Risk for primary cephalosporin resistance in Gram-negative bacteremia

Objective:
This study aimed to examine the clinical risk factors for cephalosporin resistance in patients with Gram-negative bacteremia caused by Escherichia coli (EC), Klebsiella pneumoniae (KP), Enterobacter cloacae (ENC), and Pseudomonas aeruginosa (PS).
Methods:This retrospective cohort study included 400 adults with Gram-negative bacteremia. The goal was to review 100 cases involving each species and approximately half resistant and half susceptible to first-line cephalosporins, ceftriaxone (EC or KP), or cefepime (ENC or PS). Logistic regression was used to identify factors predictive of resistance.
Results:A total of 378 cases of Gram-negative bacteremia were included in the analysis. Multivariate analysis identified significant risk factors for resistance, including admission from a chronic care hospital, skilled nursing facility, or having a history of infection within the prior 6 months (OR 3.00, P < .0001), requirement for mechanical ventilation (OR 3.76, P < .0001), presence of hemiplegia (OR 3.54, P = .0304), and presence of a connective tissue disease (OR 3.77, P = .0291).
Conclusions:Patients without the identified risk factors should be strongly considered for receiving ceftriaxone or cefepime rather than carbapenems and newer broad-spectrum agents.

In this study, we investigate directly bonded germanium-silicon interfaces to facilitate the development of high quality germanium silicon integration for Avalanche photodiode application. Angle resolved x-ray photoelectron spectroscopy data is presented which provides the chemical composition of the germanium surfaces as a function of the surface passivation. The hetero-structure is characterized by measuring forward and reverse current and comparing the measured results to TCAD simulation. The physical structure of hetero-junction is supported by high resolution transmission electron microscopy.

In a group of 32 adult volunteers given subunit influenza virus vaccine containing 250 international units (i.u.) of A/Victoria/3/75, 250 i.u. of A/Scotland/ 840/74 and 300 i.u. of B/Hong Kong/8/73, there were substantial increases in the geometric mean homologous haemagglutination-inhibiting (HI) antibody titres. There was also substantial boosting of the antibodies to the earlier variants of the Hong Kong (H3N2) series and to a later variant of the Asian (H2N2) series. There was no boosting of antibodies to the A/FM/1/47 strain, a representative member of the H1N1 series, but two individuals showed substantial rises to A/PR/8/34 (H0N1).
There were increases in the HI titre of antibodies cross reactive with two recent isolations, A/Texas/1/77, and A/Victoria/35/77, but the majority of vaccinees failed to reach antibody titres likely to be protective against such strains.
Use of balloon expandable stents in the palliative relief of obstructed right ventricular conduits

Objectives
The purpose of this study was to determine the efficacy of balloon expandable stents in the relief of obstructed conduits in the right heart, and to review both the immediate success and longevity of this palliation.
Methods and resultsEndovascular stents where placed in 17 children, median age 4.7 yrs (range 0.46–16.2 yrs), at a median of 1.6 yrs (range 0.33–6.7 yrs) after placement of conduits at surgery. A second stent was required in two children, either because of migration of the initial stent or residual proximal stenosis. Mean right ventricular to systemic pressure ratios were reduced from 0.77 ±0.24 to 0.50±0.13, and the minimum area of the pulmonary outflow tract increased from 24±12% to 69±26% of that expected for body surface area. Further surgery was needed in 8 patients following placement of the stent, in 5 within 10 months of implantation, due to inadequate relief of obstruction. Nonetheless, 3 of these 8 children achieved satisfactory palliation 18, 21 and 24 months prior to requiring insertion of a new conduit. Actuarial freedom from reoperation was 44% at 24 months. The remaining 9 children continue to benefit from the relief in obstruction provided by the stent at a mean interval from implantation of 24±8 months. In this group, the mean Doppler gradient across the right ventricular outflow tract was 35 ±13 mmHg (n=6).
ConclusionsThese early data indicate that endovascular stents are useful in the palliation of obstructed right ventricular conduits. In the short-term, the relief of obstruction achieved is well maintained, and has either delayed or obviated the need for insertion of new conduits.

