Background
Item 9 of the Patient Health Questionnaire-9 (PHQ-9) queries about thoughts of death and self-harm, but not suicidality. Although it is sometimes used to assess suicide risk, most positive responses are not associated with suicidality. The PHQ-8, which omits Item 9, is thus increasingly used in research. We assessed equivalency of total score correlations and the diagnostic accuracy to detect major depression of the PHQ-8 and PHQ-9.
MethodsWe conducted an individual patient data meta-analysis. We fit bivariate random-effects models to assess diagnostic accuracy.
Results16 742 participants (2097 major depression cases) from 54 studies were included. The correlation between PHQ-8 and PHQ-9 scores was 0.996 (95% confidence interval 0.996 to 0.996). The standard cutoff score of 10 for the PHQ-9 maximized sensitivity + specificity for the PHQ-8 among studies that used a semi-structured diagnostic interview reference standard (N = 27). At cutoff 10, the PHQ-8 was less sensitive by 0.02 (−0.06 to 0.00) and more specific by 0.01 (0.00 to 0.01) among those studies (N = 27), with similar results for studies that used other types of interviews (N = 27). For all 54 primary studies combined, across all cutoffs, the PHQ-8 was less sensitive than the PHQ-9 by 0.00 to 0.05 (0.03 at cutoff 10), and specificity was within 0.01 for all cutoffs (0.00 to 0.01).
ConclusionsPHQ-8 and PHQ-9 total scores were similar. Sensitivity may be minimally reduced with the PHQ-8, but specificity is similar.

Background
Different diagnostic interviews are used as reference standards for major depression classification in research. Semi-structured interviews involve clinical judgement, whereas fully structured interviews are completely scripted. The Mini International Neuropsychiatric Interview (MINI), a brief fully structured interview, is also sometimes used. It is not known whether interview method is associated with probability of major depression classification.
AimsTo evaluate the association between interview method and odds of major depression classification, controlling for depressive symptom scores and participant characteristics.
MethodData collected for an individual participant data meta-analysis of Patient Health Questionnaire-9 (PHQ-9) diagnostic accuracy were analysed and binomial generalised linear mixed models were fit.
ResultsA total of 17 158 participants (2287 with major depression) from 57 primary studies were analysed. Among fully structured interviews, odds of major depression were higher for the MINI compared with the Composite International Diagnostic Interview (CIDI) (odds ratio (OR) = 2.10; 95% CI = 1.15–3.87). Compared with semi-structured interviews, fully structured interviews (MINI excluded) were non-significantly more likely to classify participants with low-level depressive symptoms (PHQ-9 scores ≤6) as having major depression (OR = 3.13; 95% CI = 0.98–10.00), similarly likely for moderate-level symptoms (PHQ-9 scores 7–15) (OR = 0.96; 95% CI = 0.56–1.66) and significantly less likely for high-level symptoms (PHQ-9 scores ≥16) (OR = 0.50; 95% CI = 0.26–0.97).
ConclusionsThe MINI may identify more people as depressed than the CIDI, and semi-structured and fully structured interviews may not be interchangeable methods, but these results should be replicated.

The association of gas turbines with aircraft designed to make effective use of the special characteristics of jet propulsion has brought the trend of development into a state of transition and uneasy experiment. Already speeds equivalent to a low level Mach number of 0.80 have been recorded under observed conditions.

Our knowledge of the universe comes from recording the photon and particle fluxes incident on the Earth from space. We thus require sensitive measurement across the entire energy spectrum, using large telescopes with efficient instrumentation located on superb sites. Technological advances and engineering constraints are nearing the point where we are recording as many photons arriving at a site as is possible. Major advances in the future will come from improving the quality of the site. The ultimate site is, of course, beyond the Earth’s atmosphere, such as on the Moon, but economic limitations prevent our exploiting this avenue to the degree that the scientific community desires. Here we describe an alternative, which offers many of the advantages of space for a fraction of the cost: the Antarctic Plateau.

