Development of a model to predict antidepressant treatment response for depression among Veterans

Background
Only a limited number of patients with major depressive disorder (MDD) respond to a first course of antidepressant medication (ADM). We investigated the feasibility of creating a baseline model to determine which of these would be among patients beginning ADM treatment in the US Veterans Health Administration (VHA).
MethodsA 2018–2020 national sample of n = 660 VHA patients receiving ADM treatment for MDD completed an extensive baseline self-report assessment near the beginning of treatment and a 3-month self-report follow-up assessment. Using baseline self-report data along with administrative and geospatial data, an ensemble machine learning method was used to develop a model for 3-month treatment response defined by the Quick Inventory of Depression Symptomatology Self-Report and a modified Sheehan Disability Scale. The model was developed in a 70% training sample and tested in the remaining 30% test sample.
ResultsIn total, 35.7% of patients responded to treatment. The prediction model had an area under the ROC curve (s.e.) of 0.66 (0.04) in the test sample. A strong gradient in probability (s.e.) of treatment response was found across three subsamples of the test sample using training sample thresholds for high [45.6% (5.5)], intermediate [34.5% (7.6)], and low [11.1% (4.9)] probabilities of response. Baseline symptom severity, comorbidity, treatment characteristics (expectations, history, and aspects of current treatment), and protective/resilience factors were the most important predictors.
ConclusionsAlthough these results are promising, parallel models to predict response to alternative treatments based on data collected before initiating treatment would be needed for such models to help guide treatment selection.

Background
Fewer than half of patients with major depressive disorder (MDD) respond to psychotherapy. Pre-emptively informing patients of their likelihood of responding could be useful as part of a patient-centered treatment decision-support plan.
MethodsThis prospective observational study examined a national sample of 807 patients beginning psychotherapy for MDD at the Veterans Health Administration. Patients completed a self-report survey at baseline and 3-months follow-up (data collected 2018–2020). We developed a machine learning (ML) model to predict psychotherapy response at 3 months using baseline survey, administrative, and geospatial variables in a 70% training sample. Model performance was then evaluated in the 30% test sample.
Results32.0% of patients responded to treatment after 3 months. The best ML model had an AUC (SE) of 0.652 (0.038) in the test sample. Among the one-third of patients ranked by the model as most likely to respond, 50.0% in the test sample responded to psychotherapy. In comparison, among the remaining two-thirds of patients, <25% responded to psychotherapy. The model selected 43 predictors, of which nearly all were self-report variables.
ConclusionsPatients with MDD could pre-emptively be informed of their likelihood of responding to psychotherapy using a prediction tool based on self-report data. This tool could meaningfully help patients and providers in shared decision-making, although parallel information about the likelihood of responding to alternative treatments would be needed to inform decision-making across multiple treatments.

Background
Positive symptoms are a useful predictor of aggression in schizophrenia. Although a similar pattern of abnormal brain structures related to both positive symptoms and aggression has been reported, this observation has not yet been confirmed in a single sample.
MethodTo study the association between positive symptoms and aggression in schizophrenia on a neurobiological level, a prospective meta-analytic approach was employed to analyze harmonized structural neuroimaging data from 10 research centers worldwide. We analyzed brain MRI scans from 902 individuals with a primary diagnosis of schizophrenia and 952 healthy controls.
ResultsThe result identified a widespread cortical thickness reduction in schizophrenia compared to their controls. Two separate meta-regression analyses revealed that a common pattern of reduced cortical gray matter thickness within the left lateral temporal lobe and right midcingulate cortex was significantly associated with both positive symptoms and aggression.
ConclusionThese findings suggested that positive symptoms such as formal thought disorder and auditory misperception, combined with cognitive impairments reflecting difficulties in deploying an adaptive control toward perceived threats, could escalate the likelihood of aggression in schizophrenia.
Temporal dynamics of seedling emergence among four fire ephemerals: the interplay of after-ripening and embryo growth with smoke

