Human milk, breastfeeding, and early neurodevelopmental outcomes for infants with critical CHD

Human milk improves neurodevelopment for preterm infants, but relationships between human milk and neurodevelopment for infants with critical CHD are unknown. We aimed to (1) explore associations between human milk/direct breastfeeding and neurodevelopment at 1-year and 2-year follow-up and (2) describe patterns of human milk (maternal, donor) and commercial formula during hospitalisation in the first year of life.
This retrospective cohort study included infants who underwent surgery for CHD < 6 months old. The primary outcome was neurodevelopment via Bayley Scales of Infant Development-IV. Analysis included adjusted linear regression for associations between exclusive human milk while inpatient during the first 6 months or any direct breastfeeding while inpatient during the first year of life and 1-year Bayley-IV scores. Models were adjusted for race, insurance type, genetic diagnosis, and length of stay.
Of 98 eligible infants, 40% followed up at 1 year; 27% at 2 years. There were differences in follow-up related to demographics (race, ethnicity) and social determinants of health (insurance type, distance from clinic). In adjusted models, infants who directly breastfed had 13.18 points higher cognition (95% CI: 0.84–25.53, p = 0.037); 14.04 points higher language (2.55–25.53, p = 0.018); and 15.80 points higher motor scores (3.27–28.34, p = 0.015) at 1-year follow-up. Infants fed exclusive human milk had 12.64 points higher cognition scores (−0.53–25.82, p = 0.059).
Future investigation into nutrition and neurodevelopment in the context of critical CHD is warranted. As neurodevelopmental follow-up becomes standard of care in this population, efforts are needed to mitigate disparities in access to this care.

We applaud Goldin-Meadow & Brentari's (G-M&B's) significant efforts to consider the linkages between sign, gesture, and language. Research on deaf children and sign language acquisition can broaden the G-M&B approach by considering how language readiness is also a social phenomenon and that distinctions between imagistic and categorical formats rely on language practices and contexts.

Summary
Introduction
Much has been written on GVCs over the past few years. However, this literature has often gone in different directions, resulting in inconsistent analysis using different terminology. This has tended to muddy the waters. Authors have discussed the key concept of ‘governance’ in different contexts with alternate meanings in relation to the operation of GVCs, which has resulted in confusion for analysts and practitioners of trade policy.
Since the focus of authors has often been different, there has been no consensus around the notion of ‘governance’ of GVCs. Many economists have written about governance of value chains with what, in this chapter, we are going to call ‘g’. Our definition covers industrial and national policy frameworks from the perspective of firms and individual countries.
Note that the term ‘governance’ is also being used to refer more broadly to the WTO, the entire global trading system, and its relationship to this new phenomenon. In this chapter we call the latter ‘G’, as it is more concerned with systemic issues.
The aim of this chapter is to bring some clarity and shed light on the various perspectives that have been used by economists in the GVC literature and to remedy terminological confusion when talking about governance with respect to GVCs. This should remove some of the misunderstandings that have been generated by discussing the same term with underlying reference to different contexts.
A typology of GVC governance
We propose to set out a typology of GVC governance in three different categories:
• industrial policy frameworks and the governance of GVCs from a firm perspective;
• national policy frameworks and governance of GVCs from an individual country perspective; and
• global trading system framework and governance of GVCs from the WTO perspective and/or that of major preferential trade agreements.
For the purpose of this chapter, governance of GVCs from the firm and individual country perspectives will be discussed with a small ‘g’ while governance of GVCs from the perspective of the multilateral trading system or multiparty trade agreements will be discussed with a capital ‘G’. This is because in the first and second cases governance is used in a micro sense, applied to either firm behaviour or national government policy, while in the third case governance is used in a broader, systemic sense as affecting all economic actors in the world economy or in a regional economic grouping.