Ecphora is one of the most easily recognized gastropod genera in late Oligocene, Miocene, and Pliocene marine formations in the southeastern United States. Its generally large size, strong spiral ribs, and brown, calcitic exterior are much prized by fossil collectors. However, despite speculations by Petuch (1988), its evolutionary origins have remained obscure. The present shell microstructural and mineralogical evidence suggests that the earliest ecphoras were entirely aragonitic and that the thick calcitic outer layer of the later Miocene and Pliocene species originated as a thin calcitic crust on the crests of the ribs of a latest Oligocene or earliest Miocene species.

Salmonellosis is an internationally important disease of mammals and birds. Unique epidemics in New Zealand in the recent past include two Salmonella serovars: Salmonella enterica subsp. enterica serovar Typhimurium definitive type (DT) 160 (S. Typhimurium DT160) and S. Brandenburg. Although not a major threat internationally, in New Zealand S. Typhimurium DT160 has been the most common serovar isolated from humans, and continues to cause significant losses in wildlife. We have identified DNA differences between the first New Zealand isolate of S. Typhimurium DT160 and the genome-sequenced strain, S. Typhimurium LT2. All the differences could be accounted for in one cryptic phage ST64B, and one novel P22-like phage, ST160. The majority of the ST160 genome is almost identical to phage SE1 but has two regions not found in SE1 which are identical to the P22-like phage ST64T, suggesting that ST160 evolved from SE1 via two recombination events with ST64T. All of the New Zealand isolates of DT160 were identical indicating the clonal spread of this particular Salmonella. Some overseas isolates of S. Typhimurium DT160 differed from the New Zealand strain and contained SE1 phage rather than ST160. ST160 was also identified in New Zealand isolates of S. Typhimurium DT74 and S. Typhimurium RDNC-April06 and in S. Typhimurium DT160 isolates from the USA. The emergence of S. Typhimurium DT160 as a significant pathogen in New Zealand is postulated to have occurred due to the sensitivity of the Salmonella strains to the ST160 phage when S. Typhimurium DT160 first arrived.

Experiments have been described in which mice were infected by the inhalation of a mono-dispersed aerosol of Salm. typhimurium. The disease so produced was characterized by a specific primary pneumonia. The lethal dose was very much smaller than that required by ingestion, and was approximately equal to the intraperitoneal lethal dose. Possible implications have been discussed.

Structures formed during wind deflation of poorly sorted beach sediments have rarely been described in the literature. However, a number of distinct forms exist including capped pedestals, asymmetrical ‘tear’ structures and scoured or impacted crusts. All these phenomena result from differential spatial erosion of the beach surface by wind, caused by textural or compositional variations. Ephemeral structures are indicative of rapidly changing geomorphological conditions and their presence may also exert control over continuing aeolian processes.

Conformation of carcasses from 107 Romney ewe lambs was determined using two visual scoring systems and the scores related to chemical and dissected carcass composition. The conformation system which evaluated the lateral profile of the hind leg failed to improve the prediction of carcass composition. The Meat and Livestock Commission system used in Great Britain separated carcasses on weight and fatness, the higher classes having the heaviest and fattest carcasses. After adjustment for carcass weight and fatness the higher conformation classes had the lightest bones and the heaviest weight of muscle, and some of the muscles weighed were shorter and deeper than those of the lower conformation classes.

