Associations between insomnia and reward learning in clinical depression

Background
Depression and insomnia commonly co-occur. Yet, little is known about the mechanisms through which insomnia influences depression. Recent research and theory highlight reward system dysfunction as a potential mediator of the relationship between insomnia and depression. This study is the first to examine the impact of insomnia on reward learning, a key component of reward system functioning, in clinical depression.
MethodsThe sample consisted of 72 veterans with unipolar depression who endorsed sleep disturbance symptoms. Participants completed the Structured Clinical Interview for DSM-IV, self-report measures of insomnia, depression, and reward processing, and a previously validated signal detection task (Pizzagalli et al., 2005, Biological Psychiatry, 57(4), 319–327). Trial-by-trial response bias (RB) estimates calculated for each of the 200 task trials were examined using linear mixed-model analyses to investigate change in reward learning.
ResultsFindings demonstrated diminished rate and magnitude of reward learning in the Insomnia group relative to the Hypersomnia/Mixed Symptom group across the task. Within the Insomnia group, participants with more severe insomnia evidenced the lowest rates of reward learning, with increased RB across the task with decreasing insomnia severity.
ConclusionsAmong individuals with depression, insomnia is associated with decreased ability to learn associations between neutral stimuli and rewarding outcomes and/or modify behavior in response to differential receipt of reward. This attenuated reward learning may contribute to clinically meaningful decreases in motivation and increased withdrawal in this comorbid group. Results extend existing theory by highlighting impairments in reward learning specifically as a potential mediator of the association between insomnia and depression.

Background
The analysis of latent inhibition to be developed in this chapter had its origins in the attempt to study a (seemingly) quite different phenomenon – the acquired distinctiveness of cues. James (1890) had suggested that similar cues could be rendered more discriminable by training in which each became linked with a very different associate. The idea was taken up by Lawrence (1949) in his classic experiments with rats, and the 1950s and 1960s saw a rush of experiments (reviewed by Hall,1991) exploring the phenomenon with human subjects. A theoretical account of the effect (based on that of Sutherland & Mackintosh, 1971) was put forward by Mackintosh (1975). He proposed that the representation of any cue has a given level of associability (represented, usually, by the symbol α) but that the value of the parameter α can be changed by experience. When a cue predicts a consequence reliably (or, at least does so more reliably than other cues that might be present) its α value increases; when it predicts a consequence no better than other cues its α value declines. Initial training in which two cues are reliably followed by different consequences will enhance the α value of each of them and subsequent discrimination tasks involving these cues will be facilitated – the acquired distinctiveness effect.
Empirical evidence of the acquired distinctiveness effect came largely from studies of discrimination learning, but Mackintosh's (1975) account implies that the effect should be found in simple conditioning too.

A hundred-and-twenty-five school phobic youngsters had been treated in a psychiatric in-patient unit for young adolescents of secondary school age over a seven-year period; a hundred of them were reviewed on average three years after discharge. About a third of cases were found to have improved little; they had persistent severe symptoms of emotional disturbance and continuing social impairment. Another third had improved appreciably and were affected by neurotic symptoms rather than social impairment. The remaining third had improved substantially or completely. School attendance difficulties had remained in about half of all cases. Subsequent difficulties in going to work were less pronounced. The best predictor of outcome was clinical state on discharge. High intelligence also emerged as a significant predictor of poor outcome. Five girls and a boy had already developed severe and persistent agoraphobic difficulties when reviewed. It was found that severe school phobia in early adolescence resembled adult affective disorders in some clinical features and in outcome.
