Developing the WE BEAT Well-Being Education Programme to foster resilience and build connection in paediatric heart disease

Background:
The study of psychological well-being and related resilient outcomes is of increasing focus in cardiovascular research. Despite the critical importance of psychological well-being and related resilient outcomes in promoting optimal cardiac health, there have been very few psychological interventions directed towards children with heart disease. This paper describes the development and theoretical framework of the WE BEAT Wellbeing Education Program, a group-based psychoeducation and coping skills training intervention designed to improve psychological well-being and resilience in adolescents with paediatric heart disease.
Methods:Program development was informed by patient and family needs and input gathered via large, international survey methods as well as qualitative investigation, a theoretical framework, and related resilience intervention research.
Results:An overview of the WE BEAT intervention components and structure of the programme is provided.
Conclusions:The WE BEAT Wellbeing Education Program was developed as one of the first resiliency-focused interventions in paediatric heart disease with an overall objective to foster positive psychological well-being and resilient outcomes through a health promotion and prevention lens in an accessible format while providing access to safe, peer-to-peer community building. Feasibility pilot results are forthcoming. Future directions include mobile app-based delivery and larger-scale efficacy and implementation trials.

Observational studies suggest that 25-hydroxy vitamin D (25(OH)D) concentration is inversely associated with pain. However, findings from intervention trials are inconsistent. We assessed the effect of vitamin D supplementation on pain using data from a large, double-blind, population-based, placebo-controlled trial (the D-Health Trial). 21 315 participants (aged 60–84 years) were randomly assigned to a monthly dose of 60 000 IU vitamin D3 or matching placebo. Pain was measured using the six-item Pain Impact Questionnaire (PIQ-6), administered 1, 2 and 5 years after enrolment. We used regression models (linear for continuous PIQ-6 score and log-binomial for binary categorisations of the score, namely ‘some or more pain impact’ and ‘presence of any bodily pain’) to estimate the effect of vitamin D on pain. We included 20 423 participants who completed ≥1 PIQ-6. In blood samples collected from 3943 randomly selected participants (∼800 per year), the mean (sd) 25(OH)D concentrations were 77 (sd 25) and 115 (sd 30) nmol/l in the placebo and vitamin D groups, respectively. Most (76 %) participants were predicted to have 25(OH)D concentration >50 nmol/l at baseline. The mean PIQ-6 was similar in all surveys (∼50·4). The adjusted mean difference in PIQ-6 score (vitamin D cf placebo) was 0·02 (95 % CI (−0·20, 0·25)). The proportion of participants with some or more pain impact and with the presence of bodily pain was also similar between groups (both prevalence ratios 1·01, 95 % CI (0·99, 1·03)). In conclusion, supplementation with 60 000 IU of vitamin D3/month had negligible effect on bodily pain.

The severe acute respiratory coronavirus virus 2 (SARS-CoV-2) delta variant is highly transmissible, and current vaccines may have reduced effectiveness in preventing symptomatic infection. Using epidemiological and genomic analyses, we investigated an outbreak of the variant in an acute-care setting among partially and fully vaccinated individuals. Effective outbreak control was achieved using standard measures.

This study presents enhanced surveillance data from 2004 to 2018 for all community-associated methicillin-resistant Staphylococcus aureus (CA-MRSA) specimens collected in Western Australia (WA), and describes the changing epidemiology over this period. A total of 57 557 cases were reviewed. Annual incidence rates increased from 86.2 cases per 100 000 population to 245.6 per 100 000 population (IRR = 2.9, CI95 2.7–3.0). The proportion of isolates carrying Panton–Valentine leucocidin (PVL)-associated genes increased from 3.4% to 59.8% (χ2 test for trend 7021.9, P < 0.001). The emergence of PVL-positive, ‘Queensland CA-MRSA’ (ST93-IV) and ‘WA 121’ (ST5-IV) accounted for the majority of increases in CA-MRSA across the study period. It is unclear why some clones are more prolific in certain regions. In WA, CA-MRSA rates increase as indices of temperature and humidity increase after controlling for socioeconomic disadvantage. We suggest climatic conditions may contribute to transmission, along with other socio-behavioural factors. A better understanding of the ability for certain clones to form ecological niches and cause outbreaks is required.

