Diabetes in Appalachia: providers’ perspectives

Background:
Southeastern Appalachian Ohio has more than double the national average of diabetes and a critical shortage of healthcare providers. Paradoxically, there is limited research focused on primary care providers’ experiences treating people with diabetes in this region. This study explored providers’ perceived barriers to and facilitators for treating patients with diabetes in southeastern Appalachian Ohio.
Methods:We conducted in-depth interviews with healthcare providers who treat people with diabetes in rural southeastern Ohio. Interviews were transcribed, coded, and analyzed via content and thematic analyses using NVivo 12 software (QSR International, Chadstone, VIC, Australia).
Results:Qualitative analysis revealed four themes: (1) patients’ diabetes fatalism and helplessness: providers recounted story after story of patients believing that their diabetes was inevitable and that they were helpless to prevent or delay diabetes complications. (2) Comorbid psychosocial issues: providers described high rates of depression, anxiety, incest, abuse, and post-traumatic stress disorder among people with diabetes in this region. (3) Inter-connected social determinants interfering with diabetes care: providers identified major barriers including lack of access to providers, lack of access to transportation, food insecurity, housing insecurity, and financial insecurity. (4) Providers’ cultural understanding and recommendations: providers emphasized the importance of understanding of the values central to Appalachian culture and gave culturally attuned clinical suggestions for how to use these values when working with this population.
Conclusions:Evidence-based interventions tailored to Appalachian culture and training designed to increase the cultural competency and cultural humility of primary care providers may be effective approaches to reduce barriers to diabetes care in Appalachian Ohio.
Prepregnancy obesity is associated with lower psychomotor development scores in boys at age 3 in a low-income, minority birth cohort

Whether maternal obesity and gestational weight gain (GWG) are associated with early-childhood development in low-income, urban, minority populations, and whether effects differ by child sex remain unknown. This study examined the impact of prepregnancy BMI and GWG on early childhood neurodevelopment in the Columbia Center for Children’s Environmental Health Mothers and Newborns study. Maternal prepregnancy weight was obtained by self-report, and GWG was assessed from participant medical charts. At child age 3 years, the Psychomotor Development Index (PDI) and Mental Development Index (MDI) of the Bayley Scales of Infant Intelligence were completed. Sex-stratified linear regression models assessed associations between prepregnancy BMI and pregnancy weight gain z-scores with child PDI and MDI scores, adjusting for covariates. Of 382 women, 48.2% were normal weight before pregnancy, 24.1% overweight, 23.0% obese, and 4.7% underweight. At 3 years, mean scores on the PDI and MDI were higher among girls compared to boys (PDI: 102.3 vs. 97.2, P = 0.0002; MDI: 92.8 vs. 88.3, P = 0.0001). In covariate-adjusted models, maternal obesity was markedly associated with lower PDI scores in boys [b = −7.81, 95% CI: (−13.08, −2.55), P = 0.004], but not girls. Maternal BMI was not associated with MDI in girls or boys, and GWG was not associated with PDI or MDI among either sex (all-P > 0.05). We found that prepregnancy obesity was associated with lower PDI scores at 3 years in boys, but not girls. The mechanisms underlying this sex-specific association remain unclear, but due to elevated obesity exposure in urban populations, further investigation is warranted.
Sedimentary Proxy Evidence of a Mid-Holocene Hypsithermal Event in the Location of a Current Warming Hole, North Carolina, USA

A wetland deposit from the southern Appalachian mountains of North Carolina, USA, has been radiocarbon dated and shows continuous deposition from the early Holocene to the present. Non-coastal records of Holocene paleoenvironments are rare from the southeastern USA. Increased stable carbon isotope ratios (?13C) of sedimentary organic matter and pollen percentages indicate warm, dry early- to mid-Holocene conditions. This interpretation is also supported by n-alkane biomarker data and bulk sedimentary C/N ratios. These warm, dry conditions coincide with a mid-Holocene hypsithermal, or altithermal, documented elsewhere in North America. Our data indicate that the southeastern USA warmed concurrently with much of the rest of the continent during the mid-Holocene. If the current "warming hole" in the southeastern USA persists, during a time of greenhouse gas-induced warming elsewhere, it will be anomalous both in space and time.

A new procedure called single-word shadowing was applied to the study of lexical access in context. Subjects listened to word pairs or sentences recorded in one voice and were asked to repeat the target word signaled by a voice shift. This technique yielded rapid and robust priming effects in normal adult subjects in word pairs and in a sentence context. Regression analyses showed that the semantic priming effects were large and significant, even when several additional factors believed to affect lexical access were controlled. Evidence was found for robust semantic priming in the healthy elderly and in children from 7 to 11 years of age, and there was also evidence for a change in the size and nature of context effects across the lifespan. Because single-word shadowing works across a broad age range and does not require reading, secondary tasks, or metalinguistic judgments, it is a promising tool for the study of lexical access in a range of different populations.

Normals display selective deficits in morphology and syntax under adverse processing conditions. Digit loads do not impair processing of passives and object relatives but do impair processing of grammatical morphemes. Perceptual degradation and temporal compression selectively impair several aspects of grammar, including passives and object relatives. Hence we replicate Caplan & Waters's specific findings but reach opposite conclusions, based on wider evidence.

OBJECTIVE:
To assess the prevalence of tuberculosis (TB) or a positive skin test in healthcare workers (HCWs) providing services to human immunodeficiency virus (HIV)-infected individuals and to determine prospectively the incidence of new infections in this population.
DESIGN:This prospective cohort study enrolled 1,014 HCWs working with HIV-infected populations from 10 metropolitan areas. Purified protein derivative (PPD) tuberculin skin tests were placed at baseline and every 6 months afterwards on those without a history of TB or a positive PPD. Demographic, occupational, and TB exposure data also were collected.
SETTING:Outpatient clinics, hospitals, private practice offices, and drug treatment programs providing HIV-related healthcare and research programs.
PARTICIPANTS:A voluntary sample of staff and volunteers from 16 Community Programs for Clinical Research on AIDS units.
RESULTS:Factors related to prior TB or a positive skin test at baseline included being foreign-born, increased length of time in health care, living in New York City, or previous bacille Calmette-Guerín vaccination. The rate of PPD conversion was 1.8 per 100 person years of follow-up. No independent relation was found between the amount or type of contact with HIV-infected populations and the risk of TB infection.
CONCLUSION:These data provide some reassurance that caring for HIV-infected patients is not related to an increased rate of TB infection among HCWs in these settings.
