Implementing nasal povidone-iodine decolonization to reduce infections in hemodialysis units: a qualitative assessment

Background:
A substantial proportion of patients undergoing hemodialysis carry Staphylococcus aureus in their noses, and carriers are at increased risk of S. aureus bloodstream infections. Our pragmatic clinical trial implemented nasal povidone-iodine (PVI) decolonization for the prevention of bloodstream infections in the novel setting of hemodialysis units.
Objective:We aimed to identify pragmatic strategies for implementing PVI decolonization among patients in outpatient hemodialysis units.
Design:Qualitative descriptive study.
Setting:Outpatient hemodialysis units affiliated with five US academic medical centers. Units varied in size, patient demographics, and geographic location.
Interviewees:Sixty-six interviewees including nurses, hemodialysis technicians, research coordinators, and other personnel.
Methods:We conducted interviews with personnel affiliated with all five academic medical centers and conducted thematic analysis of transcripts.
Results:Hemodialysis units had varied success with patient recruitment, but interviewees reported that patients and healthcare personnel (HCP) found PVI decolonization acceptable and feasible. Leadership support, HCP engagement, and tailored patient-focused tools or strategies facilitated patient engagement and PVI implementation. Interviewees reported both patients and HCP sometimes underestimated patients’ infection risks and experienced infection-prevention fatigue. Other HCP barriers included limited staffing and poor staff engagement. Patient barriers included high health burdens, language barriers, memory issues, and lack of social support.
Conclusion:Our qualitative study suggests that PVI decolonization would be acceptable to patients and clinical personnel, and implementation is feasible for outpatient hemodialysis units. Hemodialysis units could facilitate implementation by engaging unit leaders, patients and personnel, and developing education for patients about their infection risk.

Objectives:
Data from neurocognitive assessments may not be accurate in the context of factors impacting validity, such as disengagement, unmotivated responding, or intentional underperformance. Performance validity tests (PVTs) were developed to address these phenomena and assess underperformance on neurocognitive tests. However, PVTs can be burdensome, rely on cutoff scores that reduce information, do not examine potential variations in task engagement across a battery, and are typically not well-suited to acquisition of large cognitive datasets. Here we describe the development of novel performance validity measures that could address some of these limitations by leveraging psychometric concepts using data embedded within the Penn Computerized Neurocognitive Battery (PennCNB).
Methods:We first developed these validity measures using simulations of invalid response patterns with parameters drawn from real data. Next, we examined their application in two large, independent samples: 1) children and adolescents from the Philadelphia Neurodevelopmental Cohort (n = 9498); and 2) adult servicemembers from the Marine Resiliency Study-II (n = 1444).
Results:Our performance validity metrics detected patterns of invalid responding in simulated data, even at subtle levels. Furthermore, a combination of these metrics significantly predicted previously established validity rules for these tests in both developmental and adult datasets. Moreover, most clinical diagnostic groups did not show reduced validity estimates.
Conclusions:These results provide proof-of-concept evidence for multivariate, data-driven performance validity metrics. These metrics offer a novel method for determining the performance validity for individual neurocognitive tests that is scalable, applicable across different tests, less burdensome, and dimensional. However, more research is needed into their application.

Good education requires student experiences that deliver lessons about practice as well as theory and that encourage students to work for the public good—especially in the operation of democratic institutions (Dewey 1923; Dewy 1938). We report on an evaluation of the pedagogical value of a research project involving 23 colleges and universities across the country. Faculty trained and supervised students who observed polling places in the 2016 General Election. Our findings indicate that this was a valuable learning experience in both the short and long terms. Students found their experiences to be valuable and reported learning generally and specifically related to course material. Postelection, they also felt more knowledgeable about election science topics, voting behavior, and research methods. Students reported interest in participating in similar research in the future, would recommend other students to do so, and expressed interest in more learning and research about the topics central to their experience. Our results suggest that participants appreciated the importance of elections and their study. Collectively, the participating students are engaged and efficacious—essential qualities of citizens in a democracy.

Objectives: Health technology assessment (HTA) yields information that can be ideally used to address deficiencies in health systems and to create a wider understanding of the impact of different policy considerations around technology reimbursement and use. The structure of HTA programs varies across different jurisdictions according to decision-maker needs. Moreover, conducting HTA requires specialized skills. Effective decision making should include multiple criteria (medical, economic, technical, ethical, social, legal, and cultural) and requires multi-disciplinary teams of experts working together to produce these assessments. A workshop explored the multi-disciplinary skills and competencies required to build an effective and efficient HTA team, with a focus on low- and middle-income settings.
Methods: This proceeding summarizes main points from a workshop on capacity building, drawing on presentations and group discussions among attendees including different points of view.
Results and Conclusions: The workshop and thus this study would have benefited from a larger variety of stakeholders. Therefore, the conclusions arising from the workshop are not the opinion of a representative sample of HTA professionals. Nonetheless, organizations and speakers were carefully selected to provide a valuable approach to this theme. Thus, these proceedings highlight some of the gaps and needs in the education and training programs offered worldwide and calls for further investigation.

