We investigated whether the personal importance of objects influences utilitarian decision-making in which damaging property is necessary to produce an overall positive outcome. In Experiment 1, participants judged saving five objects by destroying a sixth object to be less acceptable when the action required destroying the sixth object directly (rather than as a side-effect) and the objects were personally important (rather than unimportant). In Experiment 2, we demonstrated that utilitarian judgments were not influenced by the objects’ monetary worth. Together these findings suggest that personal importance underlies people’s sensitivity to damaging property as a means for utilitarian gains.

Impact of enucleation on adult retinoblastoma survivors’ quality of life: A qualitative study of survivors' perspectives

Objectives
Retinoblastoma is the most common primary intraocular tumor of childhood with >95% survival rates in the US. Traditional therapy for retinoblastoma often included enucleation (removal of the eye). While much is known about the visual, physical, and cognitive ramifications of enucleation, data are lacking about survivors' perception of how this treatment impacts overall quality of life.
MethodsQualitative analysis of an open-ended response describing how much the removal of an eye had affected retinoblastoma survivors' lives and in what ways in free text, narrative form.
ResultsFour hundred and four retinoblastoma survivors who had undergone enucleation (bilateral disease = 214; 52% female; mean age = 44, SD = 11) completed the survey. Survivors reported physical problems (n = 205, 50.7%), intrapersonal problems (n = 77, 19.1%), social and relational problems (n = 98, 24.3%), and affective problems (n = 34, 8.4%) at a mean of 42 years after diagnosis. Three key themes emerged from survivors' responses; specifically, they (1) continue to report physical and intrapersonal struggles with appearance and related self-consciousness due to appearance; (2) have multiple social and relational problems, with teasing and bullying being prominent problems; and (3) reported utilization of active coping strategies, including developing more acceptance and learning compensatory skills around activities of daily living.
Significance of resultsThis study suggests that adult retinoblastoma survivors treated with enucleation continue to struggle with a unique set of psychosocial problems. Future interventions can be designed to teach survivors more active coping skills (e.g., for appearance-related issues, vision-related issues, and teasing/bullying) to optimize survivors' long-term quality of life.

Background:
Immune system markers may predict affective disorder treatment response, but whether an overall immune system marker predicts bipolar disorder treatment effect is unclear.
Methods:Bipolar CHOICE (N = 482) and LiTMUS (N = 283) were similar comparative effectiveness trials treating patients with bipolar disorder for 24 weeks with four different treatment arms (standard-dose lithium, quetiapine, moderate-dose lithium plus optimised personalised treatment (OPT) and OPT without lithium). We performed secondary mixed effects linear regression analyses adjusted for age, gender, smoking and body mass index to investigate relationships between pre-treatment white blood cell (WBC) levels and clinical global impression scale (CGI) response.
Results:Compared to participants with WBC counts of 4.5–10 × 109/l, participants with WBC < 4.5 or WBC ≥ 10 showed similar improvement within each specific treatment arm and in gender-stratified analyses.
Conclusions:An overall immune system marker did not predict differential treatment response to four different treatment approaches for bipolar disorder all lasting 24 weeks.

Objective
Immunological theories, particularly the sickness syndrome theory, may explain psychopathology in mood disorders. However, no clinical trials have investigated the association between overall immune system markers with a wide range of specific symptoms including potential gender differences.
MethodsWe included two similar clinical trials, the lithium treatment moderate-dose use study and clinical and health outcomes initiatives in comparative effectiveness for bipolar disorder study, enrolling 765 participants with bipolar disorder. At study entry, white blood cell (WBC) count was measured and psychopathology assessed with the Montgomery and Aasberg depression rating scale (MADRS). We performed analysis of variance and linear regression analyses to investigate the relationship between the deviation from the median WBC, and multinomial regression analysis between different WBC levels. All analyses were performed gender-specific and adjusted for age, body mass index, smoking, race, and somatic diseases.
ResultsThe overall MADRS score increased significantly for each 1.0×109/l deviation from the median WBC among 322 men (coefficient=1.10; 95% CI=0.32–1.89; p=0.006), but not among 443 women (coefficient=0.56; 95% CI=−0.19–1.31; p=0.14). Among men, WBC deviations were associated with increased severity of sadness, inner tension, reduced sleep, reduced appetite, concentration difficulties, inability to feel, and suicidal thoughts. Among women, WBC deviations were associated with increased severity of reduced appetite, concentration difficulties, lassitude, inability to feel, and pessimistic thoughts. Both higher and lower WBC levels were associated with increased severity of several specific symptoms.
ConclusionImmune system alterations were associated with increased severity of specific mood symptoms, particularly among men. Our results support the sickness syndrome theory, but furthermore emphasise the relevance to study immune suppression in bipolar disorder. Due to the explorative nature and cross-sectional design, future studies need to confirm these findings.

