Over-expansion of right ventricle to pulmonary artery conduits during transcatheter pulmonary valve placement

Objectives:
To determine the safety and feasibility of over-expansion of right ventricle to pulmonary artery conduits during transcatheter pulmonary valve placement.
Background:Transcatheter pulmonary valve placement is an alternative to surgical pulmonary valve replacement. Traditionally, it was thought to be unsafe to expand a conduit to >110% of its original size.
Methods:This retrospective cohort study from two centers includes patients with right ventricle to pulmonary artery conduits with attempted transcatheter pulmonary valve placement from 2010 to 2017. Demographic, procedural, echocardiographic and follow-up data, and complications were evaluated in control and overdilation (to >110% original conduit size) groups.
Results:One hundred and seventy-two patients (51 overdilation and 121 control) had attempted transcatheter pulmonary valve placement (98% successful). The overdilation group was younger (11.2 versus 16.7 years, p < 0.001) with smaller conduits (15 versus 22 mm, p < 0.001); however, the final valve size was not significantly different (19.7 versus 20.2 mm, p = 0.2). Baseline peak echocardiographic gradient was no different (51.8 versus 55.6 mmHg, p = 0.3). Procedural complications were more frequent in overdilation (18%) than control (7%) groups (most successfully addressed during the procedure). One patient from each group required urgent surgical intervention, with no procedural mortality. Follow-up echocardiographic peak gradients were similar (24.1 versus 26 mmHg, p = 0.5).
Conclusions:Over-expansion of right ventricle to pulmonary artery conduits during transcatheter pulmonary valve placement can be performed successfully. Procedural complications are more frequent with conduit overdilation, but there was no difference in the rate of life-threatening complications. There was no difference in valve function at most recent follow-up, and no difference in rate of reintervention. The long-term outcomes of transcatheter pulmonary valve placement with conduit over-expansion requires further study.

Summary
The pipeline metaphor used to characterize dwindling interest in science and STEM-related careers has gradually been replaced by alternative models that convey complex pathways into, through and out of science by young men and women. In this chapter, we review literatures from educational psychology, cognitive development, and science education and present our own mixed methods approach to developing a model of the roles that children, parents and teachers play in launching, supporting, and sustaining pathways to science interest from early childhood to the transition to college. We use our longitudinal data to describe cases that illustrate these critical developmental inflection points. These rich cases illustrate the advantages of using qualitative methods, when possible, to augment developmental models derived from more quantitative approaches depicted through path diagrams, phase models, or Sankey diagrams. The cases discussed highlight critical roles that parents and teachers might play in nurturing science interests among males and females. Implications for future research and suggestions for practice are considered.
Development, in fact, may be viewed best as a set of multiple developmental trajectories, and our task as developmentalists is to discover how the interplay between different trajectories of children and adults accounts for outcomes.

Coenzyme Q10 (CoQ10) is a powerful antioxidant that buffers the potential adverse consequences of free radicals produced during oxidative phosphorylation in the inner mitochondrial membrane. Oxidative stress, resulting in glutathione loss and oxidative DNA and protein damage, has been implicated in many neurodegenerative disorders, including Alzheimer's disease, Parkinson's disease, and Huntington's disease. Experimental studies in animal models suggest that CoQ10 may protect against neuronal damage that is produced by ischemia, atherosclerosis and toxic injury. Though most have tended to be pilot studies, there are published preliminary clinical trials showing that CoQ10 may offer promise in many brain disorders. For example, a 16-month randomized, placebo-controlled pilot trial in 80 subjects with mild Parkinson's disease found significant benefits for oral CoQ10 1,200 mg/day to slow functional deterioration. However, to date, there are no published clinical trials of CoQ10 in Alzheimer's disease. Available data suggests that oral CoQ10 seems to be relatively safe and tolerated across the range of 300–2,400 mg/day. Randomized controlled trials are warranted to confirm CoQ10's safety and promise as a clinically effective neuroprotectant.
Cannabis use and first-episode psychosis: relationship with manic and psychotic symptoms, and with age at presentation

Background
Cannabis use has been reported to be associated with an earlier onset of symptoms in patients with first-episode psychosis, and a worse outcome in those who continue to take cannabis. In general, studies have concentrated on symptoms of psychosis rather than mania. In this study, using a longitudinal design in a large naturalistic cohort of patients with first-episode psychosis, we investigated the relationship between cannabis use, age of presentation to services, daily functioning, and positive, negative and manic symptoms.
MethodClinical data on 502 patients with first-episode psychosis were collected using the MiData audit database from seven London-based Early Intervention in psychosis teams. Individuals were assessed at two time points – at entry to the service and after 1 year. On each occasion, the Positive and Negative Syndrome Scale, Young Mania Rating Scale and Global Assessment of Functioning Scale disability subscale were rated. At both time points, the use of cannabis and other drugs of abuse in the 6 months preceding each assessment was recorded.
ResultsLevel of cannabis use was associated with a younger age at presentation, and manic symptoms and conceptual disorganization, but not with delusions, hallucinations, negative symptoms or daily functioning. Cannabis users who reduced or stopped their use following contact with services had the greatest improvement in symptoms at 1 year compared with continued users and non-users. Continued users remained more symptomatic than non-users at follow-up.
ConclusionsEffective interventions for reducing cannabis use may yield significant health benefits for patients with first-episode psychosis.