In experiments at Rothamsted in 1984–86, seven factors, each at two levels, were tested in factorial combination on winter barley (cv. Panda) grown as a third take-all susceptible crop. The factors were seed rate, a growth regulator prior to stem extension, amounts of N in spring, ‘winter’ nitrogen, an autumn insecticide, a fungicide applied to the seed (‘Baytan’) and a programme of fungicide sprays in spring and summer. Sowing 50% more seeds than normal increased the number of ears/unit area but had no effect on mean grain yield because grains were smaller. There were, however, significant, but unexplained, interactions between seed rate and the fungicide ‘Baytan’ applied to the seed. A growth regulator applied prior to stem extension had little effect on crop growth and no significant effect on grain yield. If sufficient N was applied in April there was little benefit from applying ‘winter’ N (30 kg/ha in November and again in February/March) except in 1985 when the amount of NO3-N in the soil, measured in the previous October, was lowest. Insecticide sprays applied in autumn to control the aphid vectors of barley yellow dwarf virus (BYDV) had no significant effect on grain yield but infectivity indices were below the threshold needed for treatment in each year. On average, ‘Baytan’ applied to the seed increased grain yield by 0·28 t/ha and this was associated with decreases in the severity of take-all. Over the three years, programmes of fungicide sprays, applied during spring and summer, increased grain yield by 0·92 t/ha but the mean response was largest where most N was applied.
The experiments also allowed the importance of interactions between different agronomic factors to be examined. A combined analysis of grain yields for all three years (based on 192 plot values) showed that only six 2- or 3-factor interactions, out of the 73 estimated, were significant (P < 0·05). Two of these interactions reflected variable responses to ‘winter’ N and fungicide sprays in the three years and three of the remaining four involved ‘Baytan’.
Additional plots of barley grown after oats had little take-all and yielded 1·14 t/ha more grain than similarly treated plots grown after barley. These responses were obtained despite evidence that oat residues had adverse effects on the growth of barley seedlings. Additional plots of barley grown after a bare fallow also had little take-all and gave even larger total yields (grain plus straw) than did barley after oats but the mean yield of grain was less than after oats because more of the dry matter after a fallow was straw. In 1984, when take-all was relatively slight, plots after a fallow gave even less grain than plots after barley (−0·77 t/ha) despite producing 2·12 t/ha more dry matter in grain plus straw.
Spectrum of nasal disease in an asthma clinic: when is an ENT opinion indicated?

Aims:
To characterise the spectrum of nasal symptomatology and nasendoscopic abnormalities seen in patients attending an asthma clinic, and to relate these symptoms to the likelihood of finding nasendoscopic abnormalities which merit treatment.
Methods:Forty-three patients attending a problem asthma clinic were enrolled in an observational study. Cardinal nasal symptoms – obstruction, congestion, hyposmia, rhinorrhoea, sneezing, epistaxis or other symptoms – were graded as none (zero), mild (one), moderate (two) or severe (three), giving a maximum nasal symptom score of 21. Asthma symptoms and lung function were measured. Nasendoscopy was then performed.
Results:Obstruction was the most common cardinal nasal symptom (seen in 15 patients), the median nasal symptom score was 5.3 (range zero to 14) and only three patients had no nasal symptoms. There was no correlation between nasal symptom score and severity of asthma symptoms or forced expiratory volume in one second. Twenty-two patients had a normal appearance on ENT examination (median nasal symptom score four). The nasendoscopic abnormalities seen comprised polyps (n = 8; median nasal symptom score five), deviated nasal septum (n = 7; median nasal symptom score four), oedematous mucosa (n = 4; median nasal symptom score seven) and other abnormalities (n = 2). Individual nasal symptoms were poor predictors of individual nasal pathologies, with hyposmia the best individual predictor of any abnormality (positive predictive value 80 per cent). The presence of a combination of symptoms increased the likelihood of any nasendoscopic abnormality, with obstruction, rhinorrhoea and hyposmia together having a positive predictive value of 100 per cent.
Conclusions:Nasal symptoms are much more frequent than structural abnormalities in patients attending a problem asthma clinic. The threshold for ENT referral should be lower when the patient complains of a symptom complex including hyposmia. Furthermore, concurrent hyposmia, obstruction and rhinorrhoea should be seen as an indication for ENT referral.