Summary
FIRST ENCOUNTERS WITH THE FORM
LAFCADIO HEARN (1850–1904, native of the British Isles who took Japanese citizenship) and Basil Hall Chamberlain (1850–1935, English Japanese scholar), both writing around the turn of the twentieth century, are thought to be the first to introduce readers in the British Isles to the Japanese haiku. Both employed the older term hokku, rather than ‘haiku’; not a mistake, as the Japanese reformist Masaoki Shiki had only recently preferred the use of ‘haiku’. But they did miss a mark by describing the hokku as ‘the Japanese epigram’.
Hearn offers ‘a small selection of hokku ’ in his Kwaidan - Stories and Studies of Strange Things (1904); and it is very much as ‘strange things’ he presents these minimalist poems to us. He could use them, he thought, to illustrate ‘Japanese interest in the aesthetic side of the subject’ of butterflies. He has no real belief that the hokku might ever catch on as a form practised by Western poets:
The taste for Japanese poetry of the epigrammatic sort is a taste that must be slowly acquired; and it is only by degrees, after patient study, that the possibilities of such composition can be fairly estimated. Hasty criticism has declared that to put forward any serious claim on behalf of seventeen syllables ‘would be absurd’.
A hundred years later, James Fenton, Professor of Poetry at Oxford, writing in An Introduction to English Poetry, was still finding the idea of assimilation absurd:
The most familiar form of syllabic verse is the haiku, borrowed from the Japanese, in which the poem adds up to seventeen syllables divided into three lines of five, seven and five syllables respectively. To me this seems like an oriental tradition which, however enthusiastically adopted (particularly in schools, I find), is unlikely to have an equivalent effect in the West. A bit like the tea ceremony.
This essay will seek to convince the reader that Fenton's opinion is far from the truth.
Lafcadio Hearn's versions take the form of a single sentence of prose, not formally presented as a monostich.

BACKGROUND
Influenza A (H1N1) pdm09 became the predominant circulating strain in the United States during the 2013–2014 influenza season. Little is known about the epidemiology of severe influenza during this season.
METHODSA retrospective cohort study of severely ill patients with influenza infection in intensive care units in 33 US hospitals from September 1, 2013, through April 1, 2014, was conducted to determine risk factors for mortality present on intensive care unit admission and to describe patient characteristics, spectrum of disease, management, and outcomes.
RESULTSA total of 444 adults and 63 children were admitted to an intensive care unit in a study hospital; 93 adults (20.9%) and 4 children (6.3%) died. By logistic regression analysis, the following factors were significantly associated with mortality among adult patients: older age (>65 years, odds ratio, 3.1 [95% CI, 1.4–6.9], P=.006 and 50–64 years, 2.5 [1.3–4.9], P=.007; reference age 18–49 years), male sex (1.9 [1.1–3.3], P=.031), history of malignant tumor with chemotherapy administered within the prior 6 months (12.1 [3.9–37.0], P<.001), and a higher Sequential Organ Failure Assessment score (for each increase by 1 in score, 1.3 [1.2–1.4], P<.001).
CONCLUSIONRisk factors for death among US patients with severe influenza during the 2013–2014 season, when influenza A (H1N1) pdm09 was the predominant circulating strain type, shifted in the first postpandemic season in which it predominated toward those of a more typical epidemic influenza season.

Although beta-amyloid, anxiety and depression have been linked cross-sectionally to reduced memory function in healthy older adults without dementia, prospective data evaluating these associations are lacking. Using data from an observational cohort study of 178 healthy older adults without dementia followed for 3 years, we found that anxiety symptoms significantly moderated the relationship between beta-amyloid level and decline in verbal (Cohen's d = 0.65) and episodic (Cohen's d = 0.38) memory. Anxiety symptoms were additionally linked to greater decline in executive function, irrespective of beta-amyloid and other risk factors. These findings suggest that interventions to mitigate anxiety symptoms may help delay memory decline in otherwise healthy older adults with elevated beta-amyloid.

