Pathways to eating disorder care: A European multicenter study

Background
The aim of this study was to assess barriers and facilitators in the pathways toward specialist care for eating disorders (EDs).
MethodsEleven ED services located in seven European countries recruited patients with an ED. Clinicians administered an adapted version of the World Health Organization “Encounter Form,” a standardized tool to assess the pathways to care. The unadjusted overall time needed to access the ED unit was described using the Kaplan–Meier curve.
ResultsFour-hundred-nine patients were recruited. The median time between the onset of the current ED episode and the access to a specialized ED care was 2 years. Most of the participants did not directly access the specialist ED unit: primary “points of access” to care were mental health professionals and general practitioners. The involvement of different health professionals in the pathway, seeking help for general psychiatric symptoms, and lack of support from family members were associated with delayed access to ED units.
ConclusionsEducational programs aiming to promote early diagnosis and treatment for EDs should pay particular attention to general practitioners, in addition to mental health professionals, and family members to increase awareness of these illnesses and of their treatment initiation process.

Background
Cognitive deficits are a core feature of early stages in schizophrenia. However, the extent to which antipsychotic (AP) have a deleterious effect on cognitive performance remains under debate. We aim to investigate whether anticholinergic loadings and dose of AP drugs in first episode of psychosis (FEP) in advanced phase of remission are associated with cognitive impairment and the differences between premorbid intellectual quotient (IQ) subgroups.
MethodsTwo hundred and sixty-six patients participated. The primary outcomes were cognitive dimensions, dopaminergic/anticholinergic load of AP [in chlorpromazine equivalents (Eq-CPZ) and the Anticholinergic Risk Scale (ARS), respectively].
ResultsImpairments in processing speed, verbal memory and global cognition were significantly associated with high Eq-CPZ and verbal impairment with high ARS score. Moreover, this effect was higher in the low IQ subgroup.
ConclusionsClinicians should be aware of the potential cognitive impairment associated with AP in advanced remission FEP, particularly in lower premorbid IQ patients.
The effects of spatial survey bias and habitat suitability on predicting the distribution of threatened species living in remote areas

Knowledge of a species’ potential distribution and the suitability of available habitat are fundamental for effective conservation planning and management. However, the quality of information on the distribution of species and their required habitats is highly variable in terms of accuracy and availability across taxa and regions, particularly in tropical landscapes where accessibility is especially challenging. Species distribution models (SDMs) provide predictive tools for addressing gaps for poorly surveyed species, but they rarely consider biases in geographical distribution of records and their consequences. We applied SDMs and variation partitioning analyses to investigate the relative importance of habitat characteristics, human accessibility, and their joint effects in the global distribution of the Critically Endangered Blue-throated Macaw Ara glaucogularis, a species endemic to the Amazonian flooded savannas of Bolivia. The probability of occurrence was skewed towards more accessible areas, mostly secondary roads. Variability in observed occurrence patterns was mostly accounted for by the pure effect of habitat characteristics (76.2%), indicating that bias in the geographical distribution of occurrences does not invalidate species-habitat relationships derived from niche models. However, observed spatial covariation between land-use at a landscape scale and accessibility (joint contribution: 22.3%) may confound the independent role of land-use in the species distribution. New surveys should prioritise collecting data in more remote (less accessible) areas better distributed with respect to land-use composition at a landscape scale. Our results encourage wider application of partitioning methods to quantify the extent of sampling bias in datasets used in habitat modelling for a better understanding of species-habitat relationships, and add insights into the potential distribution of our study species and opportunities for its conservation.
Competitive inhibition of three novel bacteria isolated from faeces of breast milk-fed infants against selected enteropathogens

Numerous in vitro and in vivo studies conducted using different probiotic micro-organisms have demonstrated their ability to interfere with the growth and virulence of a variety of enteropathogens. The reported beneficial effects of the use of probiotics to complement antibiotic therapy or prevent diarrhoea or gastrointestinal infection in infants have increased in recent years. In the present study, we demonstrated the capacity of supernatants obtained from three novel probiotics (Lactobacillus paracasei CNCM I-4034, Bifidobacterium breve CNCM I-4035 and Lactobacillus rhamnosus CNCM I-4036) isolated from the faeces of breastfed infants to inhibit the growth of enterotoxigenic and enteropathogenic (EPEC) bacteria, such as Escherichia coli, Salmonella and Shigella. To assess their potential antimicrobial activity, the 17 and 24 h cell-free supernatants broth concentrates (10 × ) having 1, 2 or 4 % of the three probiotics were incubated with EPEC bacteria strains. After 17 h of co-culture, the supernatants were able to inhibit the growth of E. coli, Salmonella and Shigella up to 40, 55 and 81 %, respectively. However, the inhibitory capacity of some supernatants was maintained or completely lost when the supernatants (pH 3·0) were neutralised (pH 6·5). Overall, these results demonstrated that L. paracasei CNCM I-4034, B. breve CNCM I-4035 and L. rhamnosus CNCM I-4036 produce compounds that exhibited strain-specific inhibition of enterobacteria and have the potential to be used as probiotics in functional foods.

