Securely sharing DSMB reports to speed decision making from multiple, concurrent, independent studies of similar treatments in COVID-19

Introduction:
As clinical trials were rapidly initiated in response to the COVID-19 pandemic, Data and Safety Monitoring Boards (DSMBs) faced unique challenges overseeing trials of therapies never tested in a disease not yet characterized. Traditionally, individual DSMBs do not interact or have the benefit of seeing data from other accruing trials for an aggregated analysis to meaningfully interpret safety signals of similar therapeutics. In response, we developed a compliant DSMB Coordination (DSMBc) framework to allow the DSMB from one study investigating the use of SARS-CoV-2 convalescent plasma to treat COVID-19 to review data from similar ongoing studies for the purpose of safety monitoring.
Methods:The DSMBc process included engagement of DSMB chairs and board members, execution of contractual agreements, secure data acquisition, generation of harmonized reports utilizing statistical graphics, and secure report sharing with DSMB members. Detailed process maps, a secure portal for managing DSMB reports, and templates for data sharing and confidentiality agreements were developed.
Results:Four trials participated. Data from one trial were successfully harmonized with that of an ongoing trial. Harmonized reports allowing for visualization and drill down into the data were presented to the ongoing trial’s DSMB. While DSMB deliberations are confidential, the Chair confirmed successful review of the harmonized report.
Conclusion:It is feasible to coordinate DSMB reviews of multiple independent studies of a similar therapeutic in similar patient cohorts. The materials presented mitigate challenges to DSMBc and will help expand these initiatives so DSMBs may make more informed decisions with all available information.

Objectives
To assess the general health and activity levels of 4- and 5-year-old children after intervention for congenital cardiac disease.
MethodsHealth behaviour outcomes were assessed in 91 children who had surgery or catheter intervention for congenital cardiac disease. The children were classified into four groups according to severity. The main parameters of classification were the presence of residual symptoms, frequency of visits to general practitioner or the Accident and Emergency Department, and ability to participate in physical activity according to a calculated “activity score”.
ResultsChildren had very few residual symptoms after “corrective surgery”. Those with complex congenital cardiac disease post-Fontan-type repair still had symptoms on average 18.2 days per month. Surprisingly, the complex group had fewer days “sick” from non-cardiac causes and had fewer visits to general practitioner or Accident and Emergency Departments. Regression analysis indicates that three variables had significant relevance to the general practitioner or Accident and Emergency visits: complex congenital cardiac disease, fewer visits; Townsend score – more deprivation – more visits; and maternal worry – higher maternal worry score – more visits. Regression analysis indicates that lower activity score is significantly related to complex cardiac disease and higher maternal worry score.
ConclusionsThe majority of this group of 4- and 5-year-old children had few residual symptoms and had good exercise tolerance. Maternal worry is a significant factor in influencing both activity levels and frequency of unscheduled health service demands – general practitioner or Accident and Emergency visits.

A circular letter was sent out to all members of the Commission in December 1937, to which the majority have replied. While work is going on steadily in the Observatories where meridian observations are carried out, comparatively few catalogues have been published since 1935. In view of the very full report made three years ago it is only necessary to draw attention to the progress which has been made in the interval.

The Ferguson case in 1917 represents the only instance of the impeachment and conviction of a state official in Texas. The law of impeachment must then be sought in the proceedings of the Ferguson trial, the opinions of the attorney-general, and the opinion of the supreme court in 1924 in the case of Ferguson v. Maddox, which reviewed the legality of the 1917 proceedings in determining whether Mr. Ferguson was eligible to have his name placed on the primary election ballot as a candidate for governor.
Impeachment at a Special Session. The constitution provides that the governor may on extraordinary occasions convene the legislature in special sessions limited to thirty days' duration, during which there shall be no legislation upon subjects other than those designated in the proclamation of the governor calling the session or presented by him. The facts in the Ferguson case may be reviewed briefly. The speaker of the House of Representatives, on his own motion, had issued a call for the House to meet in special session on August 1, 1917, to consider the impeachment of the governor. Before the members could assemble, the governor issued a call for a special session of the legislature to meet at the same time as that set by the speaker's call for the purpose of considering the matter of university appropriations. The House proceeded with the investigation and impeached the governor, who was thereupon suspended from office.

Students of centralized purchasing give Texas the credit for being the pioneer state in the movement. With the creation of the office of purchasing agent for the eleemosynary institutions in 1899, centralized purchasing in state government is said to have had its beginning. Purchasing organization in Texas remained unchanged until 1919, when, following an investigation into all departments by a legislative committee, a number of departments, including that of the state purchasing agent, were consolidated into a state board of control.
This board is composed of three members appointed by the governor with the consent of the Senate, for a term of six years, one member being appointed every two years. Each member receives $5,000 a year. A division of purchasing is one of six divisions under the board, others being (1) public printing, (2) auditing, (3) design, construction, and maintenance, (4) estimates and appropriations, and (5) eleemosynary institutions. The board is authorized to appoint a chief of the purchasing division, whose qualifications are prescribed by law. But since its organization in 1920, no chief of the purchasing division has been appointed, a member of the board taking direct charge of this division. In addition to the board member, four other people are engaged in the work of this division, including an assistant purchasing agent. No increase in personnel has occurred since the organization of the board. Responsibility is vested in the board for purchasing all supplies used by the departments and institutions of the state, including furniture and fixtures, and all other things except strictly perishable goods, technical instruments, and books.
