Psychological and demographic characteristics of 368 patients with dissociative seizures: data from the CODES cohort

Background
We examined demographic, clinical, and psychological characteristics of a large cohort (n = 368) of adults with dissociative seizures (DS) recruited to the CODES randomised controlled trial (RCT) and explored differences associated with age at onset of DS, gender, and DS semiology.
MethodsPrior to randomisation within the CODES RCT, we collected demographic and clinical data on 368 participants. We assessed psychiatric comorbidity using the Mini-International Neuropsychiatric Interview (M.I.N.I.) and a screening measure of personality disorder and measured anxiety, depression, psychological distress, somatic symptom burden, emotional expression, functional impact of DS, avoidance behaviour, and quality of life. We undertook comparisons based on reported age at DS onset (<40 v. ⩾40), gender (male v. female), and DS semiology (predominantly hyperkinetic v. hypokinetic).
ResultsOur cohort was predominantly female (72%) and characterised by high levels of socio-economic deprivation. Two-thirds had predominantly hyperkinetic DS. Of the total, 69% had ⩾1 comorbid M.I.N.I. diagnosis (median number = 2), with agoraphobia being the most common concurrent diagnosis. Clinical levels of distress were reported by 86% and characteristics associated with maladaptive personality traits by 60%. Moderate-to-severe functional impairment, high levels of somatic symptoms, and impaired quality of life were also reported. Women had a younger age at DS onset than men.
ConclusionsOur study highlights the burden of psychopathology and socio-economic deprivation in a large, heterogeneous cohort of patients with DS. The lack of clear differences based on gender, DS semiology and age at onset suggests these factors do not add substantially to the heterogeneity of the cohort.
How Political Contestation Over Judicial Nominations Polarizes Americans’ Attitudes Toward the Supreme Court

Contemporary US Supreme Court nominations are unavoidably and inevitably political. Although observers worry that political contestation over nominations undermines support for qualified nominees and threatens the Court's legitimacy, there is little empirical evidence to support these claims. The authors argue that political contestation over judicial nominations provides cues that shape the public's impressions about nominees and the Court and polarizes public opinion across partisan lines. Data from a conjoint experiment administered in the first days of the Trump presidency support this argument. Political rhetoric attributed to President Trump and Senate Democrats substantially polarized partisans’ views of nominees and evaluations of the Court's legitimacy, with Republicans (Democrats) expressing significantly more (less) favorable attitudes. Additional analyses suggest that contestation generates divergent partisan responses by affecting views about the nominee's impartiality. These findings challenge existing perspectives that depict attitudes toward the judiciary as resistant to partisan considerations and have important implications for the Court's legitimacy in a polarized era.

Though the demographic characteristics of judicial nominees in the United States have gained increased political attention in recent years, relatively little is known about how they affect public opinion toward judicial nominees and courts. We evaluate these relationships in the context of race and gender using a conjoint experiment conducted during a recent vacancy on the U.S. Supreme Court. We find consistent evidence that Americans are more supportive of coracial nominees, particularly among white Republicans and Black Democrats, but no evidence of a similar effect on the basis of gender. Our results have important implications for theories of descriptive representation and suggest limits to its use as a means for generating political support for judicial nominees.

