ‘Terrapene’ corneri lacks characters of crown group Terrapene and may therefore represent a stem box turtle. The provenance of the holotype of ‘Terrapene longinsulae’ is more poorly known than previously recognized and this specimen may originate from Kansas or Nebraska and be early Miocene to late Pleistocene in age. Terrapene parornata is therefore the oldest demonstrable representative of crown group Terrapene (ca. 5.3–4.6 Ma). ‘Terrapene’ corneri from the late Barstovian of Nebraska and fragmentary material from the middle Barstovian of Nebraska by contrast are the oldest representative of the Terrapene lineage (ca. 14.5–11.5 Ma). A review of morphological characters related to shell kinesis reveals that most are highly correlated. The results of the phylogenetic analysis converge upon those of molecular data when these correlated characters are omitted from the analysis.

Nosocomial Infection Rates at an Oncology Center

Nosocomial infection rates were computed for 5,031 patients at an oncology center during a 20-month period. Twelve percent of the patients developed nosocomial infections, accounting for a total of 802 infections. The overall incidence of nosocomial infections during this study period was 6.27 infections per 1,000 patient days. The highest incidence of nosocomial infections was found in patients having acute myelogenous leukemia (30.49 infections per 1,000 patient days); bone and joint cancer (27.27 infections per 1,000 patient days); and liver cancer (26.58 infections per 1,000 patient days). The respiratory tract was the most common site of infection, followed by blood-stream, surgical wound, and urinary tract infections. Staphylococcus aureus, Escherichia coli, Pseudomonas aeruginosa, Klebsiella pneumoniae, and coagulase-negative staphylococci were most frequently implicated as pathogens. The distribution of specific types of infection according to underlying malignancy was also tabulated. These data provide nosocomial infection rates, common pathogens, and sites of infection for cancer patients, thus assisting in directing appropriate therapy for these patients.
