53 Change in Cerebral Metabolite Concentrations Following Bariatric Surgery

Objective:
Obesity is associated with adverse effects on brain health, including increased risk for neurodegenerative diseases. Changes in cerebral metabolism may underlie or precede structural and functional brain changes. While bariatric surgery is known to be effective in inducing weight loss and improving obesity-related medical comorbidities, few studies have examined whether it may be able to improve brain metabolism. In the present study, we examined change in cerebral metabolite concentrations in participants with obesity who underwent bariatric surgery.
Participants and Methods:35 patients with obesity (BMI > 35 kg/m2) were recruited from a bariatric surgery candidate nutrition class. They completed single voxel 1H-proton magnetic resonance spectroscopy at baseline (pre-surgery) and within one year post-surgery. Spectra were obtained from a large medial frontal brain region. Tissue-corrected absolute concentrations for metabolites including choline-containing compounds (Cho), myo-inositol (mI), N-acetylaspartate (NAA), creatine (Cr), and glutamate and glutamine (Glx) were determined using Osprey. Paired t-tests were used to examine within-subject change in metabolite concentrations, and correlations were used to relate these changes to other health-related outcomes, including weight loss and glycemic control.
Results:Bariatric surgery was associated with a reduction in cerebral Cho (f[34j = -3.79, p < 0.001, d = -0.64) and mI (f[34] = -2.81, p < 0.01, d = -0.47) concentrations. There were no significant changes in NAA, Glx, or Cr concentrations. Reductions in Cho were associated with greater weight loss (r = 0.40, p < 0.05), and reductions in mI were associated with greater reductions in HbA1c (r = 0.44, p < 0.05).
Conclusions:Participants who underwent bariatric surgery exhibited reductions in cerebral Cho and mI concentrations, which were associated with improvements in weight loss and glycemic control. Given that elevated levels of Cho and mI have been implicated in neuroinflammation, reduction in these metabolites after bariatric surgery may reflect amelioration of obesity-related neuroinflammatory processes. As such, our results provide evidence that bariatric surgery may improve brain health and metabolism in individuals with obesity.

Many players know that the secret of winning the game of nim (and other “impartial” combinatorial games) is to write the sizes of the game’s piles in base 2 and then add them together without carry. The proof of this well-known procedure (described below) is both straightforward and convincing. Nonetheless, the procedure still appears magical, as though a rabbit has been pulled out of a hat. Astute students (and frustrated professors) often ask why the winning strategy for such games involves base 2, and not some other base. After all, nothing about the game of nim itself – the game rules, the configuration of the tokens, etc. – provides any hints about the origin of base 2 in this setting. Minimal insight is offered by most published proofs, which themselves tend to either appear almost wizardly in nature (i.e. assume the base-2 method and show that it miraculously solves the problem) or employ combinatorial arguments that supply little abstract intuition (at least to the authors of this article).

Joseph R. Biden was elected President of the United States during a period of compound crises for global health and security: the worst pandemic in a century, as well as steep reverses in progress toward reducing poverty, hunger, and disease. The United States has been in full retreat from global health leadership, fraying relationships with allies, weakening global institutions, and engaging in nationalist populism that threatens global cooperation to address worldwide challenges. Yet these tragic circumstances are also fertile soil for deep structural reforms. President Biden can both bolster the immediate responses to COVID-19 and its vast ramifications, and spearhead lasting changes to create a healthier and safer world, from which the United States would richly benefit. His immediate task will be to bring U.S. economic and scientific strength to the COVID-19 response in partnership with the World Health Organization (WHO). The Biden administration should also assume financial and strategic leadership in bolstering world efforts to achieve the Sustainable Development Goals (SDGs), including its singular pledge to leave no one behind. Finally, President Biden should empower the WHO and lead on reforms to the global health architecture to advance the right to health.

OBJECTIVES/GOALS: There is an imperative need to initiate translational genetic studies of hidradenitis suppurativa (HS). Such work requires large cohorts and no HS registries exist. Precision medicine initiatives provide new resources and methods for efficiently constructing cohorts, but empirically informed best practice guidelines are needed. METHODS/STUDY POPULATION: Traditional methods for building cohorts rely on clinical encounters to identify patients and collect phenotype data. Precision medicine initiatives aim to decrease the time and cost of data collection by using alternative sources, including electronic health records (EHR) and remote collection of patient-reported data. The public’s use of the Internet to obtain and exchange health-related information coupled with the success of direct-to-consumer genetic companies suggests that it is feasible to remotely ascertain research participants for genetic studies. Importantly, Internet cohorts provide an opportunity to include research participants who are disconnected from healthcare, and thus remain hidden from research that relies on EHR or clinical services. RESULTS/ANTICIPATED RESULTS: First, to conduct studies in EHR we are developing an analytic pipeline for the automated extraction of an accurate HS diagnosis using natural language processing of clinical notes. In our preliminary work we are also using ICD codes to build cohorts in two EHR systems with and without linked genetic data. Second, we have developed Internet advertising campaigns for symptom-based recruitment. Informed consent and patient-reported data is collected on-line through a series of short surveys. Patients who complete the surveys and express interest in participating in genetic studies are sent saliva collection kits and return mailing material. Finally, we have established an HS biobank that has DNA from 300 participants identified through clinical services. Enrollment is on going. DISCUSSION/SIGNIFICANCE OF IMPACT: Our goal is to assemble an HS cohort that is large enough to power genetic discoveries. Our work is generating empirical evidence for precision medicine guidelines and will improve our knowledge about HS. The methods we are developing can be applied to efficiently create new cohorts for genetic studies of other diseases across different clinical areas.