Objective
Clinical observations indicate that patients with advanced cancer and depression report higher symptom burden than nondepressed patients. This is rarely examined empirically. Study aim was to investigate the association between self-reported depression disorder (DD) and symptoms in patients with advanced cancer controlled for prognostic factors.
MethodThe sample included 935 patients, mean age 62, 52% males, from an international multicentre observational study (European Palliative Care Research Collaborative – Computerised Symptom Assessment and Classification of Pain, Depression and Physical Function). DD was assessed by the Patient Health Questionnaire-9 and scored with Diagnostic and Statistical Manual of Mental Disorder-5 algorithm for major depressive disorder, excluding somatic symptoms. Symptom burden was assessed by summing scores on somatic Edmonton Symptom Assessment Scale (ESAS) symptoms, excluding depression, anxiety, and well-being. Item-by-item scores and symptom burden of those with and without DD were compared using nonparametric Mann-Whitney U tests. The relative importance of sociodemographic, medical, and prognostic factors and DD in predicting symptom burden was assessed by hierarchical, multiple regression analyses.
ResultPatients with DD reported significantly higher scores on ESAS items and a twofold higher symptom burden compared with those without. Factors associated with higher symptom burden were as follows. Diagnosis: lung (β = 0.15, p < 0.001) or breast cancer (β = 0.08, p < 0.05); poorer prognosis: high C-reactive protein (β = 0.08, p < 0.05), lower Karnofsky Performance Status (β = −0.14, p < 0.001), and greater weight loss (β = −0.15, p < 0.001); taking opioids (β = 0.11, p < 0.01); and having DD (β = 0.23, p < 0.001). The full model explained 18% of the variance in symptom burden. DD explained 4.4% over and above that explained by all the other variables.
Significance of resultsDepression in patients with advanced cancer is associated with higher symptom burden. These results encourage improved routines for identifying and treating those suffering from depression.

In the Southern Great Plains, producers of hard red winter wheat seek sustainable methods for controlling cheat and improving economic returns. Experiments were conducted at two sites in north-central Oklahoma to determine the effect of cheat management programs, with various weed control strategies, on cheat densities and total net returns. The cheat management programs, initiated following harvest of winter wheat, included conventionally tilled, double-crop grain sorghum (Sorghum bicolor L.) followed by soybean (Glycine max L.); and continuous winter wheat. Rotating out of winter wheat for one growing season increased yield of succedent wheat up to 32% and 42% at Billings and Ponca City, respectively. Dockage due to cheat in the succedent wheat was reduced up to 78% and 87% by rotating out of winter wheat for one growing season at Billings and Ponca City, respectively. Cheat management programs including a crop rotation with herbicides applied to the grain sorghum, except for an application of atrazine + metolachlor at Ponca City, improved total net returns over the nontreated continuous wheat option. Cheat panicles in the succedent wheat were reduced up to 87% by rotation out of winter wheat for one growing season.

At the Savannah River Site (SRS) we are currently finalizing the design for a multi-system vitrification process that will be installed in the F-Canyon Multi-Purpose Process Facility (MPPF), an existing highly shielded, remotely operated facility. Authorization to proceed beyond the preliminary design based on the recommendation of a Formal Design Review Board was requested in May of 1999.
The Savannah River Technology Center (SRTC) Process Development Group has been conducting research and developing a process to identify equipment design bases and process operating parameters since 1996. The goal of the project is to stabilize a tank of ∼11,000 liters of nitric acid solution containing valuable isotopes of americium (Am) and curium (Cm). Vitrification has been selected as the most attractive alternative for stabilization and provides the opportunity for recovery and eventual reuse of the actinides. The final glass form will be placed in interim storage awaiting a disposition by the Department of Energy. This paper presents a brief history of the stabilization program and an overview of the entire Am/Cm stabilization process. This paper also provides details of a specific processing issue related to drain tube pluggage (devitrification) that was encountered during the development of the baseline batch vitrification process, and the remedy employed to reduce the potential for further drain tube pluggage.
A Comparison of Various NLTE Codes in Computing the Charge-State Populations of an Argon Plasma

A comparison among nine computer codes shows surprisingly large differences where it had been believed that the computational physics was well understood. The codes simulate a plasma that is in steady state but not in local thermodynamic equilibrium (NLTE codes). In this study each code treats an “easy” problem, which is an argon plasma, optically thin and with no external photon flux; densities are varied from near-coronal to an intermediate 1021 electrons/cc and above. The temperatures are high enough that most ions have two or fewer bound electrons, which for this plasma means temperatures above about 300 eV.
The present study asks only if the codes compute similar charge-state populations (a surprising “no”), and, if not, why not. It does not claim accuracy for any code either by comparison to experiment or by appeal to a concensus.