The object of this paper is to set down an overall picture of the practical aspects of compressor blade vibration. In doing so it is inevitable that a certain content is effectively a repeat of other people's contributions to the subject. The authors make no apology for this and offer the paper as a "state of the art" as practised at Bristol Siddeley Engines Ltd. In bringing together the many relevant issues of this problem, it is hoped that a useful presentation is obtained.
There is little hesitation in indicating the most obvious and important practical aspect of blade vibration as that of the inherent danger of blade failure and it is known that the axial flow compressor is susceptible to complete failure caused by the fatigue of one blade.
The multi-stage axial flow compressor is a highly complex aerodynamic machine with, its early and late stages required to operate over wide ranges of incidence and Mach number conditions. To some extent, of course, use of variable geometry can reduce this to less taxing proportions. The aerodynamic environment of front stages has broad boundaries somewhat wider than those relating to rear stages, while middle blade rows generally operate under sensibly constant conditions. This diminishing “severity” of operation, front, rear to middle stage blading is not unrelated to the incidence of blade vibration problems which shows a similar pattern.

Production of High Surface Area Activated Carbon from Coconut Husk

We present preliminary results on a processing protocol by chemical activation that transforms organic waste product such as coconut husk into high surface area activated carbon. Dried raw materials of the coconut husk were carbonized anaerobically into char. The char was impregnated with KOH of different ratios and were activated at 800°C and 900°C. The transmission electron microscope was used to acquire structural and morphological information of the activated carbon, and the surface area and porosity analysis were performed using Micromeritics ASAP 2020 analyzer. The activated carbons show both micropores and mesopores with specific surface area as high as 2900m2/g.

Silicone lymphadenopathy: an unexpected cause of neck lumps

Objective:
To report a rare case of silicone lymphadenopathy solely affecting the left supraclavicular lymph nodes.
Case report:Our patient presented with a painless swelling in the left supraclavicular region. Notably, she had previously undergone cosmetic breast augmentation using silicone-containing implants. Radiological imaging and subsequent excisional biopsy of the swelling produced findings consistent with a silicone foreign body reaction secondary to bilateral breast implant rupture.
Conclusion:Silicone lymphadenopathy following breast augmentation primarily affects the axillary nodes. Supraclavicular lymph node involvement is unusual. To our knowledge, this is the first report in the English language literature of silicone lymphadenopathy manifesting solely in the supraclavicular lymph nodes. Although the need to exclude malignancy in such cases is of the utmost importance, silicone lymphadenopathy should also be considered in the differential diagnosis. Fine needle aspiration cytology is a useful initial investigation, which may be followed up by excisional biopsy and histological analysis for further confirmatory diagnostic information.
Epidemiology of classic and AIDS-related Kaposi's sarcoma in the USA: incidence, survival, and geographical distribution from 1975 to 2005

This study aimed to examine trends in incidence, geographical distribution, and survival of classic and AIDS-related Kaposi's sarcoma (KS) in the general US population using Surveillance, Epidemiology, and End Results (SEER) tumour registries with 12 066 patients diagnosed with KS between 1975 and 2005. Although the age-adjusted standardized incidence ratio (SIR) of AIDS-related KS (1·9) during 1980–2005 was not significantly higher than that of classic KS (1·4) during 1975–2005 (P = 0·78), the trends in annual SIR rates revealed distinct patterns. While the SIR for AIDS-related KS declined across all registries from the early 1990s (4·6) to late-1990s (0·3) (P = 0·05), the SIR of classic KS remained relatively steady (1·7). In both forms the SIR of KS was highest in metropolitan areas. The 5-year survival rates for patients with AIDS-related KS improved from 12·1% (1980–1995) to 54% (1996–2005) (P = 0·05). Survival rates for patients with classic KS remained stable, ranging from 75·7% to 88·6% during the 30-year period. These results may reflect improved HIV treatment.

New Zealand is currently the only major fruit producing country in the world that is free of economically important fruit flies. As part of the effort to maintain this status, there is a need to supplement quarantine decision-making procedures with a means of rapidly identifying immature life stage infestations to the species level. Here we describe a molecular method that achieves this, using simple restriction patterns of ribosomal DNA (rDNA) as diagnostic markers. The 18S and 18S plus internal transcribed spacer (ITS) regions were amplified from larval DNA by the polymerase chain reaction (PCR). Nineteen species, spanning four genera (including five subgenera of Bactrocera) were analysed. Restriction analysis of the 18S PCR product provided poor resolution, even at the generic level. Digestion of the 18S + ITS PCR product, however, generated thirteen diagnostic haplotypes as defined by the composite restriction patterns from RsaI, Sau3a HaeIII and AluI. No variation was detected at these restriction sites within or between populations. Twenty two restriction enzymes have been screened, but diagnostic RFLPs have yet to been found for six out of the ten Bactrocera (Bactrocera) species; B. passiflorae (Froggatt) cannot be distinguished from B. facialis (Coquillet), nor B. kirki (Froggatt) from B. trilineola (Froggatt) or B. neohumeralis (Hardy) from B. tryoni (Froggatt). Geographic origin could assist in distinguishing the first four species, but the latter pair are very closely related with overlapping origins, hosts and adult morphology. All six species, however, are considered high risk with respect to their likely establishment in New Zealand. Therefore diagnosis based on this molecular technique would support the same quarantine decision. We consider this method could be useful as a diagnostic technique and discuss directions for further development.

