Efficacy and acceptability of psychosocial interventions in asylum seekers and refugees: systematic review and meta-analysis

Aims
In the past few years, there has been an unprecedented increase in the number of forcibly displaced migrants worldwide, of which a substantial proportion is refugees and asylum seekers. Refugees and asylum seekers may experience high levels of psychological distress, and show high rates of mental health conditions. It is therefore timely and particularly relevant to assess whether current evidence supports the provision of psychosocial interventions for this population. We conducted a systematic review and meta-analysis of randomised controlled trials (RCTs) assessing the efficacy and acceptability of psychosocial interventions compared with control conditions (treatment as usual/no treatment, waiting list, psychological placebo) aimed at reducing mental health problems in distressed refugees and asylum seekers.
MethodsWe used Cochrane procedures for conducting a systematic review and meta-analysis of RCTs. We searched for published and unpublished RCTs assessing the efficacy and acceptability of psychosocial interventions in adults and children asylum seekers and refugees with psychological distress. Post-traumatic stress disorder (PTSD), depressive and anxiety symptoms at post-intervention were the primary outcomes. Secondary outcomes include: PTSD, depressive and anxiety symptoms at follow-up, functioning, quality of life and dropouts due to any reason.
ResultsWe included 26 studies with 1959 participants. Meta-analysis of RCTs revealed that psychosocial interventions have a clinically significant beneficial effect on PTSD (standardised mean difference [SMD] = −0.71; 95% confidence interval [CI] −1.01 to −0.41; I2 = 83%; 95% CI 78–88; 20 studies, 1370 participants; moderate quality evidence), depression (SMD = −1.02; 95% CI −1.52 to −0.51; I2 = 89%; 95% CI 82–93; 12 studies, 844 participants; moderate quality evidence) and anxiety outcomes (SMD = −1.05; 95% CI −1.55 to −0.56; I2 = 87%; 95% CI 79–92; 11 studies, 815 participants; moderate quality evidence). This beneficial effect was maintained at 1 month or longer follow-up, which is extremely important for populations exposed to ongoing post-migration stressors. For the other secondary outcomes, we identified a non-significant trend in favour of psychosocial interventions. Most evidence supported interventions based on cognitive behavioural therapies with a trauma-focused component. Limitations of this review include the limited number of studies collected, with a relatively low total number of participants, and the limited available data for positive outcomes like functioning and quality of life.
ConclusionsConsidering the epidemiological relevance of psychological distress and mental health conditions in refugees and asylum seekers, and in view of the existing data on the effectiveness of psychosocial interventions, these interventions should be routinely made available as part of the health care of distressed refugees and asylum seekers. Evidence-based guidelines and implementation packages should be developed accordingly.

Introduction: Emergency Departments (EDs) are frequently the first point of entry to access health services for First Nation (FN) members. In Alberta, FN members visit EDs at almost double the rate of non-FN persons. Furthermore, preliminary evidence demonstrates differences in ED experience for FN members as compared to the general population. The Alberta First Nations Information Governance Centre, Maskwacis Health Services, Yellowhead Tribal Council, Treaty 8 First Nations of Alberta, and Alberta Health Services are working together to research FN members ED experiences and concerns. Methods: This is participatory research guided by a two-eyed seeing approach that acknowledges the equal value of both Western and Indigenous worldviews. FN and non-FN leaders researchers are full partners in the development of the research project. Six sharing circles will be held in February 2018 across Alberta, with Elders, FN patients, FN and non-FN clinicians and FN and non-FN administrators. Sharing circles are similar to focus groups, but emphasize everyone having a turn to speak and demonstrating respect among participants in accordance with FN protocols. Elders will select the questions for discussion based on topics that arose in initial team meetings. Sharing circle discussions will be audio recorded and transcribed. Analysts will include both Western and Indigenous worldview researchers, who will collaboratively interpret findings. Elders will review, discuss, contextualize and expand upon study findings. The research is also guided by FN principles of Ownership, Control, Access, and Possession of FN information. It is through these principles that First Nation research projects can truly be classified as FN lead and driven. Results: Based on initial team meeting discussions, results of sharing circles are expected to provide insights on issues such as: healing, patient-provider communication (verbal and non-verbal), shared decision making, respect for patient preferences, experiences leading to trust or distrust, understandings of wait times and triage, times when multiple (repeat) ED presentations occur, distances travelled for care, choosing specific EDs when seeking care, impacts of stereotypes about FN patients, and racism and reconciliation. Conclusion: Understanding FN ED experience and bringing FN perspectives to Western conceptions of the goals and provision of ED care are important steps toward reconciliation.

