The Trial Innovation Network Liaison Team: building a national clinical and translational community of practice

In 2016, the National Center for Advancing Translational Science launched the Trial Innovation Network (TIN) to address barriers to efficient and informative multicenter trials. The TIN provides a national platform, working in partnership with 60+ Clinical and Translational Science Award (CTSA) hubs across the country to support the design and conduct of successful multicenter trials. A dedicated Hub Liaison Team (HLT) was established within each CTSA to facilitate connection between the hubs and the newly launched Trial and Recruitment Innovation Centers. Each HLT serves as an expert intermediary, connecting CTSA Hub investigators with TIN support, and connecting TIN research teams with potential multicenter trial site investigators. The cross-consortium Liaison Team network was developed during the first TIN funding cycle, and it is now a mature national network at the cutting edge of team science in clinical and translational research. The CTSA-based HLT structures and the external network structure have been developed in collaborative and iterative ways, with methods for shared learning and continuous process improvement. In this paper, we review the structure, function, and development of the Liaison Team network, discuss lessons learned during the first TIN funding cycle, and outline a path toward further network maturity.

Introduction
Mental health regional differences during pregnancy through the COVID-19 pandemic is understudied.
ObjectivesWe aimed to quantify the impact of the COVID-19 pandemic on maternal mental health during pregnancy.
MethodsA cohort study with a web-based recruitment strategy and electronic data collection was initiated in 06/2020. Although Canadian women, >18 years were primarily targeted, pregnant women worldwide were eligible. The current analysis includes data on women enrolled 06/2020-11/2020. Self-reported data included mental health measures (Edinburgh Perinatal Depression Scale (EPDS), Generalized Anxiety Disorders (GAD-7)), stress. We compared maternal mental health stratifying on country/continents of residence, and identified determinants of mental health using multivariable regression models.
ResultsOf 2,109 pregnant women recruited, 1,932 were from Canada, 48 the United States (US), 73 Europe, 35 Africa, and 21 Asia/Oceania. Mean depressive symptom scores were lower in Canada (EPDS 8.2, SD 5.2) compared to the US (EPDS 10.5, SD 4.8) and Europe (EPDS 10.4, SD 6.5) (p<0.05), regardless of being infected or not. Maternal anxiety, stress, decreased income and access to health care due to the pandemic were increasing maternal depression. The prevalence of severe anxiety was similar across country/continents. Maternal depression, stress, and earlier recruitment during the pandemic (June/July) were associated with increased maternal anxiety.
ConclusionsIn this first international study on the impact of the COVID-19 pandemic, CONCEPTION has shown significant country/continent-specific variations in depressive symptoms during pregnancy, whereas severe anxiety was similar regardless of place of residence. Strategies are needed to reduce COVID-19’s mental health burden in pregnancy.

Dementia cases are increasing worldwide; thus, investigators seek to identify interventions that might prevent or ameliorate cognitive decline in later life. Extensive research confirms the benefits of physical exercise for brain health, yet only a fraction of older adults exercise regularly. Interactive mental and physical exercise, as in aerobic exergaming, not only motivates, but has also been found to yield cognitive benefit above and beyond traditional exercise. This pilot study sought to investigate whether greater cognitive challenge while exergaming would yield differential outcomes in executive function and generalize to everyday functioning. Sixty-four community based older adults (mean age=82) were randomly assigned to pedal a stationary bike, while interactively engaging on-screen with: (1) a low cognitive demand task (bike tour), or (2) a high cognitive demand task (video game). Executive function (indices from Trails, Stroop and Digit Span) was assessed before and after a single-bout and 3-month exercise intervention. Significant group × time interactions were found after a single-bout (Color Trails) and after 3 months of exergaming (Stroop; among 20 adherents). Those in the high cognitive demand group performed better than those in the low cognitive dose condition. Everyday function improved across both exercise conditions. Pilot data indicate that for older adults, cognitive benefit while exergaming increased concomitantly with higher doses of interactive mental challenge. (JINS, 2015, 21, 768–779)

Members of the public are exposed to radiation because of where they live and their habits bring them into contact with sources of radiation. They may be exposed directly by the source, by direct irradiation or inhalation of released activity or indirectly by exposure to contaminated environmental materials such as food. Consequently habit data are an essential part of dose assessment for members of the public [1]. Previously no detailed surveys had been undertaken in Ireland of habit data relevant to the assessment of doses to the population from radioactivity in the marine environment. Instead RPII dose assessments were made on the basis of assumed or notional data inferred from habit surveys undertaken elsewhere and from national average consumption figures. Following a tender process the Centre for Environment, Fisheries & Aquaculture Science, Cefas, UK, were commissioned to undertake a Habits Survey. Its report [2] provides an assessment of aquatic radiation exposure pathways in Ireland relating to anthropogenic radioactivity in the Irish Sea.

