Objective:
Assessment of performance validity during neuropsychology evaluation is essential to reliably interpret cognitive test scores. Studies (Webber et al., 2018; Wisdom, et al., 2012) have validated the use of abbreviated measures, such as Trial 1 (T1) of the Test of Memory Malingering (TOMM), to detect invalid performance. Only one study (Bauer et al., 2007) known to these authors has examined the utility of Green’s Word Memory Test (WMT) immediate recall (IR) as a screening tool for invalid performance. This study explores WMT IR as an independent indicator of performance validity in a mild TBI (mTBI) veteran population.
Participants and Methods:Participants included 211 (Mage = 32.1, SD = 7.4; Medu = 13.1, SD = 1.64; 94.8% male; 67.8% White) OEF/OIF/OND veterans with a history of mTBI who participated in a comprehensive neuropsychological evaluation at one of five participating VA Medical Centers. Performance validity was assessed using validated cut scores from the following measures: WMT IR and delayed recall (DR); TOMM T1; WAIS-IV reliable digit span; CVLT-II forced choice raw score; Wisconsin Card Sorting Test failure to maintain set; and the Rey Memory for Fifteen Items test, combo score. Sensitivity and specificity were calculated for each IR score compared with failure on DR. In addition, sensitivity and specificity were calculated for each WMT IR score compared to failure of at least one additional performance validity measure (excluding DR), two or more measures, and three or more measures, respectively.
Results:Results indicated that 46.8% participants failed to meet cut offs for adequate performance validity based on the standard WMT IR cut score (i.e., 82.5%; M = 81.8%, SD = 17.7%); however, 50.2% participants failed to meet criteria based on the standard WMT DR cut score (M = 79.8% SD = 18.6%). A cut score of 82.5% or below on WMT IR correctly identified 82.4% (i.e., sensitivity) of subjects who performed below cut score on DR, with a specificity of 94.2%. Examination of IR cutoffs compared to failure of one or more other PVTs revealed that the standardized cut score of 82.5% or below had a sensitivity of 78.2% and a specificity of 72.4%; whereas, a cut score of 65% or below had a sensitivity of 41% and a specificity of 91.3%. Similarly, examination of IR cutoffs compared to failure of two or more additional PVTs revealed that the cut score of 60% or below had a sensitivity of 45.7% and specificity of 93.1% ; whereas, a cut score of 57.5% or below had a sensitivity of 57.9% and specificity of 90.9% when using failure of three or more PVTs as the criterion.
Conclusions:Results indicated that a cut score of 82.5% or below on WMT IR may be sufficient to detect invalid performance when considering WMT DR as criterion. Furthermore, WMT IR alone, with adjustments to cut scores, appears to be a reasonable way to assess symptom validity compared to other PVTs. Sensitivity and specificity of WMT IR scores may have been adversely impacted by lower sensitivity of other PVTs to independently identify invalid performance.

Infection prevention program leaders report frequent use of criteria to distinguish recently recovered coronavirus disease 2019 (COVID-19) cases from actively infectious cases when incidentally positive asymptomatic patients were identified on routine severe acute respiratory coronavirus virus 2 (SARS-CoV-2) polymerase chain reaction (PCR) testing. Guidance on appropriate interpretation of high-sensitivity molecular tests can prevent harm from unnecessary precautions that delay admission and impede medical care.

In order to maximize the utility of future studies of trilobite ontogeny, we propose a set of standard practices that relate to the collection, nomenclature, description, depiction, and interpretation of ontogenetic series inferred from articulated specimens belonging to individual species. In some cases, these suggestions may also apply to ontogenetic studies of other fossilized taxa.

Surgical site infections (SSIs) are among the most common healthcare-associated infections in low- and middle-income countries. To encourage establishment of actionable and standardized SSI surveillance in these countries, we propose simplified surveillance case definitions. Here, we use NHSN reports to explore concordance of these simplified definitions to NHSN as ‘reference standard.’

Objective
Characteristics associated with interventions and barriers that influence health care workers’ willingness to report for duty during an influenza pandemic were identified. Additionally, this study examined whether workers who live in proximal geographic regions shared the same barriers and would respond to the same interventions.
MethodsHospital employees (n=2965) recorded changes in willingness to work during an influenza pandemic on the basis of interventions aimed at mitigating barriers. Distance from work, hospital type, job role, and family composition were examined by clustering the effects of barriers from reporting for duty and region of residence.
ResultsAcross all workers, providing protection for the family was the greatest motivator for willingness to work during a pandemic. Respondents who expressed the same barriers and lived nearby shared common responses in their willingness to work. Younger employees and clinical support staff were more receptive to interventions. Increasing distance from home to work was significantly associated with a greater likelihood to report to work for employees who received time off.
ConclusionsHospital administrators should consider the implications of barriers and areas of residence on the disaster response capacity of their workforce. Our findings underscore communication and development of preparedness plans to improve the resilience of hospital workers to mitigate absenteeism (Disaster Med Public Health Preparedness. 2015;9:175-185).

