Balloon dilatation versus surgical valvotomy for congenital aortic stenosis: a propensity score matched study

Background:
Balloon valvuloplasty and surgical aortic valvotomy have been the treatment mainstays for congenital aortic stenosis in children. Choice of intervention often differs depending upon centre bias with limited relevant, comparative literature.
Objectives:This study aims to provide an unbiased, contemporary matched comparison of these balloon and surgical approaches.
Methods:Retrospective analysis of patients with congenital aortic valve stenosis who underwent balloon valvuloplasty (Queensland Children’s Hospital, Brisbane) or surgical valvotomy (Royal Children’s Hospital, Melbourne) between 2005 and 2016. Patients were excluded if pre-intervention assessment indicated ineligibility to either group. Propensity score matching was performed based on age, weight, and valve morphology.
Results:Sixty-five balloon patients and seventy-seven surgical patients were included. Overall, the groups were well matched with 18 neonates/25 infants in the balloon group and 17 neonates/28 infants in the surgical group. Median age at balloon was 92 days (range 2 days – 18.8 years) compared to 167 days (range 0 days – 18.1 years) for surgery (rank-sum p = 0.08). Mean follow-up was 5.3 years. There was one late balloon death and two early surgical deaths due to left ventricular failure. There was no significant difference in freedom from reintervention at latest follow-up (69% in the balloon group and 70% in the surgical group, p = 1.0).
Conclusions:Contemporary analysis of balloon aortic valvuloplasty and surgical aortic valvotomy shows no difference in overall reintervention rates in the medium term. Balloon valvuloplasty performs well across all age groups, achieving delay or avoidance of surgical intervention.

Scherer's industrial organization model is modified to characterize the grain marketing system. The modified model identifies the important relationships between economic characteristics and the type of grain facilities found in three states; Alabama, Illinois, and Ohio. Multivariate linear discriminant analysis is used to identify the important basic supply, demand, and transportation variables within and among these states. It is concluded that the structural differences among selected grain marketing regions can be explained by regional differences in basic supply, demand, and transportation variables. The findings suggest that Scherer's industrial organization model can be adapted to explain the diverse structure of the grain marketing system in selected states.

Glutamate excitotoxicity is implicated in the pathogenesis of amyotrophic lateral sclerosis (ALS). We report the results of a double blind, placebo controlled, trial using 100 mg of oral daily lamotrigine (3,5-diamino-6-(2,3 dichlorophenyl)-l,2,4-triazine) which inhibits glutamate release. 67 patients were entered and at trial termination of 1.5 years 15 had withdrawn (9 active and 6 placebo) and 12 had died (6 active and 6 placebo). Mean age at entry was 57.5 years for the active and 58.6 years for the placebo groups. Patients were seen at 3 monthly intervals and scored according to neurological deficit based upon age of onset, bulbar and respiratory involvement, ambulation and functional disability. The mean change in clinical scores for the active versus placebo groups over the trial period was 7.1 ± 3.3 and 9.0 ± 3.3 respectively (0.05 < p < 0.10). Changes in cortical threshold and MEP/CMAP ratios to magnetic stimulation also did not differ significantly between the two groups. We conclude that lamotrigine in the doses administered does not alter the course of ALS.

Douglas-fir tussock moth cocoons were collected and their distribution recorded by crown level on white fir, Abies concolor, in northeastern California. Nine trees were sampled in two defoliation classes, light (20–30% defoliated) and moderate to heavy (50–80% defoliated). Cocoons were more abundant in the lower crown levels as defoliation became greater. The proportion of female cocoons increased toward the lower levels of the crown. An overall sex ratio of 1.2♂:1.0♀ was recorded. Significantly more male cocoons were parasitized than female. A list of the natural enemies of the tussock moth from the study area is given.

We present results from an attempt to detect radio emission from the interaction between a transiting extra-solar planet and its host star. We determine a new upper limit of 47 mJy on the radio flux density from HD 189733b, in the frequency range 327–347 MHz.