Democratization, decentralization, and development. These three sequential forces have swept the world over the last decade and have redrawn the maps of politics, power, and prosperity.
Giugale and Webb (2000)INTRODUCTION
The implicit assumption in this recent World Bank book on Mexico is that democratization leads to decentralization and that the latter leads to development. In all fairness, the publication and the Bank have been careful to point out that not all decentralization is good for development. This chapter argues that, in practice, not all democracies have equally auspicious forces driving the decentralization process.
Decentralization can, but does not necessarily, improve accountability, equity, and government performance. From the citizen's point of view, the specific benefits that decentralization can bring about depend on the way they are represented politically as well as the kinds of institutions that form and implement government policy. Moreover, the costs and benefits of decentralization depend largely on the entry points and intermediate goals for the transition from the centralized state to a decentralized public sector. There is no standard path toward decentralization; and countries also rarely plan their path toward a final version of the decentralized state – the process is more often the result of internally inconsistent accumulated reforms adopted over time in response to changes in the political balance.
This chapter is centered on two interconnected developments that account for the main challenges for Mexico's decentralization today.

The role of the switch II region in the conformational transition of activation of Ha-ras-p21 has been investigated by mutating residues predicted to act as hinges for the conformational transition of this loop (Ala59, Gly60, and Gly75) (Diaz JF, Wroblowski B, Schlitter J, Engelborghs Y, 1997, Proteins 28:434–451), as well as mutating the catalytic residue Gln61. The proposed mutations of the hinge residues decrease the rate of the conformational transition of activation as measured by the binding of BeF3− to the GDP-p21 complex. Also, the thermodynamic parameters of the binding reaction are altered by a factor between three and five, depending on the temperature. (Due to changes in activation and reaction enthalpies, partially compensated by entropy changes.) The control mutation Q61H in which only the catalytic residue is changed has only a limited effect on the kinetic rate constants of the conformational transition and on the thermodynamic parameters of the reaction.
The fact that mutations of the hinge residues of the switch II region affect both the binding of the phosphate analog and the conformational transition of activation indicates that the switch II is implicated both in the early and the late states of the transition.
A step toward the prediction of the fluorescence lifetimes of tryptophan residues in proteins based on structural and spectral data

A method is presented that allows the calculation of the lifetimes of tryptophan residues on the basis of spectral and structural data. It is applied to two different proteins. The calcium binding protein from the sarcoplasm of the muscles of the sand worm Nereis diversicolor (NSCP) changes its conformation upon binding of Ca2+ or Mg2+. NSCP contains three tryptophan residues at position 4, 57, and 170, respectively. The fluorescence lifetimes of W57 are investigated in a mutant in which W4 and W170 have been replaced. The time resolved fluorescence properties of W57 are linked to its different microconformations, which were determined by a molecular dynamics simulation map. Together with the determination of the radiative rate constant from the wavelength of maximum intensity of the decay associated spectra, it was possible to determine an exponential relation between the nonradiative rate constant and the distance between the indole CE3 atom and the carbonyl carbon of the peptide bond reflecting a mechanism of electron transfer as the main determinant of the value for the nonradiative rate constant. This result allows the calculation of the fluorescence lifetimes from the protein structure and the spectra. This method was further tested for the tryptophan of Ha-ras p21 (W32) and for W43 of the Tet repressor, which resulted in acceptable values for the predicted lifetimes.

This work experimentally confirms the pathway of activation of Ha-ras-p21, which was calculated by the method of Targeted Molecular Dynamics (TMD) (Díaz JF, Wroblowski B, Schlitter J, Engelborghs Y, 1997a, Proteins Struct Funct Genet 28:434–451). The process can be studied experimentally by analyzing the binding of BeF3− to the GDP complex of the active fluorescent mutant Y32W (Díaz JF, Sillen A, Engelborghs Y, 1997b, J Biol Chem 227:23138–23143). Two mutants, V29G and I36G, have been constructed at both sides of the effector loop of the active fluorescent mutant. This was done to check the proposed reaction pathway and to provide further insight into the mechanism of the activation of ras proteins. Both mutations accelerate the conformational isomerization with two orders of magnitude, demonstrating convincingly the role of these residues as hinges of the effector loop in one or more of the transitions of the conformational change. These results provide experimental support to the pathway calculated by TMD analysis.