Summary
Three groups of mammals that occur in coniferous forests of western North America are closely associated with large healthy, decaying, or dead trees: bats, arboreal rodents, and forest carnivores. Detailed descriptions of the ecological relations of these species are presented elsewhere in this book (Buskirk and Zielinski 2003, Hayes 2003, Smith et al. 2003). Although many other kinds of mammals use large vertical forest structures to some degree, these are the species groups that depend on them to meet their life history requirements. Consequently, these are also the mammals that are most likely to suffer population declines in forests where these structures are reduced in abundance. The need to provide for large snags and decadent trees in managed forests to maintain populations of cavity-using birds and mammals has received much attention in the literature (e.g., Balda 1975, Thomas 1979, Hoover and Wills 1984, Brown 1985). However, the perceived consequences of providing inadequate numbers and sizes of these structures in managed forests have generally been limited to the decline or loss of the wildlife species that depend on them. The broader ecological consequences that could also result from the loss or decline of tree-dwelling birds and mammals have received relatively little attention (but see Machmer and Steeger 1995, Aubry and Raley 2002).
Managing forests primarily for timber production often involves not only the removal of a substantial proportion of large, healthy trees from each harvest unit, but also the elimination of large dead or decadent trees.

We study plane shear waves in a continuum model for a saturated soil, assuming a hypoplastic flow rule. Following an earlier paper on velocity-controlled waves [1], we now specify a time- periodic stress disturbance on the boundary of a semi-infinite medium. As before, the stress saturates, tending at large times to a position-independent value, away from the boundary. By contrast, the dependence of this asymptotic state on the boundary data is strikingly different: in the previous case, this dependence was wildly discontinuous, while here it is very simple and smooth.

The Riemann initial value problem is studied for scalar conservation laws whose fluxes have a single inflection point. For a regularization consisting of balanced diffusive and dispersive terms, the travelling wave criterion is used to select admissible shocks. In some cases, the Riemann problem solution contains an undercompressive shock. The analysis is illustrated by exploring parameter space for the Buckley–Leverett flux. The boundary of the set of parameters for which there is a physical solution of the Riemann problem for all data is computed. Within the region of acceptable parameters, the solution hasseveral different forms, depending on the initial data; the different forms are illustrated by numerical computations. Qualitatively similar behaviour is observed in Lax–Wendroff approximations of solutions of the Buckley–Leverett equation with no dissipation or dispersion.
Fibxtem— Focussed Ion Beam Milling for TEM Sample Preparation

In the semiconductor industry, shrinking geometries and increasing process complexity have greatly increased the demand for TEM analysis of specific submicron regions. Until recently, samples of this nature have been difficult if not impossible to prepare. We have combined cross-sectional TEM sample preparation (XTEM) and the precise material sputtering of focussed ion beam milling (FIB) to thin samples to electron transparency. We call this sample preparation technique FIBXTEM.
Three advantages of this technique are: 1) The area of interest can be analyzed in the scanning electron microscope before final thinning; 2) Any specific defect area becomes a candidate for TEM analysis, including failed sub-micron structures; and 3) Samples are generally artifact-free and of uniform thickness.
Key elements of the FIBXTEM technique include precision planar polishing, unique holders for mounting and transferring samples between systems, and the FIB-induced deposition of a sacrificial protective layer over the area of interest during ion thinning.
This technique extends the use of TEM analysis into new areas of semiconductor process development and failure analysis. Recent applications for materials problem solving and failure analysis are discussed.

A case of erotomania (de Clérambault's syndrome) in a female with Schneider-positive schizophrenia is presented. The factors in her environment which may have precipitated the illness are discussed. The secondary nature of the syndrome is further illustrated by a discussion of the literature. In erotomania, the deluded victim, usually a woman, believes that a man, unattainable because of his social status, is in love with her. The nosological purity of the syndrome, despite its relative rarity, remains a subject of considerable debate. We present a case of erotomania in a female with schneiderian first rank symptoms (FRS) of schizophrenia (Schneider, 1959).