Saproxylic insect assemblages inhabiting dead wood in Canadian forests are highly diverse and variable but quite poorly understood. Adequate assessment of these assemblages poses significant challenges with respect to sampling, taxonomy, and analysis. Their assessment is nonetheless critical to attaining the broad goals of sustainable forest management because such species are disproportionately threatened elsewhere by the reductions in dead wood generally associated with commercial exploitation of northern forests. The composition of the saproxylic fauna is influenced by many factors, including tree species, degree of decay, stand age, and cause of tree death. Wildfire and forest harvesting have differential impacts on saproxylic insect assemblages and on their recovery in postdisturbance stands. Exploration of saproxylic insect responses to variable retention harvesting and experimental burns is contributing to the development of prescriptions for conserving saproxylic insects in boreal forests. Understanding of processes that determine diversity patterns and responses of saproxylic insects would benefit from increased attention to natural history. Such work should aim to provide a habitat-classification system for dead wood to better identify habitats (and associated species) at risk as a result of forest management. This tool could also be used to improve strategies to better maintain saproxylic organisms and their central nutrient-cycling functions in managed forests.

Three Years of SETI@home: A Status Report

The SETI@home project has recently completed its third year of active data analysis. Over 4 million volunteers have joined the search, providing a combined total of over 1 million CPU-years of processing power. SETI@home performs a sensitive search for extraterrestrial signals in a 2.5 MHz band centered on 1420 MHz. SETI@home searches a wide parameter space including 14 octaves of signal bandwidth and 15 octaves of pulse period with Doppler drift corrections from −50 Hz/s to +50 Hz/s. We will briefly describe the SETI@home project and the algorithms used in the SETI@home client. We will describe the post-processing methods we use to reject RFI and select candidate signals from the nearly 4 billion “hits” returned by SETI@home clients.
Imprinting of distal mouse chromosome 2 is associated with phenotypic anomalies in utero

Previous studies have shown that the distal region on mouse chromosome (Chr) 2 is subject to imprinting as mice with maternal duplication/paternal deficiency (MatDp.dist2) and the reciprocal (PatDp.dist2) for this region exhibit phenotypic anomalies at birth and die neonatally. We show here that imprinting effects are detectable in utero. Notably PatDp.dist2 embryos show an increase in wet weight compared with normal, which peaks at 16·5 d post coitum (dpc), and diminishes by birth, whereas the wet weight of placenta is slightly reduced in the latter half of gestation. Newborns have increased length of the long bones. By contrast, the wet weight of MatDp.dist2 embryos decreases during the second half of gestation. Measurements of dry weights of embryos at 16·5 dpc have indicated that there is no difference in either PatDp.dist2 or MatDp.dist2 compared with normal so that the wet weight differences are due to fluid retention in PatDp.dist2 but fluid loss in MatDp.dist2. In PatDp.dist2 embryos excess fluid is particularly prominent in the subcuticular skin layer, whereas by birth fluid is evident around the neck and tongue. At 16·5 dpc the PatDp.dist2 embryos are severely oedematous, as the average fluid content per unit dry weight per embryo was increased by 40%, whereas the MatDp.dist2 embryos are dehydrated as the average water content per unit dry weight per embryo was reduced by 6%. A preliminary conclusion is that there is neither growth enhancement in PatDp.dist2 nor growth retardation in MatDp.dist2 offspring.

Longitudinal neuropsychological outcome in infants and preschoolers with traumatic brain injury

Neuropsychological outcome was evaluated in a prospective, longitudinal follow-up study of children age 4 months to 7 years at injury with either mild-to-moderate (N = 35) or severe (N = 44) traumatic brain injury (TBI). Age-appropriate tests were administered at baseline, 6 months, 12 months, and 24 months after the injury. Performance was compared on (1) composite IQ and motor, (2) receptive and expressive language, and (3) Verbal and Perceptual–Performance IQ scores. In comparison to mild-to-moderate TBI, severe TBI in infants and preschoolers produced deficits in all areas. Interactions between task and severity of injury were obtained. Motor scores were lower than IQ scores, particularly after severe TBI. Both receptive and expressive scores were reduced following severe TBI. Expressive language scores were lower than receptive language scores for children sustaining mild-to-moderate TBI. While severe TBI lowered both Verbal and Perceptual– Performance IQ scores, Verbal IQ scores were significantly lower than Perceptual–Performance IQ scores after mild-to-moderate TBI. Mild injuries may produce subtle linguistic changes adversely impacting estimates of Verbal IQ and expressive language. Within the limited age range evaluated within this study, age at injury was unrelated to test scores: The impact of TBI was comparable in children ages 4 to 41 months versus 42 to 72 months at the time of injury. All neuropsychological scores improved significantly from baseline to the 6-month follow-up. However, no further change in scores was observed from 6 to 24 months after the injury. The persistent deficits and lack of catch-up over time suggest a reduction in the rate of acquisition of new skills after severe TBI. Methodological issues in longitudinal studies of young children were discussed. (JINS, 1997, 3, 581–591.)
A New Major Seti Project Based on Project Serendip Data and 100,000 Personal Computers