Physical activity and prevalence and incidence of mental disorders in adolescents and young adults

Background
Although positive effects of physical activity on mental health indicators have been reported, the relationship between physical activity and the development of specific mental disorders is unclear.
MethodA cross-sectional (12-month) and prospective-longitudinal epidemiological study over 4 years in a community cohort of 2548 individuals, aged 14–24 years at outset of the study. Physical activity and mental disorders were assessed by the DSM-IV Composite International Diagnostic Interview (CIDI) with an embedded physical activity module. Multiple logistic regression analyses controlling for age, gender and educational status were used to determine the cross-sectional and prospective associations of mental disorders and physical activity.
ResultsCross-sectionally, regular physical activity was associated with a decreased prevalence of any and co-morbid mental disorder, due to lower rates of substance use disorders, anxiety disorders and dysthymia. Prospectively, subjects with regular physical activity had a substantially lower overall incidence of any and co-morbid mental disorder, and also a lower incidence of anxiety, somatoform and dysthymic disorder. By contrast, the incidence of bipolar disorder was increased among those with regular physical activity at baseline. In terms of the population attributable fraction (PAF), the potential for preventive effects of physical activity was considerably higher for men than for women.
ConclusionsRegular physical activity is associated with a substantially reduced risk for some, but not all, mental disorders and also seems to reduce the degree of co-morbidity. Further examination of the evidently complex mechanisms and pathways underlying these associations might reveal promising new research targets and procedures for targeted prevention.

Summary
Abstract
Although the problem of the existence of a resolution of singularities in characteristic zero was already proved by Hironaka in the 1960s and although algorithmic proofs of it have been given independently by the groups of Bierstone and Milman and of Encinas and Villamayor in the early 1990s, the explicit construction of a resolution of singularities of a given variety is still a very complicated computational task. In this article, we would like to outline the algorithmic approach of Encinas and Villamayor and simultaneously discuss the practical problems connected to the task of implementing the algorithm.
Introduction
The problem of existence and construction of a resolution of singularities is one of the central tasks in algebraic geometry. In its shortest formulation it can be stated as: Given a variety X over a field K, a resolution of singularities of X is a proper birational morphism π : Y → X such that Y is a non-singular variety.
Historically, a question of this type has first been considered in the second half of the 19th century – in the context of curves over the field of complex numbers. It was already a very active area of research at that time with a large number of contributions (of varying extent of rigor) and eventually lead to a proof of existence of resolution of singularities in this special situation at the end of the century.

Incorporation of ligands and metals into polymers by controlled methods yields multifunctional responsive materials. Dibenzoylmethane (dbm) is a beta-diketone ligand that binds metal and metalloid ions such as boron, iron or europium in a bidentate fashion producing complexes with one or more dbm chelates. Modification of dbm with biocompatible poly(lactic acid) (PLA) allows materials processing and leads to responsive metallobiomaterials. This is accomplished by modification of dbm with hydroxyl groups (e.g. dbmOH), that serve as initiators for lactide polymerization. Improved control is noted when dbm is protected as iron or boron complexes, namely Fe(dbmOH)3 or BF2(dbmOH). Furthermore, the iron center in iron tris(dbmOH) also functions as a catalyst for the polymerization; no tin catalyst is required. Redorange iron-centered three arm star polymers, Fe tris(dbmPLA), are obtained. Acid sensitivity of these materials provides a method for dbm ligand dissociation, demetalation and production of dbmPLA macroligands for coordination to other metals such as Eu. Currently, dbmPLA and its polymeric iron complexes are being fabricated as nanoparticles. Light emitting B and Eu dbm complexes are also under investigation. Progress in the synthesis, characterization and nanoscale fabrication of dbm ligand and metal based polymeric biomaterials is reported.

What do we find at the origin of the work which does most honour to mankind, at the roots of a movement which has been spread across the world for the past hundred years? There is of course one man, Henry Dunant, but there is also a book. A slight volume which was soon to stir the hearts and consciences of Europe from end to end, by its fluent style, its stark reality and its warmhearted appeal. This was A Memory of Solferino.