Determination of the variation of absolute and relative electron-excited x-ray production rates as a function of electron beam energy and sample atomic number is necessary for calculation of the "stopping power" atomic number correction and the relative amount of characteristic fluorescence and for development of “standardless” and Monte Carlo algorithms for quantitative x-ray analysis. Critical to the calculation of x-ray production rates is an accurate expression for the inner shell electron ionization cross section. A large number of expressions have been proposed for the relative x-ray production rates (used in the fluorescence correction)1 and for the ionization cross section used in the atomic number correction, and these yield quite different results. In order to evaluate which expressions gave the most accurate results when applied to quantitative x-ray emission measurements, we performed a series of high precision measurements of x-ray intensities as a function of electron beam accelerating potential for a series of pure element and simple oxide, phosphide, sulfide, and chloride standards for 65 elements ranging in Z from C to U

Abstract
In a prospective, randomised, controlled trial to determine whether comprehensive lifestyle changes affect coronary atherosclerosis after 1 year, 28 patients were assigned to an experimental group (low-fat vegetarian diet, stopping smoking, stress management training, and moderate exercise) and 20 to a usual-care control group. 195 coronary artery lesions were analysed by quantitative coronary angiography. The average percentage diameter stenosis regressed from 40-0 (SD 16-9)% to 37-8 (16-5)% in the experimental group yet progressed from 42-7 (15-5)% to 46-1 (18-5)% in the control group. When only lesions greater than 50% stenosed were analysed, the average percentage diameter stenosis regressed from 61-1 (8-8)% to 55-8 (11-0)% in the experimental group and progressed from 61-7 (9-5)% to 64-4 (16-3)% in the control group. Overall, 82% of experimental-group patients had an average change towards regression. Comprehensive lifestyle changes may be able to bring about regression of even severe coronary atherosclerosis after only 1 year, without use of lipid-lowering drugs. Lancet 1990; 336: 129-33.
Introduction
The Lifestyle Heart Trial is the first randomised, controlled clinical trial to determine whether patients outside hospital can be motivated to make and sustain comprehensive lifestyle changes and, if so, whether regression of coronary atherosclerosis can occur as a result of lifestyle changes alone. Over twenty clinical trials are being carried out to determine whether the progression of coronary atherosclerosis can be modified; in all of these, cholesterol-lowering drugs, plasmapheresis, or partial ileal bypass surgery are the primary interventions.
We carried out trials in 1977 and 1980 to assess the short-term effects of lifestyle changes on coronary heart disease with non-invasive endpoint measures (improvements in cardiac risk factors, functional status, myocardial perfusion, and left ventricular function). However, the subjects of those studies were not living in the community during the trial, and we did not use angiography to assess changes in coronary atherosclerosis.
Patients and methods
Patients with angiographically documented coronary artery disease were randomly assigned to an experimental group or to a usual-care control group. Experimental-group patients were prescribed a lifestyle programme that included a low-fat vegetarian diet, moderate aerobic exercise, stress management training, stopping smoking, and group support. Control-group patients were not asked to make lifestyle changes, although they were free to do so. Progression or regression of coronary artery lesions was assessed in both groups by quantitative coronary angiography at baseline and after about a year.

As a technique for non-destructive materials analysis, computed tomography (CT) has been especially useful for studying the dependence of the structure of ceramics on manufacturing processes. CT also has been used for characterizing the lithology of reservoir cores while they are still contained in preservation material or a core barrel.
The parameter measured by CT is the X-ray attenuation coefficient, which is a function of both material density and material composition.

Experimentally it is shown that variation of axial spacing between neighbouring blade rows can alter the stall characteristics of a compressor stage. The stall cell patterns of this compressor stage are described in detail and changes of slope in stage temperature and pressure rise characteristics are explained in terms of stall propagation. Strain gauge tests on stator blades are used to indicate the possibility of large dynamic stresses occurring when the stall cell frequency is equal to the blade natural bending frequency.