Background
Ethnic variations have previously been identified in the duration of untreated psychosis (DUP) and pathways into psychiatric services. These have not been examined in the context of early intervention services, which may alter these trajectories.
AimsTo explore ethnic differences in the nature and duration of pathways into early intervention services.
MethodIn a naturalistic cohort study, data were collected for 1024 individuals with psychotic disorders accepted for case management by eight London early intervention services.
ResultsDuration of untreated psychosis was prolonged in the White British group compared with most other ethnic groups. White British individuals were more likely to make contact with their general practitioner and less likely to be seen within emergency medical services. All Black patient groups were more likely than their White British counterparts to experience involvement of criminal justice agencies.
ConclusionsVariations continue to exist in how and when individuals from different ethnic groups access early intervention services. These may account for disparities in DUP.

Lethality of intestinal tissue extracts from Eimeria-infected chickens

Evidence exists that lethal activity is associated with extracts from Eimeria tenella-infected caecal tissues. We examined tissues of the small intestine infected with other species of Eimeria for a similar lethal activity. Extracts were prepared from intestinal tissues of chickens infected with either E. acervulina, E. brunetti, E. maxima, E. mitis or E. necatrix. Appropriate control extracts from tissues of non-infected chickens were also prepared. Intravenous injection of the extracts into healthy chickens showed that lethality of infected tissue extracts was higher than control extracts and was independent of the site of infection within the intestine. Comparison of the lethality of caecal extracts from chickens infected with embryo-adapted or parent lines of E. tenella showed that lethality was independent of the recognized pathogenic characteristics of the lines. Also, it was demonstrated that lethal activity is associated with extracts of tissues containing massive numbers of arrested and non-arrested sporozoites and early trophozoites.

Histomoniasis was transmitted to poults inoculated orally with recently harvested whole intact male worms in three trials (4/5, 5/6, 2/4), but not with female worms (0/5, 0/9, 0/3). When triturated male worms were given in two trials, the rate of transmission was reduced (3/5, 0/10). Triturated female worms given orally produced histomoniasis in 1/9 poults. These results suggest that further unknown steps may be involved in histomonad transmission by heterakid worms.

INTRODUCTION
Explorations at sea differ from those on land. A ship at sea is a self-contained community that must provide all the services needed by those aboard, including medical care. Though most expeditions actually require relatively few medical services, proper planning is necessary to ensure that the appropriate services, including emergency care, are available when needed. In addition to actual medical treatment, public health and preventive services are also essential to a safe and rewarding venture. The early planning stages of the expedition should be used to begin to identify the needed resources. The specifics will vary with the characteristics of the expedition. The success of the expedition in fulfilling its mission is often dependent upon adequately addressing these health issues prior to leaving shore.
The first section addresses the variables that determine the health care needs underway, and how those needs can be met. The next section discusses several medical conditions that are relatively common on seagoing expeditions. The last section addresses safety, public health, and personal health behaviors. Proper predeployment planning and careful attention to health concerns underway are key to a successful seagoing expedition.
PREDICTING AND ADDRESSING HEALTH CARE NEEDS AFLOAT
As the expedition leader begins the early planning process, questions are asked such as: What pharmaceuticals should we bring? and What vaccinations do we need? Unfortunately these questions don't have standard answers applicable to every expedition.

The Brahmin dialect of Tamil (Dravidian) has an unusual inventory of five distinctive liquid sounds: plain and retroflex rhotics, and plain and retroflex laterals, and a fifth liquid which has been variously described as a rhotic, a lateral, a glide and/or a fricative. This paper investigates the articulatory, acoustic, and perceptual properties of these liquids, and, in particular, the fifth liquid. Electropalatography (EPG) and static palatography were used to examine the articulatory properties of the liquids, the acoustic properties of the liquids were examined, and we tested the intelligibility of the EPG recordings in a perception experiment. Our intent is to propose a classification for the fifth liquid based on these studies. The fifth liquid is classified as a retroflex central approximant, with characteristics that make it distinct from the other liquids along three dimensions of contrast: static ∼ dynamic, central ∼ lateral and retroflex ∼ non-retroflex.