We prove the existence of extremal Sasakian structures occurring on a countably infinite number of distinct contact structures on
${T}^{2} \times {S}^{3} $ and certain related 5-manifolds. These structures occur in bouquets and exhaust the Sasaki cones in all except one case in which there are no extremal metrics.

We study the complexity of the isomorphism relation on classes of computable structures. We use the notion of FF-reducibility introduced in [9] to show completeness of the isomorphism relation on many familiar classes in the context of all
equivalence relations on hyperarithmetical subsets of ω.

The problem of determining a square integrable function from its modulus and that of its Fourier transform has been considered in an article by Corbett and Hurst ([1]). In this work we point out an error in one of the main results of the cited article concerning the Pauli uniqueness of real states, provide a proof of uniqueness for non-negative states, and present various related examples and discussion of the problem.
Respiratory Isolation of Tuberculosis Patients Using Clinical Guidelines and an Automated Clinical Decision Support System

Objective:
To evaluate a clinical guideline and an automated computer protocol for detection and respiratory isolation of tuberculosis (TB) patients.
Design:An automated computer protocol was tested on a retrospective cohort of adult culture-positive TB patients admitted from 1992 to 1993 to Columbia-Presbyterian Medical Center and evaluated prospectively from July 1995 until July 1996.
Setting:A large teaching hospital in New York City.
Patients:171 adult patients admitted from 1992 to 1993 and 43 patients admitted between July 1995 and July 1996.
Interventions:The 1990 Centers for Disease Control and Prevention guidelines for preventing transmission of TB were adapted to formulate clinical guidelines to ensure early isolation of TB patients at Columbia-Presbyterian Medical Center.
Results:Implementation of a clinical respiratory isolation protocol resulted in a significant improvement in TB patient isolation rates, from 45 (51%) of 88 in 1992 to 62 (75%) of 83 in 1993 (P<.001). In testing automated protocols, the theoretical improvement would have identified an additional 27 patients not isolated by clinicians, making the overall isolation rate 134 (78%) of 171.
For the prospective evaluation, 30 (70%) of 43 TB patients were isolated by clinicians adhering to the clinical protocol. Four additional patients were identified by the automated TB protocol, making the combined isolation rate 34 (79%) of 43.
Conclusions:A clinical policy to isolate TB patients and suspected human immunodeficiency virus-infected patients with cough, fever, or radiographic abnormalities improved isolation of culture-documented TB patients from 1992 to 1993. Automated computer protocols were successful in identifying additional potentially infectious patients that clinicians failed to place on respiratory isolation. Clinical and automated protocols combined resulted in better isolation rates than a clinical protocol alone.

Summary
Introduction
In this chapter we shall extend some of the theory developed so far by introducing the notion of the product integral of a measure. This extension is a quite natural one from several points of view. On the one hand, from a purely integration-theoretic viewpoint, one might envision the possibility of product integrating the most general objects of ordinary integration theory, especially in the light of the development so far. Of course, it would be expected that such a theory would be closely related to the theory of (linear) differential equations with measures as coefficients. Such equations have occasionally been considered ([VG], [DP]) at least in special cases, but usually not in any systematic manner. They have some importance, however; for example, one problem in (quantum mechanical) scattering theory concerns the description of self-adjoint operators H with a part unitarily equivalent to operating in L2(ℝn), and some of these operators are of the form H = H0 + μ, with μ a measure. The scattering theory associated with such operators may be developed by using the product integral to study the asymptotic form of solutions of the equation Hψ = k2ψ.

Summary
In the present chapter, we shall discuss additional results and generalizations of the theory that we have presented so far. A large part of our discussion will focus on the work of other authors. Some of this work, e.g., product integration of nonlinear operator-valued functions, has not been developed in the previous chapters and will only be discussed briefly here. This is partly due to the fact that for these results, a really systematic and complete presentation is simply not available at the present time. On the other hand, what we have presented above has been somewhat determined by our own predispositions; a treatise of reasonable length covering all aspects of product integration in detail is hardly feasible, and we have therefore selected material which is of the most interest to us and which at the same time is not readily accessible in the modern literature. Nevertheless, the results we shall discuss in this chapter are of considerable significance and importance and certainly deserve some attention in any general discussion of product integration.
If we consider the theory of product integration of matrix-valued functions presented in Chapter 1 as a starting point, then there are several possible directions for generalization. For example, a first type of generalization might focus on reducing to a minimum the regularity assumptions on the function A(x) being product integrated with regard to its dependence on x, while not changing essentially the nature of the values of this function.

Originally published in 1979, this book shows the beautiful simplifications that can be brought to the theory of differential equations by treating such equations from the product integral viewpoint. The first chapter of the book, dealing with linear ordinary differential equations, should be accessible to anyone with a knowledge of matrix theory and elementary calculus. Later chapters assume more sophistication on the part of the reader. The essential unity of these subjects is illustrated by the fact that the idea of the product integral can be naturally and effectively used to deal with all of them.
