Future outlook of people living alone with early-stage dementia and their non-resident relatives and friends who support them

Little is known about the experiences of people living alone with dementia in the community and their non-resident relatives and friends who support them. In this paper, we explore their respective attitudes and approaches to the future, particularly regarding the future care and living arrangements of those living with dementia. The study is based on a qualitative secondary analysis of interviews with 24 people living alone with early-stage dementia in North Wales, United Kingdom, and one of their relatives or friends who supported them. All but four of the dyads were interviewed twice over 12 months (a total of 88 interviews). In the analysis, it was observed that several people with dementia expressed the desire to continue living at home for ‘as long as possible’. A framework approach was used to investigate this theme in more depth, drawing on concepts from the existing studies of people living with dementia and across disciplines. Similarities and differences in the future outlook and temporal orientation of the participants were identified. The results support previous research suggesting that the future outlook of people living with early-stage dementia can be interpreted in part as a response to their situation and a way of coping with the threats that it is perceived to present, and not just an impaired view of time. Priorities for future research are highlighted in the discussion.

Objective.
To identify the behavioral determinants—both barriers and enablers—that may impact physician hand hygiene compliance.
Design.A qualitative study involving semistructured key informant interviews with staff physicians and residents.
Setting.An urban, 1,100-bed multisite tertiary care Canadian hospital.
Participants.A total of 42 staff physicians and residents in internal medicine and surgery.
Methods.Semistructured interviews were conducted using an interview guide that was based on the theoretical domains framework (TDF), a behavior change framework comprised of 14 theoretical domains that explain health-related behavior change. Interview transcripts were analyzed using thematic content analysis involving a systematic 3-step approach: coding, generation of specific beliefs, and identification of relevant TDF domains.
Results.Similar determinants were reported by staff physicians and residents and between medicine and surgery. A total of 53 specific beliefs from 9 theoretical domains were identified as relevant to physician hand hygiene compliance. The 9 relevant domains were knowledge; skills; beliefs about capabilities; beliefs about consequences; goals; memory, attention, and decision processes; environmental context and resources; social professional role and identity; and social influences.
Conclusions.We identified several key determinants that physicians believe influence whether and when they practice hand hygiene at work. These beliefs identify potential individual, team, and organization targets for behavior change interventions to improve physician hand hygiene compliance.

This study developed and evaluated a puppet interview that allows children to self-report on temperamental constructs. Structured child self-report measures are rarely utilised in clinical assessment of young children under the age of 7—8 years. Given that clinical assessment is often characterised by low convergence between raters, such a measure may offer important contributions. The present study developed and evaluated a measure based on items from the Colorado Childhood Temperament Inventory and reports two studies with child participants aged 4 to 5 years. Independent observations of the children were also made. Results showed moderate levels of internal consistency and stability, and convergence between child self-report and teacher/parent raters was low, but similar levels of agreement were achieved between adult informants. The puppet interview thus showed some potential but highlighted the difficulties of self-report in young children within a multiple informant framework in clinical assessment.

This article examines the prevalence and prediction of internalising and externalising disorders from temperament and parenting in children aged 4 to 6 years (N = 491). Children were assessed via parent and teacher reports over 14 months and clinical interviews with parents were included at follow-up, along with parent and teacher reports of behavioural and emotional difficulties. Prevalence rates for internalising disorders (8.7%) were higher than for externalising disorders (5.8%), and internalising rates were similar to that found for older children in the same city. Accuracy of prediction of disorders, as well as overall behavioural and emotional difficulties, was low to moderate, and externalising problems were better predicted than internalising problems. The results highlight that while psychological disorders can be predicted from measures of temperament in infancy, the accuracy is too low to recommend these children receive selective prevention and treatment programs.