The flora of Mediterranean ecosystems contains families with species having fully and under-developed embryos in their seeds. After-ripening for physiological dormancy release and smoke influence germination in many species. We investigated how after-ripening and embryo growth interact with smoke to influence the temporal dynamics of seedling emergence among fire ephemerals. Seeds were placed in the field and under standardized (50% relative humidity, 30°C) laboratory conditions to test the effects of summer conditions on physiological dormancy loss. Germination was tested with water or smoke compounds (smoke water, KAR1) at a simulated autumn/winter temperature (18/7°C). The timing and amount of seedling emergence with smoke was observed for seeds exposed to near-natural conditions. During summer, physiological dormancy was broken in all species, enabling germination at autumn/winter but not summer temperatures; no embryo growth occurred in seeds with under-developed embryos. At the start of the wet season, seedling emergence from seeds with fully developed embryos occurred earlier than from seeds with under-developed embryos. In a non-consistent manner among our study species, smoke and smoke compounds influenced the rate of embryo growth and amount of germination. Effects of smoke were noticeable in terms of number of emergents in the first emergence season. Among ecologically similar species, we have shown (1) that both thermal and embryo traits exclude germination in the summer, (2) how embryo size influences the timing of seedling emergence in autumn–winter, and (3) a reduced requirement for smoke in the second emergence season after a fire with a shift to reliance on seasonal cues for emergence.
Tall Ironweed (Vernonia altissima Nutt.) Control in Pastures with Fall-Applied Herbicides

Tall ironweed is a troublesome perennial weed that infests cool-season grass pastures in Kentucky. Field experiments were conducted in 2000 through 2003 to evaluate the efficacy of fall-applied herbicides on established tall ironweed following a midsummer mowing. Triclopyr-containing treatments showed the greatest suppression of tall ironweed 12 mo after treatment (MAT), across all years. With triclopyr at 0.56 and 0.63 kg/ha, tall ironweed control was 80% or greater in 2 of the 3 yr. Dicamba initially provided 87% control 8 MAT in 2 of 3 yr and declined to less than 60% 12 MAT. Tall ironweed shoot density was also reduced 66% or more 12 MAT with fall-applied triclopyr-containing treatments. In contrast, tall ironweed density increased approximately twofold in dicamba-treated plots between 8 to 12 MAT in all 3 yr. The impact of herbicide treatment on dry matter (DM) yield of spring-seeded red clover (Trifolium pratense L.), tall ironweed, and forage grasses was also evaluated. Red clover DM yield in the herbicide-treated plots in 2002 showed no significant differences from the untreated control. However, red clover DM yield in 2003 was lowest for the two triclopyr + clopyralid treatments, indicating a decrease in DM production compared with that of the nontreated control. Results indicated that fall-applied triclopyr-containing herbicides following a midsummer mowing is an effective program for removing tall ironweed from grass pastures, but further research is needed to evaluate the establishment of red clover following herbicide treatment.

Background and objective:
For the conduct of controlled clinical trials, epidemiologic surveys or even of medical practice of varieties of peripheral neuropathy, the usefulness, error rate and cost-effectiveness of scannable case-report forms has not been studied. Materials and
Methods:The overall performance, the frequency of the problems identified and corrected, and the time saved from use of a standard paper case report form was evaluated in multicenter treatment trials, single center epidemiologic surveys and in our neurologic practice. The paper case report form (Clinical Neuropathy Assessment [CNA]) for pen entry at study medical centers for patient, disease and demographic information (Lower Limb Function [LLF] and Neuropathy Impairment Score [NIS]) can be faxed to a core Reading and Quality Assurance Center where the form and data is electronically and interactively evaluated and corrected, if needed, by participating medical centers before electronic entry into database.
Observations and conclusions:1) The approach provides a standard, scannable paper case report form for pen entry of neuropathy symptoms, impairments and disability at the bedside or in the office which is retained as a source document at the participating medical center but a facsimile can be transferred instantaneously, its data can be programmed, interactively evaluated, modified and stored while maintaining an audit trail; 2) it allowed efficient and accurate reading, transfer, analysis, and storage of data of more than 15,000 forms used in multicenter trials; 3) in 500 consecutive CNA evaluations, software programs identified and facilitated interactive corrections of omissions, discrepancies, and disease and study inconsistencies, introducing only a few readily identified and corrected entry errors; and 4) use of programmed, as compared to non-programmed assessment, was more accurate than double keyboard entry of data and was approximately five times faster.

Imidazoles present a tunable, versatile and economical platform for the development of novel liquid solvents and polymer membranes for CO2 capture. An overview of our studies in this area is presented, with emphasis on characterization of structure-property relationships in imidazole-based materials through both experimental and computational studies. To this end, a growing library of systematically varied imidazole compounds has been synthesized using only commercial available starting materials and straightforward reactions. Using this library of compounds, we have sought to understand and develop predictive models for thermophysical properties relating to process design, including: density, viscosity, vapor pressure, pKa and CO2 absorption capacity. Furthermore, we have discovered that imidazoles are stable in the presence of SO2 and can form reversible 1:1 adducts, which can be beneficial as SO2 is typically present at ppm levels alongside CO2 in flue gas from coal-fired power plants.

