Assessment of perinatal anxiety: diagnostic accuracy of five measures

Background
Anxiety in pregnancy and after giving birth (the perinatal period) is highly prevalent but under-recognised. Robust methods of assessing perinatal anxiety are essential for services to identify and treat women appropriately.
AimsTo determine which assessment measures are most psychometrically robust and effective at identifying women with perinatal anxiety (primary objective) and depression (secondary objective).
MethodWe conducted a prospective longitudinal cohort study of 2243 women who completed five measures of anxiety and depression (Generalized Anxiety Disorder scale (GAD) two- and seven-item versions; Whooley questions; Clinical Outcomes in Routine Evaluation (CORE-10); and Stirling Antenatal Anxiety Scale (SAAS)) during pregnancy (15 weeks, 22 weeks and 31 weeks) and after birth (6 weeks). To assess diagnostic accuracy a sample of 403 participants completed modules of the Mini-International Neuropsychiatric Interview (MINI).
ResultsThe best diagnostic accuracy for anxiety was shown by the CORE-10 and SAAS. The best diagnostic accuracy for depression was shown by the CORE-10, SAAS and Whooley questions, although the SAAS had lower specificity. The same cut-off scores for each measure were optimal for identifying anxiety or depression (SAAS ≥9; CORE-10 ≥9; Whooley ≥1). All measures were psychometrically robust, with good internal consistency, convergent validity and unidimensional factor structure.
ConclusionsThis study identified robust and effective methods of assessing perinatal anxiety and depression. We recommend using the CORE-10 or SAAS to assess perinatal anxiety and the CORE-10 or Whooley questions to assess depression. The GAD-2 and GAD-7 did not perform as well as other measures and optimal cut-offs were lower than currently recommended.

A retrospective review of 29 patients with neurovascular compression syndrome (NVCS) involving the anterior visual pathway was conducted. Various patterns of NVCS and visual defects were identified, most commonly involving the optic nerve and internal carotid artery. Most patients were stable, except one with progressive visual field defects. Although mostly asymptomatic, NVCS can rarely cause compressive optic neuropathy. NVCS should be kept in the differential diagnosis of normal tension glaucoma, especially with progressive visual loss despite treatment. Patients with progressive visual loss may require decompression surgery. Non-contrast computed tomography scan may miss NVCS, and magnetic resonance imaging is diagnostic.

It is usual to put the New Woman writer Sarah Grand alongside Oscar Wilde to mark their differences. However, this essay suggests that these two authors had more in common than at first appears, both with regards to the fashioning of their literary identities and to their literary productions. Grand's compendious best-selling novel The Heavenly Twins (1893) is usually seen as realist fiction, but its interlude titled “The Tenor and the Boy,” which was actually composed much earlier, presents a stand-alone narrative that owes more to the romance mode and is much more playful in tone and spirit. It also deals with the decadent theme of the secret double life, although crucially and dramatically Grand focuses on female subterfuge. After juxtaposing Wilde's “Lady Alroy” (1887), another text about the female double life, with Grand's interlude, I subsequently consider decadent qualities common to both “The Tenor and the Boy” and other Wildean texts including theatricality, doubling, gender ambiguity, and queer desire.

Summary
British Decadence was, in large part, inspired by the poetry and prose of France. The cross-Channel traffic in advanced literature saw extremes of Francophobia and Francophilia in the British press, with writers such as Émile Zola having translations of their work censored and attacked in the House of Commons while receiving rapturous receptions when lecturing in London. A central figure in this traffic of Decadent literature was the poet Paul Verlaine, whose dissolute life scandalized the British public. As this chapter demonstrates, British writers took from their French counterparts both formal innovation and an antagonistic approach to moral orthodoxies. Verlaine’s queer sexuality and relentlessly self-scrutinizing approach to poetry came to symbolize for a generation of young English writers an intoxicating possibility of a poetic revolution against cultural hegemony. Verlaine’s lecture tour of England in late 1893 represents the highpoint of the British obsession with French décadence before the Wilde trials saw progressive literature retreat into the margins.

Summary
Algernon Charles Swinburne was the dominant literary influence on many of the best-known writers of the late Victorian period, his verse still representing an apparently unsurpassable model to the aspirant young poets of the early twentieth century. Swinburne's own development as a writer was the product of many diverse influences including British Romanticism, modern French literature, Elizabethan and Jacobean drama and Italian revolutionary politics, but his contact with the Pre-Raphaelites, William Morris and Dante Gabriel Rossetti in particular, was undoubtedly one of the major shaping forces. This chapter briefly sketches Swinburne's links with the Pre-Raphaelite circle before assessing the impact of Pre-Raphaelitism on his writing and, most importantly, his poetry.
It is usual to think of Swinburne's first contact with Pre-Raphaelitism as his meeting with Morris on 1 November 1857 and subsequent introduction to Burne-Jones and Rossetti during the period when all three artists, along with others such as Arthur Hughes and Val Prinsep, were engaged in the famous enterprise of painting Arthurian frescoes on the walls of the Oxford Union. However, Swinburne would already have been familiar with Pre-Raphaelite art through his close friend and ‘good angel’ Lady Trevelyan. Pauline Trevelyan, wife of Sir Walter Calverley Trevelyan, was chatelaine of Wallington, a large house near Capheaton, the Northumberland home of Sir John Swinburne, Swinburne's grandfather. She is best known as a patron of the arts and for her cultivation of a circle which included her close friend and mentor John Ruskin, the sculptor Thomas Woolner, one of the original Pre-Raphaelites, and the painter and Pre-Raphaelite associate William Bell Scott, all of whom contributed artworks to decorate the hall at Wallington.
Monoamniotic Twins: What Should Be the Optimal Antenatal Management?

Monoamniotic twinning is a rare event with an incidence of 1% of all monozygotic twins and associated with a high fetal morbidity and mortality. Confident early diagnosis is possible, but optimal management is not yet established. This article presents the experience of a single centre in managing all monoamniotic twins diagnosed during 1994–2000. Seven pairs of monoamniotic twins were identified for analysis. All were managed in accord with a unit protocol that involved early diagnosis, serial ultrasound examination and elective early delivery. In four cases, the detection of monoamnionicity was made during a first trimester nuchal scan. Discordance for structural abnormality was found in three cases where the co-twin was normal. Cord entanglement was detected antenatally in four cases. Two pairs of twins died before 20 weeks. One of these had early onset twin–twin transfusion syndrome. In five cases, the pregnancy continued beyond 20 weeks. A live birth rate of 90% and intact survival of 70% were achieved in this group. We believe that ultrasound is reliable in diagnosing monoamniotic twins and the detection of cord entanglement. Timing of elective delivery is a balance between the risks of preterm birth at a specific gestational age in an individual centre compared with the unquantifiable risks of fetal death if an expectant policy were pursued. The decision to deliver and at which gestational age should combine input from the parents, neonatologist, fetal medicine consultant and the obstetrician.