It has been shown that lucerne/grass mixtures, especially those containing a high proportion of cocksfoot, can make a valuable contribution to the winter feeding of livestock when the autumn growth is allowed to stand in the field until the winter. For instance, Hughes (1954) showed that it was possible to carry store cattle right through the winter on such herbage with little supplementary feeding. Other experimental work (Barker, Hanley & Ridgman, 1955) has shown that mowing or grazing this autumn growth in September may lead to poorer production in subsequent years than October defoliation. It therefore seemed desirable to determine the effect of date of grazing in the winter on the subsequent yield of a lucerne/grass ley and two experiments for this purpose were carried out at Cambridge from 1953 to 1957.

1. Two experiments are described in which the effects of grazing a lucerne/cocksfoot ley at various intervals throughout the season were examined, together with the effects of a June hay cut and an autumn rest period. The results are discussed.
2. In the year of treatment: (a) The wet year 1954 favoured the grass fraction and the dry year 1955 favoured the lucerne fraction but, in general, the proportion of lucerne in the sward decreased as frequency of defoliation increased. (b) The largest yields of lucerne dry matter were obtained from treatments that included a June hay cut and the largest yields of grass dry matter came from those treatments that included an autumn rest period, (c) Within the chosen programmes of defoliation, there was a tendency for greater yield of dry matter from those treatments defoliated four times when compared with one cut three times, but yields from treatments defoliated more frequently tended to be smaller, with a marked reduction from seven grazings.
3. In subsequent years: (a) Treatments in the wet year had a greater effect on subsequent lucerne vigour than treatments in the dry year, (b) Yields of lucerne dry matter from three hay cuts in the first residual year were reduced by all the grazing treatments, the greatest difference occurring at the first cut. Lucerne yield decreased with increasing frequency of treatment defoliation. There was a tendency for the autumn rest period to increase lucerne yield and for the June hay cut to reduce it. (c) Total grass dry matter yield was greatly increased by grazing treatments, the majority of this difference occurring at the first cut, and it also increased with increasing frequency of treatment defoliation. (d) The proportion of cocksfoot in the grass fraction of the sward was reduced by treatments that included defoliation during the period September–October, and it was further reduced by treatments that included a June hay cut. (e) In the second year following treatment the effect of grazing on the grass fraction had disappeared but reductions in lucerne vigour from too frequent defoliation were still evident, though reduced.
4. Grazing a lucerne/cocksfoot ley reduced the subsequent vigour of the lucerne more than would have been expected from cutting it a similar number of times. This reduction is attributed to increased competition from the grass arising from its response to nutrients in the excreta of the grazing animals. In a wet year four grazings caused a permanent reduction in lucerne vigour and more frequent grazing almost eliminated the lucerne, but in a dry year four grazings caused only a temporary reduction in lucerne vigour and had no lasting effect.
5. The interval between defoliation and the subsequent onset of flowering in the lucerne was very much less in the hot dry year than in the cooler wet year. There appeared to be a relationship between the onset of flowering and the ability of the lucerne to recover from defoliation.

1. Two experiments designed to investigate the effects of different times of ploughing a clover ley on the growth and yield of the subsequent winter wheat crop are described and the results discussed.
2. Although the different times of ploughing led to considerable differences in the growth of the wheat, their effect on yield was not consistent. This in agreement with the results of an earlier series of experiments. The effects of time of ploughing are attributed mainly to the resulting differences in the physical condition of the seed-beds.
Silicate Clay Platelet Dispersion in a Polymer Matrix

The use of dynamic light scattering to monitor the dispersion of clay platelets in a polymer matrix is described. The dispersed clay/polymer composite was prepared by joining surfactant monomers to synthetic hectorite clay platelets by a cationic exchange reaction and subsequently polymerizing with acrylamide. Dispersion of the organically modified clay in aqueous solution was facilitated by preparing a clay surfactant precursor suspension with a surfactant concentration significantly above the clay cation exchange capacity. At lower surfactant concentrations the clay does not disperse but instead forms large polydisperse aggregates unsuitable for making homogeneous composites. The presence of these aggregates could not be found in an x-ray powder diffraction scan but were clearly detected using dynamic light scattering.

The rapid annealing of implant damage using thermal radiation has been shown to be a production-worthy method of achieving good activation and minimal dopant redistribution of implanted species. The method involves the shuttered exposure of a standard 3" or 4" silicon wafer to a uniform thermal radiation front produced by a graphite heater for short times (10–30 sec). The propagation of residual damage observed by TEM is significantly less than that produced by furnace annealing and device structures annealed have electrical characteristics comparable to standard furnace anneals.