The electronic properties of ThO2 single crystals were studied using x-ray photoemission spectroscopy (XPS). The XPS results show that the Th 4f core level is in an oxidation state that is consistent with that expected for Th in ThO2. The effective Debye temperature is estimated from the temperature dependent photoemission intensities of the Th 4f core level over the temperature range of 290 to 360 K. A Debye temperature of 468±32 K has been determined.

Previous studies have shown that a deficiency in DNA damage repair is associated with increased cancer risk, and exposure to UV radiation is a major risk factor for the development of malignant melanoma. High density of common nevi (moles) is a major risk factor for cutaneous melanoma. A nevus may result from a mutation in a single UV-exposed melanocyte which failed to repair DNA damage in one or more critical genes. XRCC3 and XRCC5 may have an effect on nevus count through their function as components of DNA repair processes that may be involved directly or indirectly in the repair of DNA damage due to UV radiation. This study aims to test the hypothesis that the frequency of flat or raised moles is associated with polymorphism at or near these DNA repair genes, and that certain alleles are associated with less efficient DNA repair, and greater nevus density. Twins were recruited from schools in south eastern Queensland and were examined close to their 12th birthday. Nurses examined each individual and counted all moles on the entire body surface. A 10cM genome scan of 274 families (642 individuals) was performed and microsatellite polymorphisms in XRCC3 and adjacent to XRCC5 were also typed. Linkage and association of nevus count to these loci were tested simultaneously using a structural-equation modeling approach implemented in MX. There is weak evidence for linkage of XRCC5 to a QTL influencing raised mole count, and also weak association. There is also weak evidence for association between flat mole count and XRCC3. No tests were significant after correction for testing multiple alleles, nor were any of the tests for total association significant. If variation in XRCC3 or XRCC5 influences UV sensitivity, and indirectly affects nevus density, then the effects are small.

Introduction. Rose apple fruit ripens on the tree, has a thin peel, soft flesh and deteriorates quickly after harvest. Water loss contributes to rapid deterioration. 1-MCP can extend the postharvest life of non-climacteric fruits either directly through its effect as an ethylene blocker or through indirect effects. Materials and methods. We investigated the effect of 1-MCP (1 µL·L–1) on shelf life and quality of Rose apple fruits (Syzygium jambos Alston) cv. Tabtim Chan. Fruit were exposed to 1-MCP for (0, 6, 12, 18 or 24) h, then stored at (15 ± 1) °C for 12 days. Subsequently, we investigated the effect of 1-MCP (1 µL·L–1) for 12 h on fruit stored at 5 °C or 10 °C. Weight loss, skin colour, total soluble solids, fruit firmness and percentage of diseased fruit were measured during storage. Results. 1-MCP reduced weight loss from about 13% of initial weight to 6%, irrespective of the time of exposure. 1-MCP applied to fruit for 12 h or 18 h increased firmness from 3.2 N to 3.8 N and more than halved the percentage of fruit showing disease. Exposure to 1-MCP for 12 h or 18 h was optimum for reducing disease. 1-MCP for 12 h and storage at (10 ± 1) °C doubled shelf life to 24 days compared with fruit stored at the same temperature, but without 1-MCP. Storage at less than 10 °C did not extend shelf life any further. Conclusion. 1-MCP extended the shelf life of Rose apple fruit by reducing weight loss, maintaining flesh firmness and slowing disease development.

Introduction. The peel of bananas that ripen at ≥ 24 ℃ does not turn fully yellow, but retains a green colour, reducing the commercial value of the fruit. Historically, in experimental studies of this problem, fruit are ripened at constant temperatures; for example, either 20 ℃ or 30 ℃. In our experiments, we systematically varied the temperature of banana storage during ripening to investigate early events in ripening that may determine whether or not the peel remained green. Materials and methods. Fruits were exposed to temperatures of 20 ℃ or 30 ℃ for short periods before then storing at 30 ℃ or 20 ℃ to complete ripening. The L* a* b* colour system was used to measure change in green colour in the banana peel. Results. If the temperature of the fruit stored was 20 ℃ for the first few hours (≥ 14 h), then the peel became fully yellow, even in fruit subsequently ripened at 30 ℃. Conversely, if the temperature of the bananas stored was 30 ℃ for the first few hours of ripening, then full yellow colour was not obtained even if fruits were subsequently ripened at 20 ℃. Conclusion. The temperature of the first few hours of banana ripening determines whether or not the peel reaches full yellow colour.