Introduction
Children with CHD and acquired heart disease have unique, high-risk physiology. They may have a higher risk of adverse tracheal-intubation-associated events, as compared with children with non-cardiac disease.
Materials and methodsWe sought to evaluate the occurrence of adverse tracheal-intubation-associated events in children with cardiac disease compared to children with non-cardiac disease. A retrospective analysis of tracheal intubations from 38 international paediatric ICUs was performed using the National Emergency Airway Registry for Children (NEAR4KIDS) quality improvement registry. The primary outcome was the occurrence of any tracheal-intubation-associated event. Secondary outcomes included the occurrence of severe tracheal-intubation-associated events, multiple intubation attempts, and oxygen desaturation.
ResultsA total of 8851 intubations were reported between July, 2012 and March, 2016. Cardiac patients were younger, more likely to have haemodynamic instability, and less likely to have respiratory failure as an indication. The overall frequency of tracheal-intubation-associated events was not different (cardiac: 17% versus non-cardiac: 16%, p=0.13), nor was the rate of severe tracheal-intubation-associated events (cardiac: 7% versus non-cardiac: 6%, p=0.11). Tracheal-intubation-associated cardiac arrest occurred more often in cardiac patients (2.80 versus 1.28%; p<0.001), even after adjusting for patient and provider differences (adjusted odds ratio 1.79; p=0.03). Multiple intubation attempts occurred less often in cardiac patients (p=0.04), and oxygen desaturations occurred more often, even after excluding patients with cyanotic heart disease.
ConclusionsThe overall incidence of adverse tracheal-intubation-associated events in cardiac patients was not different from that in non-cardiac patients. However, the presence of a cardiac diagnosis was associated with a higher occurrence of both tracheal-intubation-associated cardiac arrest and oxygen desaturation.
Recovery practice in community mental health teams: national survey

Background
There is consensus about the importance of ‘recovery’ in mental health services, but the link between recovery orientation of mental health teams and personal recovery of individuals has been underresearched.
AimsTo investigate differences in team leader, clinician and service user perspectives of recovery orientation of community adult mental health teams in England.
MethodIn six English mental health National Health Service (NHS) trusts, randomly chosen community adult mental health teams were surveyed. A random sample of ten patients, one team leader and a convenience sample of five clinicians were surveyed from each team. All respondents rated the recovery orientation of their team using parallel versions of the Recovery Self Assessment (RSA). In addition, service users also rated their own personal recovery using the Questionnaire about Processes of Recovery (QPR).
ResultsTeam leaders (n = 22) rated recovery orientation higher than clinicians (n = 109) or patients (n = 120) (Wald(2) = 7.0, P = 0.03), and both NHS trust and team type influenced RSA ratings. Patient-rated recovery orientation was a predictor of personal recovery (b = 0.58, 95% CI 0.31–0.85, P<0.001). Team leaders and clinicians with experience of mental illness (39%) or supporting a family member or friend with mental illness (76%) did not differ in their RSA ratings from other team leaders or clinicians.
ConclusionsCompared with team leaders, frontline clinicians and service users have less positive views on recovery orientation. Increasing recovery orientation may support personal recovery.

Several extragalactic HI surveys using a λ21 cm 13-beam focal plane array will begin in early 1997 using the Parkes 64 m telescope. These surveys are designed to detect efficiently nearby galaxies that have failed to be identified optically because of low optical surface brightness or high optical extinction. We discuss scientific and technical aspects of the multibeam receiver, including astronomical objectives, feed, receiver and correlator design and data acquisition. A comparison with other telescopes shows that the Parkes multibeam receiver has significant speed advantages for any large-area λ21 cm galaxy survey in the velocity range range 0–14000 km s−1.