The current status of SiC bulk growth is reviewed, while specific attention is given to the effect of defects in SiC substrates and epitaxial layers on device performance and yield. The progress in SiC wafer quality is reflected in the achievement of micropipe densities as low as 0.92 cm−2 for a 3-inch n-type 4H-SiC wafer, which provides the basis for a high yielding fabrication process of large area SiC power devices. Using a Murphy Probe Yield Analysis for the breakdown characteristics of 10 kV PiN diodes we have extracted an “effective” defect density for 4H-SiC material to be as low as 30 cm−2, providing valuable information to further isolate and address the specific material defects critical for device performance. We address the problematic degradation of the forward characteristics (Vf-drift) of bipolar SiC PiN diodes [CITE]. The underlying mechanism due to stacking fault formation in the epitaxial layers and possible effects of device processing are investigated. An improved device design is demonstrated, which effectively stabilizes this Vf-drift. We show the progression in the development of semi-insulating SiC grown by the sublimation technique from extrinsically doped material to high purity semi-insulating (HPSI) 4H-SiC bulk crystals of up to 100 mm diameter without resorting to the intentional introduction of elemental deep level dopants, such as vanadium. Uniform resistivities in 3-inch HPSI wafers greater than 3 × 1011 Ω-cm have been achieved. Secondary ion mass spectrometry, deep level transient spectroscopy and electron paramagnetic resonance data suggest that the semi-insulating behavior in HPSI material originates from deep levels associated with intrinsic point defects. MESFETs produced on HPSI wafers are free of backgating effects and have resulted in the best combination of power density and efficiency reported to date for SiC MESFETs of 5.2 W/mm and 63% power added efficiency (PAE) at 3.5 GHz.
Scalability of MOCVD-deposited Hafnium Oxide

Because of aggressive downscaling to increase transistor performance, the physical thickness of the SiO2 gate dielectric is rapidly approaching the limit where it will only consist of a few atomic layers. As a consequence, this will result in very high leakage currents due to direct tunneling. To allow further scaling, materials with a k-value higher than SiO2 (“high-k materials”) are explored, such that the thickness of the dielectric can be increased without degrading performance.
Based on our experimental results, we discuss the potential of MOCVD-deposited HfO2 to scale to (sub)-1-nm EOTs (Equivalent Oxide Thickness). A primary concern is the interfacial layer that is formed between the Si and the HfO2, during the MOCVD deposition process, for both H-passivated and SiO2-like starting surfaces. This interfacial layer will, because of its lower k-value, significantly contribute to the EOT and reduce the benefit of the high-k material. In addition, we have experienced serious issues integrating HfO2 with a polySi gate electrode at the top interface depending on the process conditions of polySi deposition and activation anneal used. Furthermore, we have determined, based on a thickness series, the k-value for HfO2 deposited at various temperatures and found that the k-value of the HfO2 depends upon the gate electrode deposited on top (polySi or TiN).
Based on our observations, the combination of MOCVD HfO2 with a polySi gate electrode will not be able to scale below the 1-nm EOT marker. The use of a metal gate however, does show promise to scale down to very low EOT values.
Characterization of Thick 4H-SiC Hot-Wall CVD Layers

Epitaxial 4H-SiC layers suitable for high power devices have been grown in a hot-wall chemical-vapor deposition (CVD) system. These layers were subsequently characterized for many parameters important in device development and production. The uniformity of both thickness and doping will be presented.
Doping trends vs. temperature and growth rate will be shown for the p-type dopant used. The n-type dopant drops in concentration with increasing temperature or increasing growth rate. In contrast, the p-type dopant increases in concentration with decreasing temperature or increasing growth rate. A simple descriptive model for this behavior will be presented.
The outcome from capacitance-voltage and SIMS measurements demonstrate that transitions from n to n−, or p to p−, and even n to p levels can be made quickly without adjustment to growth conditions. The ability to produce sharp transitions without process changes avoids degrading the resulting surface morphology or repeatability of the process. Avoiding process changes is particularly important in growth of thick layers since surface roughness tends to increase with layer thickness.
Device results from diodes producing two different blocking voltages in excess of 5 kV will also be shown. The higher voltage diodes exhibited a breakdown behavior which was near the theoretical limit for the epitaxial layer thickness and doping level grown.