We are now developing an innovative SETI project, tentatively named seti@home, involving massively parallel computation on desktop computers scattered around the world. The public will be uniquely involved in a real scientific project. Individuals will download a Screensaver program that will not only provide the usual attractive graphics when their computer is idle, but will also perform sophisticated analysis of SETI data using the host computer. The data are tapped off Project Serendip IV’s receiver and SETI survey operating on the 305-meter diameter Arecibo radio telescope. We make a continuous tape-recording of a 2 MHz bandwidth signal centered on the 21 cm H I line. The data on these tapes are then preliminarily screened and parceled out by a server that supplies small chunks of data (50 sec of 20 kHz bandwidth, a total of 0.25 MB) over the Internet to clients possessing the screen-saver software. After the client computer has automatically analyzed a complete chunk of data (in a much more detailed manner than Serendip normally does) a report on the best candidate signals is sent back to the server, whereupon a new chunk of data is sent out. If 50,000-100,000 customers can be achieved, the computing power will be equivalent to a substantial fraction of a typical supercomputer, and seti@home will cover a comparable volume of parameter space to that of Serendip IV.
A Controlled Trial of Cognitive–Behavioural Treatment of Hypochondriasis

Background
Hypochondriasis is a distressing disorder for which there is no adequate established treatment. This study evaluates the effect of cognitive–behavioural treatment of hypochondriasis, compared with a waiting list control.
MethodThirty-two patients were randomly assigned to either cognitive–behavioural therapy or a no treatment waiting list control. Cognitive–behavioural treatment consisted of 16 individual treatment sessions over a four-month period. The waiting list control lasted for four months and was followed by 16 sessions of cognitive–behavioural treatment. Assessments were made before allocation and after treatment or waiting list control. Patients who had cognitive–behavioural treatment were reassessed three months after completion of treatment.
ResultsPaired comparisons on post-treatment/wait scores indicated that the cognitive–behavioural group showed significantly greater improvements than the waiting list on all but one patient rating, all therapist ratings and all assessor ratings. After three months the benefits of therapy were maintained.
ConclusionsCognitive–behavioural treatment is an effective therapy for hypochondriasis. Implications of the study are discussed.
A Study of the Etiologies and Treatment of Nosocomial Pneumonia in a Community-Based Teaching Hospital

Objective:
To compare the frequency of the pathogens of nosocomial pneumonia in a community-based teaching hospital to the frequencies previously published, and to evaluate recommendations for the therapy of nosocomial pneumonia in this setting.
Design:Retrospective review of prospectively acquired data accrued during 9 randomized single-blinded and 4 single-agent investigational antibiotic studies for the therapy of pneumonia in hospitalized patients between 1981 and 1989.
Setting:The study was performed at a university affiliated, community-based teaching Department of Veterans Affairs Medical Center.
Patients:Patients were hospitalized on the acute medical/surgical and intermediate medicine wards. Informed consent was obtained prior to enrolling patients into the respective antimicrobial studies. Pneumonia was documented radiographicahy and clinically for each patient.
Results:Two hundred thirty-one episodes of nosocomial pneumonia were treated. Overall, 51% of pneumonias were caused by Streptococcus pneumoniae or Hemophilus influenzae with or without other organisms that were not gram-negative bacilli. Gram-negative bacilli, with or without other organisms, accounted for only 26% of all nosocomial pneumonias. Overall, monotherapy with a cephalosporin (usually a broad-spectrum agent) was equally efficacious compared with combination therapy (87% versus 81%, respectively). Cure rates for nosocomial pneumonias from gram-negative bacilli treated with these 2 therapies also were similar (70% versus 60%, respectively).
Conclusions:In nontertiary care settings, gram-negative bacilli may cause fewer episodes of nosocomial pneumonia (26% in this study) than noted by previously published reports, which indicated that these organisms account for 50% of nosocomial pneumonias. Further, S pneumoniae and H influenzae may account etiologicahy for many of these nosocomial pneumonias. Monotherapy with an extended-spectrum cephalosporin may be more appropriate than combined treatment with a b-lactam and an aminoglycoside in a nontertiary care setting, thereby reducing potential toxicity in an older, hospitalized patient population.