The gabbros of the Tertiary Kap Edvard Holm Layered Serieshave a stratigraphic thickness of more than 5000 m. Earlier work has shown that the range in cumulus mineral compositions is restricted (plagioclase An81—An51; olivine Fo85—Fo66; pyroxenes Ca43Mg46Fe11 to Ca43Mg37Fe20). Field evidence of magma injections is common, which together with the restricted range in mineral chemistry suggests that the magma chamber was frequently replenished by a less fractionated magma. A detailed study of a 600 m section (900–1500 m) in the Lower Layered Series reveals a period of crystallization when the magma chamber behaved as a closed system (900–1300 m). The rocks formed during this periodare well-laminated olivine–gabbros (900–110 m), which evolved to well-laminated oxide-gabbros (1100–1300 m). Compositional trends in the cumulusminerals are towards more evolved compositions (plagioclase An64—An58, pyroxene Mg# from 80 to 76) with stratigraphic height. From 1300 m to 1500 m, granular olivine-gabbros dominate, with moreprimitive mineral compositions (plagioclase An67—An76, pyroxene Mg# from 78 to 82). The transition olivine–gabbro to oxide-gabbro at 1100m is a consequence of fractional crystallization, and it is shown how changes in activities of FeO and Fe203 in the magma are reflected in the total iron content of plagioclases.The transition from oxide-gabbro to olivine-gabbro at 1300 m results from replenishment by less evolved basaltic magma. On the basis of calcic pyroxene chemistry and the mineral crystallization sequence it is concluded that the Kap Edvard Holm Layered Series crystallized from a tholeiitic magma similar to MORB. Melanogabbroic units occur throughout the intrusion as discordant to subconcordant sill-like bodies 0.2–2.0 m thick. The melanogabbroic units consist of Cr-rich augite-olivine-plagioclase heteradcumulates and contain deformed mica crystals of pre-emplacement origin. These units crystallized from a wet, MgO-rich magma which was injected into the layered host gabbros after the formation of the cumulus pile, but before the magma was completely solidified.
The Kap Edvard Holm Layered Series has several parallels with the plutonic part of ophiolite sequences. These include: cumulus mineral assemblage, compositions of the minerals and the restricted range in compositions with stratigraphic height; field evidence of repeated replenishment of basaltic magma; dyke swarms overlying the roof zone of the magma chamber; and the existence of a late intrusive suite of wet, MgO-rich magma. These parallels suggest that the processes involved in the formation of the Kap Edvard Holm Layered Series were similar to those involved in the formation of the crustalpart of many ophiolites and beneath present-day spreading ridges. The Kap Edvard Holm Layered Series is therefore believed to represent a shallow-level magma chamber which acted as a reservoir for basaltic flows at the continental margin during the opening of the North Atlantic Ocean.
Radiocarbon Dating of the Human Occupation of Australia Prior to 40 ka BP—Successes and Pitfalls

Charcoal samples from ancient human occupation sites in Australia have been subjected to a rigorous pretreatment and stepped combustion regime in order to explore the possibility that these sites may be older than previous radiocarbon dating had suggested. In one case, the Devil's Lair site in southwest Australia, the methodology has clearly removed vestiges of contamination by more modern carbon and has led to a revised radiocarbon chronology that provides evidence for human occupation of southwest Australia by at least 44 ka BP and probably by 46–47 ka BP. In contrast, charcoal from the Nauwalabila site has been so severely altered that insufficient of the original carbon remains for reliable 14C dating. Finally, where the charcoal is well preserved, such as at the Carpenter's Gap site, the new results provide reassurance that earlier 14C results of ∼40 ka BP are indeed true ages and are not simply at the limit of the 14C technique.

Lynch's Crater in northeastern Australia provides a long, continuous record of environmental change within the Late Quaternary. Here, we present newly determined radiocarbon ages, using acid-base-acid stepped combustion (ABA-SC) and acid-base-wet oxidation stepped combustion (ABOX-SC) pretreatment strategies. The new results largely confirm the original untreated radiocarbon results for the uppermost 9 m of sediments, (ca. 35 ka BP). Below this depth, results from both pretreatment methods are in stratigraphic agreement and extend the dating of the record from 38 ka BP to about 48 ka BP, although an apparent increased sedimentation rate below 12 m is questionable. The scarcity of “charcoal” in several of the samples raises questions regarding the application of ABOX-SC to lake or swamp sediments, with evidence for contributions from younger, chemically resistant bacterial carbon along with fine “charcoal” in some samples. However, the extent to which this phenomenon is significant to the final age estimate appears to be sample specific, and is probably dependent upon the length of the wet oxidation step in the pretreatment.

