Real-world clinical and cost-effectiveness of community clozapine initiation: mirror cohort study

Background
Clozapine is the only drug licensed for treatment-resistant schizophrenia (TRS) but the real-world clinical and cost-effectiveness of community initiation of clozapine is unclear.
AimsThe aim was to assess the feasibility and cost-effectiveness of community initiation of clozapine.
MethodThis was a naturalistic study of community patients recommended for clozapine treatment.
ResultsOf 158 patients recommended for clozapine treatment, 88 (56%) patients agreed to clozapine initiation and, of these, 58 (66%) were successfully established on clozapine. The success rate for community initiation was 65.4%; which was not significantly different from that for in-patient initiation (58.82%, χ2(1,88) = 0.47, P = 0.49). Following clozapine initiation, there was a significant reduction in median out-patient visits over 1 year (from 24.00 (interquartile range (IQR) = 14.00–41.00) to 13.00 visits (IQR = 5.00–24.00), P < 0.001), and 2 years (from 47.50 visits (IQR = 24.75–71.00) to 22.00 (IQR = 11.00–42.00), P < 0.001), and a 74.71% decrease in psychiatric hospital bed days (z = −2.50, P = 0.01). Service-use costs decreased (1 year: –£963/patient (P < 0.001); 2 years: –£1598.10/patient (P < 0.001). Subanalyses for community-only initiation also showed significant cost reductions (1 year: –£827.40/patient (P < 0.001); 2 year: –£1668.50/patient (P < 0.001) relative to costs prior to starting clozapine. Relative to before initiation, symptom severity was improved in patients taking clozapine at discharge (median Positive and Negative Syndrome Scale total score: initial visit: 80 (IQR = 71.00–104.00); discharge visit 50.5 (IQR = 44.75–75.00), P < 0.001) and at 2 year follow-up (Health of Nation Outcome Scales total score median initial visit: 13.00 (IQR = 9.00–15.00); 2 year follow-up: 8.00 (IQR = 3.00–13.00), P = 0.023).
ConclusionsThese findings indicate that community initiation of clozapine is feasible and is associated with significant reductions in costs, service use and symptom severity.

Background
Responses to mental disorders usually focus on treatment; socio-economic conditions are less likely to be considered.
AimsTo examine social determinants of mental disorders and attempted suicide in Australia.
MethodData from the 1997 Australian National Survey of Mental Health and Wellbeing (n=10 641) were used to estimate associations between socio-economic status, mental disorders and attempted suicide. Logistic regression was used to adjust for age, urban/rural residence and country of birth. Socio-economic status differentials in suicide attempts were also adjusted for mental disorders.
ResultsSignificant increasing gradients from high to low levels of education and occupational status (employed) were evident for affective disorders and anxiety disorders in both men and women and for substance use disorders in men. Similar gradients were found for suicide attempts, which decreased after adjusting for mental disorders, but remained significant in the working-age employed.
ConclusionsThese findings suggest social causation of mental disorders and suicide attempts, and the need for social and economic responses beyond provision of mental health services.

“Speenhamland” is a word popularized by late nineteenth-century historians as a derogatory term for the systematic subsidization of laborers' wages by allowances paid from the poor rates. This system was thought to have flourished in southern and agrarian England in the early nineteenth century, the size of the allowances determined by the size of the family and the price of bread. The unwitting “villains” were the Berkshire justices who met at the Pelican Inn, located in a tithing of Speen Parish. Moved by corn dearth and a terrible winter, the justices on May 6, 1795, set in train the fatal hemorrhaging of the Old Poor Law that, in turn, led to the draconian Poor Law Amendment Act of 1834.
Myth this may largely be, and it has been explored elsewhere; however, no one questions that subsidizing the employed from the poor rates, including allowances in aid of wages, occurred in the late eighteenth and early nineteenth centuries. It is in this sense that “Speenhamland” is used here, but to suggest a radically different and mainly constructive consequence for British economic and social development.
For subsidizing the employed poor, when it took the form of nonresident relief, could function as a kind of “Industrial Speenhamland” (freshly coined), to wit: a system of parochially funded labor migration that promoted a work force for expanding industries. This subsidization could include allowances in aid of wages as well as other welfare benefits in times of sickness and unemployment—all at the expense of the home parish or township, not of the places in which the factories and industrial workshops were located.

The Old Poor Law has recently found a forceful defender in Mark Blaug, who in two articles has challenged the critics of England's first public welfare system and displayed the clay feet of that towering structure of words, the evidence for the 1834 Report. This is a notable achievement, but Mr. Blaug's case should not be accepted in toto: indeed, that it has survived so long without criticism is surprising. G. R. Elton felicitously referred to one of his critics as a “self-appointed hound of heaven,” but scholars who propound provocative theses are likely to provoke responses. Mr. Blaug's interpretation of the Old Poor Law is, to at least one “selfappointed hound,” eminently provocative.