Site fidelity is believed to be an important life history strategy for Weddell seals (Leptonychotes weddellii), that return to traditional breeding colonies each spring. We examined four hypotheses concerning their fidelity to these colonies: 1) fidelity is stronger to natal sites (natal fidelity) than to other sites, 2) females exhibit greater site fidelity than males, 3) site fidelity for both sexes increases with age, 4) site fidelity in adult females is related to their reproductive status and their total number of offspring. Analysis of a long-term tagging database from McMurdo Sound did not support hypotheses 1 and 2. Although animals did express fidelity to specific sites over their lifetime (χ2 tests, P < 0.05), fidelity to natal colonies was lower than to other sites (χ2 test, P < 0.05). There were no differences in site fidelity between males and females (χ2 tests, P > 0.05). Hypothesis 3 was supported. Since the probability of a returning seal occupying the same colony as the previous year increased with age among both sexes to about age 12. Finally, in support of hypothesis 4, females with a higher degree of site fidelity were more likely to both have a higher reproductive rate and return to a site where they have previously given birth.
Strategic Auditing in a Political Hierarchy: An Informational Model of the Supreme Court's Certiorari Decisions

We examine how the Supreme Court uses signals and indices from lower courts to determine which cases to review. In our game theoretic model, a higher court cues from publicly observable case facts, the known preferences of a lower court, and its decision. The lower court attempts to enforce its own preferences, exploiting ambiguity in cases' fact patterns. In equilibrium, a conservative higher court declines to review conservative decisions from lower courts regardless of the facts of the case or the relative ideology of the judges. But a conservative higher court probabilistically reviews liberal decisions, with the “audit rate” tied, to observable facts and the ideology of the lower court judge. We derive comparative static results and test them with a random sample of search-and-seizure cases appealed to the Burger Court between 1972 and 1986. The evidence broadly supports the model.

Summary
In the reaction after 1945 against the policy of appeasement, headed in Britain by those leading Tory historians, who had opposed that policy at the time, and in America by those leading members of the Democratic equivalents of the Whig school of history, who had in the main supported Roosevelt and had pressed in the period before Pearl Harbor for American support of Britain, a small group of British ambassadors became the target for obloquy and denunciation. This process was greatly aided, firstly by the publication of the first volume of Churchill's memoir/history of the second world war, The Gathering Storm, in 1948, and then by the early publication of Series III of the British Foreign Office Documents for the interwar years, Documents on British Foreign Policy, 1919–1939, which covered the years 1938–9. The three western allies engaged in the publication of the captured German diplomatic documents, Documents on German Foreign Policy, 1918–1945,had in the same way decided to begin that publication with Series D, the initial volumes of which began at varying dates in 1936–7, the series as a whole covering the period until 7 December 1941.
Behind these decisions by the British, American and French governments lay the advice of their official historians. This advice was determined by the desire to avoid a new War-Guilt controversy; much the same motive lay behind the decision to publish all the documents cited in evidence at the Trial of the Major War Criminals at Nuremberg.

Nuclear fuel waste disposal research in Canada is concentrating on hard-rock disposal. The research programs concerned with the man-made components of the disposal system are reviewed. Irradiated fuel and solidified reprocessing wastes are both being researched, as are durable containers, and buffer and backfill materials. This review concentrates mainly on the more scientific aspects of the research, which contribute to the selection of preferred options for the various components of the system, and which support directly or indirectly the safety analysis of the disposal concept. Some technology development is included in the program now, and this is expected to expand as confidence in the acceptability of the disposal concept grows.

Numerous workers (Plaut, 1935; Altaman, 1937; Monouni, 1937) have reported that the blood and spinal fluid ascorbic acid levels are low in the aged. No adequate explanation of this fact has been given. Wortis (1938) has suggested that, since oral administration increases the ascorbic acid level of the spinal fluid in the aged as in the young, defective absorption probably does not play a significant part. He believes that the lower values in older persons may be due to increased intensity of metabolic processes concerned with the elaboration of ascorbic acid. Friedman, in discussing this contribution, advanced the suggestion that in older persons there may be less need for ascorbic acid because metabolism is slowed.