We present results that validate a new wet oxidation, stepped-combustion procedure for dating “old” charcoal samples. An acid–base–wet oxidation (ABOX) pretreatment procedure has been developed that is used in place of the conventional acid-base-acid (ABA) pretreatment. Combustions and graphitizations are performed in a vacuum line that is insulated from the atmosphere by a second backing vacuum to eliminate the risk of atmospheric leakage into the line at any stage of the procedure. Combustions are performed at 3 temperatures (330 °, 630 ° and 850 °) with a graphite target produced from the CO2 evolved during each combustion step. In this way, the removal of any contamination can be monitored, and a high degree of confidence can be placed on the final age. The pretreatment, combustion, graphitization, and measurement blank for the procedure, based on the analysis of a “radiocarbon-dead” graphite, is 0.5 ± 0.5 μg C (1σ, n=14), equivalent to 0.04 ± 0.02 pMC or an “age” of approximately 60 ka for a 1 mg graphite target. Analyses of a “radiocarbon-dead” natural charcoal after ABOX pretreatment and stepped combustion suggest that the total blank (including contamination not removed by pretreatment) may be higher than for graphite, ranging up to 0.10 ± 0.02 pMC. Additional experiments confirm good agreement with accepted values for the international low-14C “New Kauri” standard (0.16–0.25 pMC). They also confirm excellent reproducibility, with 3 separate dates on different aliquots of a charcoal sample from Ngarrabullgan Cave (Queensland, Australia) ranging from 35.2 to 35.5 ka 14C BP. It is also demonstrated that the ABOX pretreatment, in conjunction with the new vacuum line described here, is able to remove contamination not removed by the conventional ABA pretreatment, suggesting that the technique can be used to produce reliable 14C dates on charcoal up to at least 50 ka.
Correlates of unmet need for mental health services by children and adolescents

Background. Little is known about the extent and correlates of unmet need for mental health services in community samples of children and adolescents.
Methods. Data were obtained from the 1285 parent/youth pairs interviewed at four sites in the USA and Puerto Rico in the Methods for the Epidemiology of Child and Adolescent Mental Disorders (MECA) Study. Unmet need was defined to exist if psychopathology and associated functional impairment were present but no mental health services had been received in the previous 6 months.
Results. Of the total sample, 17·1% had unmet need. Adjusting for demographic variables, logistic regression analyses revealed that unmet need was significantly associated with: indicators of economic disadvantage, such as being on public assistance and not being covered by health insurance; opinions of the parents and children or adolescents that the latter had poor mental health; parental psychopathology; poor school grades; and parent-reported access barriers such as concern that the child would want to solve the problem unassisted, would refuse to attend mental health services, or would be hospitalized or taken away against the parent's will. No youth-reported access barriers were significantly associated with unmet need.
Conclusions. The economic correlates of unmet need may attain increased importance in the light of current reform in health care financing in the USA. Access may be facilitated by increasing parental knowledge of mental health services and enabling children and adolescents to initiate contact with services independently of their families.
Growth Temperature Uniformity of MBE-Grown GaAs Determined by Scanning Room Temperature Photoluminescence

We have used scanning room temperature photoluminescence to map GaAs quantum well widthsand AIGaAs barrier compositions over 2-inch and 3-inch diameter epitaxial layers grown by MBE at temperatures ranging from 600 to 700ºC. Analysis of these maps allows a nondestructive quantitative analysis of the GaAs growth rate uniformity from which we can calculate the temperature distribution during growth. We have used this technique to compare the thermal uniformity of various substrate holders designed for use in the Intevac ModGenII MBE system.

The role of the magnetic field in a coronal mass ejection event has not been unequivocally defined, and may in fact be quite variable in view of the large variety of shapes and sizes of coronal transients. Measurements of the magnetic field associated with these events have thus far been inferred from simultaneously observed radio bursts, which provide no information on the direction of the field and are limited in spatial resolution. Substantial improvement in these two areas could be achieved by continuous monitoring of the Faraday rotation of a linearly polarized spacecraft signal during solar occultation. A coronal event traversing the line-of-sight would yield a characteristic profile in cross section, which would be of value for discriminating between the various models of coronal transients.

Tegretol (carbamazepine; 5-carbamyl-5H-dibenz (b, f) azepine) has been under investigation since 1958 in the treatment of epilepsy on the continent of Europe, following the demonstration of its anticonvulsant properties in animals by Theobald and Kunz (1963). Clinical reports have been enthusiastic (Lorgé, 1963, Bonduelle et al., 1963). The present investigation was undertaken with a view to a more critical appraisal of this drug, avoiding the dangers of sudden alteration of anticonvulsant therapy in epileptic patients, documented by Schwab et al. (1959), while still maintaining the double-blind nature of the trial.