Incidence Trends in Pathogen-Specific Central Line–Associated Bloodstream Infections in US Intensive Care Units, 1990–2010

Objective.
To quantify historical trends in rates of central line-associated bloodstream infections (CLABSIs) in US intensive care units (ICUs) caused by major pathogen groups, including Candida spp., Enterococcus spp., specified gram-negative rods, and Staphylococcus aureus.
Design.Active surveillance in a cohort of participating ICUs through the Centers for Disease Control and Prevention, the National Nosocomial Infections Surveillance system during 1990–2004, and the National Healthcare Safety Network during 2006–2010.
Setting.ICUS.
Participants.Patients who were admitted to participating ICUs.
Results.The CLABSI incidence density rate for S. aureus decreased annually starting in 2002 and remained lower than for other pathogen groups. Since 2006, the annual decrease for S. aureus CLABSIs in nonpediatric ICU types was −18.3% (95% confidence interval [CI], −20.8% to −15.8%), whereas the incidence density rate for S. aureus among pediatric ICUs did not change. The annual decrease for all ICUs combined since 2006 was −17.8% (95% CI, −19.4% to −16.1%) for Enterococcus spp., −16.4% (95% CI, −18.2% to −14.7%) for gram-negative rods, and −13.5% (95% CI, −15.4% to −11.5%) for Candida spp.
Conclusions.Patterns of ICU CLABSI incidence density rates among major pathogen groups have changed considerably during recent decades. CLABSI incidence declined steeply since 2006, except for CLABSI due to S. aureus in pediatric ICUs. There is a need to better understand CLABSIs that still do occur, on the basis of microbiological and patient characteristics. New prevention approaches may be needed in addition to central line insertion and maintenance practices.

Electron-beam-induced deposition (EBID) using gas-phase precursor molecules is an extensively studied fabrication technique. Liquid-phase metal deposition has recently been shown to achieve higher purity levels than traditional gas-phase deposition [1]. The goal of this investigation was to characterize liquid-phase silver deposition for further studies in photonics. A Scanning Electron Microscope (SEM) (FEI Nova 600 NanoLab Dual Beam) was used to deposit silver on polyimide membranes from aqueous AgNO3 solution by accelerating electrons into the solution for silver ion reduction. Atomic Force Microscopy (AFM) and SEM were subsequently used to characterize the size dependence to electron dosage. We observed granular silver deposits with sub-75 nm particle size and 200-250 nm total aggregate diameters. The CASINO (monte CArlo SImulation of electroN trajectory in sOlids) program was used to model electron trajectory in the solution to relate the size to the electron spread.

Mechanoreceptive field and response properties of nociceptive neurons in ventral posteromedial thalamic nucleus of the rat

The somatotopic organization, mechanoreceptive field (RF) and response properties of nociceptive neurons in the ventral posteromedial thalamic nucleus (VPM) were studied in urethane/α-chloralose anesthetized rats. Microelectrode recordings were made of the activity of single neurons within VPM, and neurons were identified as either nociceptive [wide-dynamic-range (WDR), nociceptive-specific (NS)] or non-nociceptive [low-threshold mechanoreceptive (LTM)]. Of the 350 neurons tested, 317 responded to orofacial mechanical stimulation; all the RFs were contralateral. The incidence of NS neurons was significantly higher than that of WDR neurons (21.2% versus 4.2%) but LTM neurons were most common (62.9%). WDR and NS neurons were scattered among the LTM neurons in the VPM. NS neurons were significantly denser in rostral and caudal portions of VPM; most of the WDR neurons (87%) were located in the core of VPM. Neurons with an ophthalmic division RF were located most dorsally and those with a mandibular division RF most ventrally. The RF of most neurons (77%) was located in the maxillary region, with the RF of nociceptive neurons mainly in the intraoral region and that of most LTM neurons in the perioral region. The majority of LTM, WDR and NS neurons tested had a low spontaneous activity and there was a significant difference in spontaneous firing between WDR and NS neurons, and between WDR and LTM neurons. The mean mechanical activation threshold of NS neurons (195 ± 11.2 g) was significantly higher than that of WDR neurons (4.7 ± 1.3 g). Response magnitude and peak firing frequency to graded mechanical stimuli were significantly higher in the WDR neurons. These findings indicate significant differences in the spontaneous activity, RF and response properties between WDR and NS neurons in VPM. However, they also indicate that both types of nociceptive neurons can encode the spatial properties and intensity of a noxious orofacial stimulus and, thus, might contribute to the sensory-discriminative component of pain.