Summary
The Colombian government’s peace accord with the Revolutionary Armed Forces of Colombia (Fuerzas Armadas Revolucionarias de Colombia – FARC) generated hope for an end to Colombia’s more than fifty-year civil war. One of the thorniest elements of the peace process is justice for massive conflict-related human rights violations. Both during the negotiations and in the implementation of the accord, debate has raged on the effects of transitional justice on Colombia’s long-term peace and human rights.

Skipping breakfast leads to weight loss but also elevated cholesterol compared with consuming daily breakfasts of oat porridge or frosted cornflakes in overweight individuals: a randomised controlled trial

Eating breakfast may reduce appetite, body weight and CVD risk factors, but the breakfast type that produces the greatest health benefits remains unclear. We compared the effects of consuming a high-fibre breakfast, a non-fibre breakfast, or no-breakfast control on body weight, CVD risk factors and appetite. A total of thirty-six overweight participants (eighteen men and eighteen women) (mean age 33·9 (sd 7·5) years, mean BMI 32·8 (sd 4·7) kg/m2) were randomly assigned to consume oat porridge (n = 12), frosted cornflakes (n = 12) or a water control (n = 12) breakfast daily for 4 weeks. Appetite ratings were collected on the first day and weekly thereafter. Before and after the intervention, body weight, composition, blood pressure and resting energy expenditure (REE) were measured and a fasting blood sample was collected. Across the 4 weeks, fullness was higher and hunger was lower in the oat porridge group compared with the control group (P < 0·05). Mean weight change over the intervention was significantly different in the control group (−1·18 (sd 1·16) kg) compared with both the cornflakes (−0·12 (sd 1·34) kg) and oat porridge (+0·26 (sd 0·91) kg) groups (P < 0·05). However, the control group also showed elevated total cholesterol concentrations relative to the cornflakes and oat porridge groups (P < 0·05). There were no differences between groups in changes in body composition, blood pressure, REE or other CVD risk factors. In conclusion, although skipping breakfast led to weight loss, it also resulted in increased total cholesterol concentrations compared with eating either oat porridge or frosted cornflakes for breakfast.

Abstract. We describe the application of a physics-inspired renormalization technique to combinatorial games. Although this approach is not rigorous, it allows one to calculate detailed, probabilistic properties of the geometry of the P-positions in a game. The resulting geometric insights provide explanations for a number of numerical and theoretical observations about various games that have appeared in the literature. This methodology also provides a natural framework for several new avenues of research in combinatorial games, including notions of “universality,” “sensitivity-to-initial-conditions,” and “crystal-like growth,” and suggests surprising connections between combinatorial games, nonlinear dynamics, and physics. We demonstrate the utility of this approach for a variety of games–three-row Chomp, 3-D Wythoff's game, Sprague–Grundy values for 2-D Wythoff's game, and Nim and its generalizations–and show how it explains existing results, addresses longstanding questions, and generates new predictions and insights.
Introduction
In this paper we introduce a method for analyzing combinatorial games based on renormalization. As a mathematical tool, renormalization has enjoyed great success in virtually all branches of modern physics, from statistical mechanics [Goldenfeld 1992] to particle physics [Rivasseau 2003] to chaos theory [Feigenbaum 1980], where it is used to calculate properties of physical systems or objects that exhibit so-called ‘scaling’ behavior (i.e., geometric similarity on different spatial scales). In the present context we adapt this methodology to the study of combinatorial games.

Abstract
This chapter is an overview of the design and analysis of reputation systems for strategic users. We consider three specific strategic threats to reputation systems: the possibility of users with poor reputations starting afresh (whitewashing); lack of effort or honesty in providing feedback; and sybil attacks, in which users create phantom feedback from fake identities to manipulate their own reputation. In each case, we present a simple analytical model that captures the essence of the strategy, and describe approaches to solving the strategic problem in the context of this model. We conclude with a discussion of open questions in this research area.
Introduction: Why Are Reputation Systems Important?
One of the major benefits of the Internet is that it enables potentially beneficial interactions, both commercial and noncommercial, between people, organizations, or computers that do not share any other common context. The actual value of an interaction, however, depends heavily on the ability and reliability of the entities involved. For example, an online shopper may obtain better or lower-cost items from remote traders, but she may also be defrauded by a low-quality product for which redress (legal or otherwise) is difficult. If each entity's history of previous interactions is made visible to potential new interaction partners, several benefits ensue. First, a history may reveal information about an entity's ability, allowing others to make choices about whether to interact with that entity, and on what terms.
